While removing slide above screw conveyor material inside the hopper moved and dust came out of the gap left from the slide removal and got into employees eyes

EE was attempting to unplug a triple gate at the coal mill with no success. EE then introduced EE's hand into the triple gate. The gate activated, nipping the tip of EE's right middle finger.
While changing rolls employee had a piece of clinker dust get behind EE's safety glasses and in EE's eye. EE was given antibiotic eye drops after removal of particle on 03/03/2025
EE was helping a coworker install a snub roller scraper on A20 belt. EE lifted the scraper with both hands and felt a pain in right bicep. The scraper is approx. 60" long and weighs approx. 50 lbs. This became reportable when the doctor placed EE on restrictions on 12/24/24.
As employee was installing bolt on the jack of the raw mill. As the employee began installing bolts the padeye gave way. Causing the rams head to fall back down onto the employees right index, middle, and ring fingers. Crushing the tips of the fingers. The employee was taken to the hospital, and the tips of each finger were severed between the last knuckle and the nail bed.
EE was installing new dust collector bags in one of the old dust collectors. In order to properly install, EE had to push upwards above their head and twist at the same time. While doing this EE experienced a sharp pain in the middle of EE's back. The employee visited Urgent Care. The doctor gave EE restrictions.
EE was diagnosing problem with a blower. They locked/tagged/tried out the blower and was wearing gloves. They tried to maneuver the pulley manually. As they did the tip of their left thumb was pinched between the belt and pulley. They received a small fracture at the very tip of their thumb and was given a splint for the thumb.
EE was installing a takeup pulley on clinker feeder belt. It was placed on upper part of mobile work platform. When they & partner were attempting to slide it on platform to get it in place it started to slide off platform. EE reacted by trying stop the pulley and got a finger between pulley and grating of the platform causing a small fracture at the very tip right ring finger.
An employee was in the process of moving some fairly large steel cooler parts (beams) from the forklift forks onto some grating. In the process initial indications are that the beam slid, and EE's finger got caught between two solid objects causing the injury. Cut finger and fracture to finger tip.
EE smashed finger between troughing roller frame and conveyor frame while installing new troughing rollers resulting in 5th digit distal fracture.
Employee was working on a flat platform for easy access to door closure. They felt the door was loose and did not relieve pressure from the unit. The pivot arm swung once they released the connecting pin. The arm lacerated the employee's ear.
EE was pulling guzzler hose out of area, movement of house and windy conditions blew dust into the left inside corner of eye. EE rubbed their eye causing a corneal abrasion.
EE was disconnecting a water spray lance when steamed was released from line causing burns to hands and wrist
Employee was switching a valve and finger got smashed between valve handle and cement line causing a fracture to finger
The employee was attempting to remove a metal valve seal from a valve at the bottom of a rail car. As employee pulled on the metal seal, in attempt to break the metal seal, employee cut the inside of left middle finger that required professional medical treatment.
Employee was in the drum process room opening and processing drums and received a cut on their right forearm from the drum lid.
EE was helping to change out dust collector cartridges when dust or fiber from dust cartridge entered into the top of employees safety glasses causing a scratch on employee's eyeball
WHILE EMPLOYEE WAS REINSTALLING THE LIDS ON A SCREW, THE BUELLS ABOVE LET GO AND DUMPED HOT DUST WHICH SPLASHED ON THE EMPLOYEE. THIS KNOCKED EE DOWN, AND EE CRAWLED OUT OF THE AREA. EE RECIEVED 3RD DEGREE BURNS ON SEVERAL PLACES ON EE'S BODY.
While moving 515V tripper employee got foot in pinch point between roller and rail.
While installing guard on drag chain sprocket employee was stomping the guard into place and foot went through the guard causing a contusion on the right shin.
While opening a door a gust of wind swept up debris and something got into right eye. Flushed eye and went back to work. Continued to have redness but no discomfort. Sent to company eye doctor on June 22 for examination where they realized a piece of debris on the eye that needed removal.
Employee states that EE needed to get down on EE's hands and knees to hand a co-worker a grease fitting below. Employee states that when EE got down on EE's hands and knees EE felt a pull in EE's abdomen. The doctor diagnosed EE with a work related hernia on 6/23/22, related to this incident.
