A rail operator parked a fuel truck, applied the parking brake, and used three points of contact to exit the cab. Upon stepping onto flat, dry ground, the employee turned away from the vehicle and experienced sudden pain in the lower right leg and knee, resulting in restricted joint movement. EE was placed on restricted duty.

While adjusting the forks on the 6K forklift EE was pulling slide on a stuck fork that broke free back pinching EE's right ring finger between the fork and the rear support frame.

The employee was climbing up into the excavator and felt a sharp pop in the right calf. Employee immediately reported to the supervisor and the first aid process began.

Operator slammed cab door of a 980k loader onto their middle right finger. This caused a laceration to the middle finger

While using the access panel to enter the top deck screen of # 5, the metal door slammed shut onto EE's left thumb crushing and ripping the top of the skin and bleeding inside gloves.

The employee was in the process of removing a screen bearing housing from a shaft.  While attempting to get a chain mule hooked up, the housing slipped off the shaft.  The bearing housing was secured and hanging by a chain fall. The bearing housing slipped off and swung to the left and caught the employees left hand between the bearing housing and the timing gear shaft.

Employee was lowering anvils into an anvil ring and EE caught left pinky finger between the ring and an anvil. The cut required stitches.

Employee was opening a door when it fell off the hinges and smashed right thumb between the door and the handrail of the man lift the employee was in.

EE was preparing to press a new bearing into the housing for a screen. While centering the housing, EE's fingers were pinched between the housing and the frame of the press.

EE was taking blow bars off impact crusher. One of the supporting shoes came loose as it was being removed and fell. EE attempted to stop the shoe from falling and EE's hand was caught between the shoe and the metal frame of the crusher.

While removing the shiv from a 300 hp motor that assist in running a Hazemag Crusher the shiv fell off of the shaft smashing employees fingers. Employee had to have his right hand index finger stitched and his second finger splinted.

EE was lifting plastic 55-gallon drum being used as a float on a pump platform. Appx. 10-gallon of frozen water was contained inside the drum. As EE was lifting drum, it slipped and struck the EE on top of his right foot. Diagnosed with severe bruise/contusion to right foot and placed on restricted duty status.

EE was removing used 36" trough idlers. EE was sliding the idlers to his side of the truss when it slid off of the truss slipping from his left hand pinching EE's right thumb between the idler frame and truss as he was preparing to hold the other end with his right hand. The end of his thumb was crushed as the idler fell to the ground, causing an open wound.

Injured employee was feeding a measuring rope through a 6" condiut with the aid of an air compressor and mouse. He was using one hand to secure airline hose and one hand to feed rope. Airline slipped from his grip and struck him in the mouth. Injury resulted in 4 sutures outside and 4 sutures inside of lip.

Initial reported as First Aid only. On 4/2/2008 went to a Restricted Duty case and became reportable. Had been seen by two previous Doctors and released without treatment. Was installling a new deck frame on #6 screen tried to push deck into place. He felt that he may have strained a muscle in back of left shoulder.

While installing grating onto a catwalk for a new conveyor section. Injured employee had his finger pinched between the grating and angle support member of the catwalk.

EE strained his right shoulder while shoveling debris from the Tail Pulley of a conveyor.

Individual was removing the cutting edge on a bull dozer. He was attempting to hold the head of a bolt while removing the nut. The bolt spun cutting the middle finger on the right hand.

Opening the tail gate of a service truck, the gate fell and pinched the employee's right right ring finger between the tail gate and tht pintal hook of the truck's hitch. This report was orginally filed electronically on 12/21/2004. I find not record in the system. I do have a hard copy. I need to update restricted duty days.

EMPLOYEE WAS WALKING DOWN CATWALK BY 11 SCREEN PLACED LEFT HAND ON HANDRAIL, CAUGHT A BUR CAUSING LACERATION TO MIDDLE AND INDEX FINGERS OF LEFT HAND.

EE WAS SHOVELING UNDER CONVEYOR IN #6 TUNNEL REMOVING ROCK & DIRT BUILDUP, WHEN HE EXPERIENCED LOWER BACK PAIN.

EMPLOYEE WAS REMOVING CHUTE LINER FROM CHUTE ASSEMBLY. WHILE PASSING CHUTE LINER TO ANOTHER INDIVIDUAL LINER SLIPPED AND IN EFFORT TO REGAIN CONTROL OF LINER (PIECE OF STEEL) A SHARP EDGE L ACERATED EMPLOYEE'S RIGHT HAND.

INJURED REMOVED A PICE OF PIPE(SIZE APPROX 6' ROUND 2' LONG 1/2" THICK) FROM TRUCK AND PLACED ON GROUND.WHEN HE PICKED UP PIPE FROM GROUND HE FELT SHARP PAIN IN HIS LOWER BACK.

EE STRAINED HIS ABDOMEN AREA WHILE LIFTING AND PULLING PORTABLE WATER PUMPS INTO POSITION.

LIFTING SCREEN CLOTH OVER HIS HEAD TO PLACE ON SCREEN, HE LOST CONTROL OF IT CAUSING AN ACUTE DISLOCATION OF HIS RIGHT SHOULDER.

HH WAS INSTALLING A CUTTING EDGE ON A 992 LOADER WHEN LIFTING THE EDGE HE STRAINED/SPRAINED HIS BACK.

HH WAS HANGING UP PAYROLL DATABINDERS, FROM THE SWINGING MOTION INVOLVED IN LIFTING THE HEAVY BINDER UP TO THE TOP SHELF OF A LATERAL FILE CABINET, SHE EXPERIENCED A SPRAIN IN HER NECK/SHOULDE R AREA.

EMPL WAS SHOVELING WASTE ON BELT WHEN SHARP PAIN OCCURED IN BACK.

HE WAS LOADING, BY HAND, END-BIT (CUTTING EDGE FOR 980 LOADER) ONTO PICK-UP TRUCK BETWEEN TAIL GATE AND TUMBLER. IN DOING SO HE FELT A SHARP PAIN IN HIS BACK.

EE WAS HELPING INSTALL RETURN GUIDE IDLER. WHILE PULLING ON IDLER FRAME, FRAME MOVED SUDDENLY STRIKING EE ABOVE RIGHT EYE AND CUTTING EE ABOVE EYEBROW.

WHILE EE WAS INSTALLING SCREEN CLOTH. 2 ND EMPLOYEE PULLED SCREEN CLOTH, CAUSING 1 ST EE TO GET RIGHT THUMB CAUGHT BETWEEN SCREEN CLOTH AND SIDE PLATE OF SHAKER UNIT.
