Employee stated that he was cleaning up the pad and side walk outside the mine office. He stated that he was picking up small roll of belt and felt a sharp pain in his lower back. He was transported to the hospital and was treated for a pulled muscle and became a lost time accident.

Employee stated that he and the section mechanic were lifting the miner cable up to hang it across an intersection when he felt a sharp pain in his right shoulder. Employee continued to work until 12-18-2013 when he had surgery to the shoulder.

Employee was helping move a booster pump when he felt a sharp pain in his back. He continued to work until August 1st when he became a lost time injury.

Employee was working on construction of an overcast when part of the wall fell causing the constructed portion of the overcast to fall injuring employees left knee. Employee was diagnosed with a torn meniscus on 3-18-12 and became a lost time accident at that time.

Employee stated that he was installing a mandoor in a stopping when the mandoor fell over cutting his right index finger. Employee received 8 stitches at the Middlesboro Hospital. Employee returned to work on the next shift.

Employee stated that he had a rib channel lying against the roof bolter when it fell over and struck him in the mouth requiring 2 stitches to his lip. Employee returned to work on the next shift. A safety meeting was conducted with employees to prevent similar occurrence.

Employee was loading roof straps onto the roof bolter when they slipped and cut his hand though his glove resulting in 6 stitches. He did not miss any work and returned on the next shift.

EE stated that he was changing a fitting on a shuttle car and went to remove a crib block, when he caught the index finger on his left hand, crushing the tip. Approximately 1 inch of the finger tip was removed surgically. EE returned to work on the next scheduled shift.

Employee was helping install track when something pulled in his shoulder. He went to the ER where the x-rays were negative. He continued to work until 11/15/07 when this accident became a loss time.

EE loading header boards by hand onto a flat car to be transported underground when he set the boards down onto his left index finger. He was transported to the hospital and recieved 4 stitches to the tip of his finger. EE ret'd to duty next shift and continued normal duties.

EMPLOYEE STATED THAT HE WAS WORKING BUILDING A SEAL WHEN HE TURNED WITH A SOLID CONCRETE BLOCK IN HIS HANDS AND STRUCK ANOTHER BLOCK WITH HIS RIGHT HAND. HE WAS X-RAYED AND IT WAS DETERMINED THAT THE TIPS OF HIS RIGHT MIDDLE FINGER AND HIS RING FINGER WERE BROKEN.

Employee stated that he was loading rock dust onto the scoop when he turned with a bag of dust in his arms and hurt his left knee. He continued to work his regular job and consulted a Dr. who performed surgery on 3-28-07 to repair cartlidge in his knee.

Employee was pulling cable off the reel of the shuttle car when he felt something pull in his lower back. He continued to work until 06/04/07 when he missed his first shift of work and this became a lost time accident.

As employee was lifting timber, he felt a pull in his groin. Employee had hernia surgury on 8/01/2006 which was his first day missed.

While stepping off a flatcar, employee claimed to have pain running up back. He became restricted duty on 3/29/2006.

As employee was working on scoop he dropped a pinion on his left hand injuring his 3rd and 4th fingers.

As employee was reaching for something on the roof bolter, he cut his left arm on a strap. Recieved 9 staples in his arm.

As employee was loading roof bolting machine with supplies, he felt a sharp pain in his lower back. The employee began missing work on 6/29/2005 due to this injury.

EMPLOYEE WAS MOVING A BUNDLE OF ROOF STRAPS WHEN HE FELT PAIN IN HIS LOWER BACK. EE BEGAN MISSING WORK ON 3/18/05.

Ee was getting rock off miner when he mashed his right hand.

RIGHT SHOULDER STARTED HURTING DURING SHIFT. POSSIBLE REPETIVE MOTION INJURY. EMPLOYEE SHOVELS & GREASES EQUI9PMENT THROUGHOUT SHIFT. HAD SURGERY AND BECAME LOST TIME ON 8-26-04.

EMPLOYEE WAS SPLICING CABLE NEAR PERSONNEL CARRIER WHEN HE BUMPED HIS RIGHT ELBOW AGAINST THE CARRIER. EMPLOYEE HAD SURGERY ON 6/16/04 AND BEGAN MISSING WORK AT THAT TIME.

AS EMPLOYEE WAS PUTTING STRUCTURE IN BELT HE FELT SOMETHING PULL IN HIS LOWER BACK. SOUGHT MEDICAL TREATMENT AND BEGAN MISSING WORK ON 8/23/04.

EE WAS LIFTING STRUCTURE WHEN HE SUFFERED A PULLED MUSCLE IN HIS LOWER BACK.