The employee states that EE was working on a double dump that was sitting on a pallet in front of the maintenance shop. EE flipped the double dump so EE could get to the bolts on the other side. When EE did EE felt a pain the EE's left forearm area. Employee was diagnosed with an elbow strain. This became reportable on 11/22/22 when professional medical treatment was needed.
Miner was trying to get silo to feed and cement eventually began to flow at a fast rate and got in the right eye of the employee.
Miner was under feeder weight belt holding it up as it was being replaced when material fell in miner's face. As miner jerked head to the side material went in eye
Employee stated that they were greasing the bearing on the lower valve of KC0742 double dump while standing on the walkway of KC0924-1 wheelabrator screw. Employee said they pinched their arm between the counterweight and the framework of the walkway above.
EE was lifting a CO2 tank onto curb and felt a pop in abdomen.
Employee was placing a barricade around some I beams that had been removed when EE walked into the end of one of them that had a sharp edge and cut leg just above EE's boot.
The employee was screwing a cap on a glass vial when the glass vial broke in employee's hand causing a laceration to the employee's finger on left hand.
Employee was working in truck bay when they felt something get in eye and start irritating it. Employee rubbed the eye but it was still irritated. Employee then flushed it with eye wash solution but this didn't help much. The employee then went to the doctor.
The employee states that EE was getting ready to pump a truck when EE went to the pump and valve to backflush a truck. While operating the valve EE bent down to pull the valve open with EE's left arm and EE felt a strain in EE's left shoulder. The employee went to the doctor on 4/20/2021 and it became reportable.
Employee was pushing the auger bit into the chute out of the walkway. It was not noticed immediately until exiting the manlift later in the day.
EE strained elbow while picking up a medium sized nitrogen cylinder
Employee was removing welding lead from a container. EE was pulling it out from storage and setting on a pallet for relocation to the work area. Did not feel well, when reporting to work the next day said had soreness in groin. Took EE to clinic for evaluation and diagnosed as groin strain
EE was cleaning material out from underneath a conveyor belt with a shovel and felt a sharp pain in lower back.
Employee had been lifting, pushing and handling mill liners during a repair job. EE's back felt sore so EE reported the injury but continued to work. One week later it started hurting again. EE was sent to the doctor and placed on restrictions for a back strain. Placed on restrictions after completing the 10/28/2020 shift.
The employee took EE's goggles off to put EE's safety glasses on and some dust that was on EE's goggles got into EE's left eye.
Employee stated bumped right knee while working on the screen at the Secondary Crushing system. Employee was diagnosed with a fracture on 11-6-2020 and placed on restrictions on 11-9-2020.
Employee felt a sharp pain in EE's lower back when attempting to lift a full bottle of acetylene onto a welding skid by self.
Employee was struck in the hand while loading mill balls into the #3 Finish Mill. EE fractured EE's left index finger.
Employee was helping someone slide a refrigerator out from the wall to clean behind it in the lunchroom. While doing this the employee felt EE's left shoulder come out of place.
Employee states: picked up a 2.5 gallon container of oil and was pouring the oil into the discharge elevator on the #5 mill chain guard. Employee felt a spasm in lower left abdomen. The doctor diagnosed employee with a work related hernia on 4/20/21, related to this incident.
Employee had just finished cleaning the riser duct and lifted face shield and dust fell from the frame of it behind employee's safety glasses into eye.
EE was opening a rail car lid. When EE pulled up on the handle to open the lid, EE felt a strain in groin.
EE was removing a filter from a filter cart. While pulling up on the filter, EE felt a pop in EE's left shoulder.
Employee injured right shoulder on 3-15-2020. Employee was terminated from employment on 5-9-2020 for reasons unrelated to the injury. During the course of EE's employment, the injury did not meet MSHA's part 50 reporting requirements. However, after termination the employee will be receiving surgery on the right shoulder on 7-10-2020.
EE removed check valve from compressor set it up on elevated platform (~2 feet off of ground). The check valve fell off and hit EE's left foot above the steel toe of the boot.
