Miner was helping lower a chute on the secondary plant, EE and two other employees were using come-alongs to lower the chute into place. They let the chute drop halfway down, and it landed on the Miner's right hand, fracturing it.
We were not notified of the injury until 07.22.2024. Miner was lifting a hammer out of our VSI when miner's foot slipped, and miner then pushed up to keep the hammer from falling, causing a sprain/strain to miner's right shoulder rotator cuff capsule.
Lifting 8ft long chunk of steel, injured middle upper neck.
Foreman attempted to lift a heavy piece of plant equipment and injured right bicep.
Employee was walking down a hill and turned abruptly to talk with someone behind them and states that the pivoting motion caused a "pop" in their left knee. Employee describes lost range of motion but no pain. Employee had the knee wrapped and went on light duty for the rest of the day, at which time they would go to a local clinic.
Miner was exiting skid steer, slipped, and jammed thumb into bucket of skid steer.
During the previous 3 days (June 16-18), EE had been working on a project pulling a lot of hose. He felt discomfort and pain from the pulling in his groin and behind the belt area. He though it might just have been indigestion, but the pain increased and was reported on 6/19/14.
Employee attempted to repair a piece of 3 1/2" drill steel (used as grizzly bars) that broke off the static screen. Employee failed to secure the broken drill steel after placing it in its receiving saddle. The drill steel fell from its cradle resulting in a fractured and lacerated left index finger.
At the chip stacker headpulley area, the employee was welding a wiper assembly in place. He was standing inside the transfer box. He had taken off his gloves and continued to work. As he tried to reposition himself, he grabbed both sides of the box and struck his left thumb against a burr on the top of the box. This caused him to cease work and contact his supervisor.
Group of employees reinstalling guard on Nordberg HSI Crusher Drive Sheaves. Employee got left hand in way of guard sliding into position. Caused laceration to palm - required 7 stitches.
EE states to have hurt back operating cat truck. Office notified 2/28/05 of incident. Doctor released him back to work as long as he could run a different truck. Off of work 3/1 and 3/2. Returned to work 3/3. EE left work 3/9 at noon in pain. Now off 3/10 thru 3/15/05.
Employee was sliding a steel box containing a "Port-a-Power" unit from tailgate of a pickup truck. Right 3rd finger was caught between the box and the tailgate and got pinched resulting in a laceration that required sutures.
EE WAS USING A UTILITY KNIFE TO CUT A PIECE OF CONVEYOR BELT. THE KNIFE SLIPPED FROM THE JOB & THE EE CUT HIS RIGHT THIGH, RESULTING IN STITCHES.
EMPLOYEE WAS GOING TO FILE PAPERS IN BOTTOM DRAWER OF A CABINET. AS SHE BENT OVER SHE FELT A SUDDEN AND INTENSE PAIN IN LOWER BACK. SHE WAS TRANSPORTED TO THE HOSPITAL WHERE SHE WAS TREATED WITH MEDICATION. SHE RETURNED TO WORK 7/31/00. AN MRI WILL BE GIVEN WHEN SWELLING GOES DOWN.