EE WAS INSTALLING BRAKE SYSTEM ON A SHUTTLE CAR WHEN HE FELT SOMETHING PULL IN HIS LOWER BACK. BEGAN MISSING WORK ON 4-28-2003.

EE WENT THROUGH MANDOOR WHEN IT FELL CUTTING AND FRACTURING THE MIDDLE FINGER ON HIS RIGHT HAND.

EMPLOYEE PICKED UP RAIL BENDER STRAINING HIS BACK. SOUGHT MEDICAL TREATMENT ON 6/20/02 AND STARTED MISSING WORK ON THAT DAY.

AN EE WAS LIFTING UP A PIECE OF STRUCTURE, HE FELT A PAIN IN HIS GROIN. STARTED MISSING WORK ON 11-12-01 WHEN HE HAD SURGERY TO REPAIR A HERNIA.

AS EE PICKED UP A CABLE HE FELT A SHARP PAIN IN HIS LOWER BACK. SOUGHT MEDICAL TREATMENT ON 8/23/01. FIRST DAY MISSED WAS 8/23/01.

EE WAS PULLING BELT WITH VISE GRIPS WHEN HE FELT A SHARP PAIN IN HIS LEFT ARM.

EMPLOYEE WAS GETTING OXYGEN TANK OUT OF SUPPLY SHED WHEN HE STRAINED HIS BACK.

EMPLOYEE PUT A PLATE ON A ROOF BOLT AND THREW IT AHEAD SO HE COULD INSTALL THE NEXT BOLT. THE BOLT BOUNCED BACK STRIKING HIM ON THE BACK OF HIS RIGHT ARM.

UNROLLING MINER CABLE OFF SPOOL ONTO FLATCAR WHEN HIS THUMB GOT CAUGHT BETWEEN SPOOL & FORK- LIFT FORK, MASHING THUMB.

PULLING A PIECE OF WIRE ROPE OUT OF A RAILRUNNER WHEN THE END OF THE ROPE FLIPPED UP STRIKING HIM IN THE MOUTH BREAKING A TOOTH.

STRAINED LOWER BACK WHILE LOADING BELT STRUCTURE. SOUGHT MEDICAL ATTENTION ON 4/6/00. MISSED WORK ON 4/7/00.

EE DROPPED FINAL DRIVE GEAR (30-35LB) ON HIS FOOT WHILE HE WAS WORKING ON THE MINER.

WAS BOLTING IN #4 ENTRY, SAID HIS RIGHT SHOULDER AND NECK WERE HURTING FROM BENDING BOLTS.

EE WAS LAYING OUT TIMBERS ON THE PILLAR LINE WHEN HE STRAINED BACK.

BENDING ROOF BOLTS WHEN HE STRAINED HIS BACK.

EE STRAINED BACK WHILE REPLACING BEARINGS IN A PUMP BARREL.

HAD REAR END OF MANTRIP BLOCKED UP FRAMING BRAKEAXLE & HAD MEN CLEANING IT OUT. WAS STRAIGHTENING THE FRONT WHEEL IT ROLLED OFF THE CRIB BLOCK & HIT EE'S FOOT

MOVING ROCK OFF TRACK STRAINING BACK

AS EE BENT DOWN TO EMPTY DUST BOX ON ROOF BOLTER HIS KNEE POPPED AND HE EXPERIENCED PAIN.

AS EE RAISED HIMSELF OUT OF RAILRUNNER HE FELT PAIN IN HIS SHOULDER. SOUGHT MEDICAL ATTENTION ON 11-11-99.

PULLING ROCK OFF TOP OF CONTINUOS MINER, ROCK STRUCK TOP OF LEFT FOOT.

EMPLOYEE WAS MOVING PIPE (90 DEGREE ELBOW) WHEN HE STRAINED GROIN. EMPLOYEE CONTINUED WORKING. SOUGHT MEDICAL TREATMENT ON 7-16-99.

WAS REMOVING WASTE MATERIAL FROM HEAVY MEDIA SUMP. TURNED & BUMPED HEAD ON PIECE OF METAL. METALCAME IN CONTACT WITH EE'S SAFETY GLASSES & PUSHED & PUSED GLASSES INTO EYEBROW CAUSING LACERATIO N OF EYEBROW

CUMULATIVE STRAIN TO BACK FROM PULLING CABLE, ETC. SOUGHT MEDICAL ATTENTION ON 4-7-99.

EE WAS LOADING 8" BLOCK TWISTED AROUND & PULLED SOMETHING IN HIS BACK. LEARNED EE HAS BEEN TAKING PYHSICAL THERAPY TREATMENTS
