Miner was lifting a 2.5'' steel shaft (weighing between 100-150lbs). EE picked it up to slide it into the pulley that was hanging via EE's service truck crane, and when EE curled up to slide it in, EE's left arm/elbow gave out.

Mostrando los 23Miner was shoveling under the portable plant screen due to a belt overload. The area under there has a low ceiling and is awkward. When miner was bent over shoveling, they heard/felt a pop in their back. Right hand side on the belt line.
Miner was picking up a panel by EE's drill and when EE stood up and turned EE heard a crack in EE's back.
Miner was picking up a piece of 4x4 from the ground. EE picked it up underneath with left hand, not centering the weight, and caused a bicep tendon rupture to the left arm.
At approximately 11:30am, EE (newer hire) was in a scissor lift lifting and bolting sheet steel. While in the lift, the piece of sheet steel shifted and fell, jerking EE in the process. When the falling steel jerked EE, it caused a slight displacement in EE's back and strain.
EE was standing behind a log washer lifting a small screen holding back small rocks. As EE was prying it, a rock flew up and struck EE in the mouth. This caused a left molar to completely dislodge.
Miner was using big bar on conveyor belt of rock crusher machine, rock wedged on machine, worker went to remove wedged rock, right hand slipped, smashing finger against a rock.
Lifting 8ft long chunk of steel, injured middle upper neck.
The employee was pulling a piece of removable catwalk out of an HSI and EE pivoted the wrong way and twisted right knee. The knee is swollen.
The employee was changing the screen on the feeder. The metal plate slipped off the chain and fell on EE's left foot. EE's left foot is bruised and on light duty for a week.
The employee was replacing the angle iron brace on a metal door. The employee was in a man lift and the door swung down and hit them on the head and the top of the bar on the man lift.
Climbed over feeder onto the metal lip between feeder and box and foot got stuck
While working under the bucket of a wheel loader noticed that one of the braces against the cylinder of the bucket was unstable. While resolving this, one of the braces came loose and struck EE in the left eyebrow and shoulder, causing EE to fall back into a bolt rack. This caused bruising on EE's shoulder and lower right back with some bleeding from the left eyebrow.
Foreman attempted to lift a heavy piece of plant equipment and injured right bicep.
Two Miners and one supervisor were installing 24" x 25' plastic pipes. Normally equipment is used to place pipes, but this was impossible due to the location of this specific section of pipe. Miners were physically moving the pipe into place by hand. Miner felt lower-right back and leg pain while lifting and pushing pipe into place.
Employee had spend Monday 12/12/16 working on a crushing plant, climbing up and down ladders to track conveyor belts. On Tuesday 12/13/16 EE was climbing up the quarry truck to start it. EE swung EE's leg up on the step and felt a pop in EE's back.
Installing oil pressure line on cone. Tightening hose and felt back twinge. Crawled out and told crew he hurt his back. Stayed at work until 9 am and then went home to rest because he could not move. He went to the clinic on 12/9/14. Work status off until Friday when he should be seen for a follow up appointment. Injury: back sprain/strain.
During the previous 3 days (June 16-18), EE had been working on a project pulling a lot of hose. He felt discomfort and pain from the pulling in his groin and behind the belt area. He though it might just have been indigestion, but the pain increased and was reported on 6/19/14.
Moving a board over his head, sliding it, then a big couple of popping noises came out of his arm and then it felt like something tore in his forearm.
Lifted up hatch door over the hydraulic pumps to check hose. Went over and released the pressure off hydraulic tank. Door was straight up & down jammed into postion. Went to check hoses again and leaned on the door. The door cam loose and fell open. Hand slipped and got caught on inside latch of the door.
Ee had a knife lying amongst his tools in his drill - he went to move his tools & he caught his finger on knife.
EMPLOYEE WAS SHOVELING WET SAND IN OUR SUSSEX PIT AND WENT TO TURN SIDEWAYS AND THEN FELT SOMETHING PULL IN LOWER BACK.
WORKING ON LOADER & WAS PINCHED BETWEEN LOADER TIRE & CRANE.
