Injured was cleaning out under some conveyor belts using a water hose when water deflected onto injured's face an into right eye. Injured was wearing safety glasses at the time. Injured went to Dr. on 10/19/21, diagnosed with a corneal abrasion.

Employee stated EE was climbing into a screen deck to replace a screen when EE cut hand on a sharp rock.

Employee stated was installing a V belt, rolled finger between the sleeve and the v belt.

Employee was troubleshooting a flow problem on the belt press. When EE pulled a hose off, the fluid sprayed EE in the face. Employee states EE knocked safety glasses off when the fluid startled EE.

The employee was changing a screen and struck EE's elbow on a beam.

Employee stated EE grabbed the handle of a tool being worked on by another employee and burned EE's hand, unaware the handle was hot.

The employee was changing a screen when he turned and felt a pain in his back.

The employee was working on the belt press checking a water line, when he inadvertently pulled the line loose causing water to splash in his eye. No injury, antibiotic was prescribed as preventive care.

The employee stated his eye started hurting following his shift at work. Went to the doctor who indicated it is possible he got sand under his contact lense.

While an IMTR employee was unloading a drill steel from a delivery truck, the forks of the lull hung up on a cross member, when it released it pushed the drill steel onto the Fred Weber Inc. truck driver's foot.

Employee was removing a pin from a hydraulic cylinder when the cylinder fell on his thumb.

The employee was disassembling a take up, when a roller fell on his foot above the steel toe.

The employee was shoveling and experienced groin pain. It was initially diagnosed as a personal health condition. A medical specialist later determined that the condition may have been caused by heavy lifting or straining.

Employee was working under the crusher chipping welding slag and fines fell from above and went behind safety glasses into his right eye.

EE was taking a wear plate off a loader bucket, when his right hand slipped and cut his right thumb.

WHILE REPLACING SCREEN ON THE CRUSHER, A METAL SCREEN PANEL FELL OVER AND STRUCK EMPLOYEE'S RIGHT ELBOW, CAUSING CONTUSION.
