mmu 001 2 cross cuts inby SS #3843 IN No. 1 entry. Employee was pushing a roof bolt up into hole manually straining neck/shoulder/back.

MMU 001 Directly outby section power center. Employee was hanging curtain behind power center, dust fell off curtain onto his face.

The outby scoop operator was loading trash into the scoop bucket. When the employee went to load an empty pallet into the bucket the pallet mashed his left ring finger between the scoop bucket and the pallet. He was located at the designated trash hole.

The employee was pulling miner cable when he felt a strain in his right shoulder.

Employee was trying to get a bolt started in the drilled hole. The bolt was not bent enough to enter the hole and the employee tried forcing the bolt with his hands when he felt something pull in his groin area.

The EE and 4 other EE's were positioning a piece of metal to be welded onto the conveyor belt tailpiece dump box. The metal slipped and caught the EE's finger between the metal and the tail piece frame. This resulted in a laceration which required sutures.

Individual was assisting in moving miner cable from outby rib to inby rib. He said when he pushed on the cable he felt something pulling in his back.

Individual was loading belt structure in the scoop bucket when he felt something pull in his back. He went to the ER the next day. The individual is a contract employee with Bates Contracting

EE was assisting in the removal of the canopy from a shuttle car, when the bolt was removed from the canopy leg, the canopy fell catching the tip of his left index finger against the frame of the machine, this resulted in the amputation of the end of his finger, the Dr was able to reattach the end of his finger.

The employee was hanging the power cable and water line for the continuous mining machine and strained his back.

Employee had pain in lower back while bending roof bolts to be installed.

EE WAS GETTING A PIECE OF ROCK OFF OF THE MINER & HE CUT HIS RIGHT MIDDLE FINGER

EMPLOYEE WAS CHANGING A BEARING ON THE FEEDER, WHILE LIFTING THE HEAD SHAFT, HE HURT HIS NECK AND BUTTED HIS HEAD INTO A PIECE OF METAL.

EE ALLEGES WHILE HE WAS PULLING MINER CABLE, HE HURT HIS BACK. EE DID NOT NOTIFY AONONE OF HIS INJURY UNTIL 2 DAYS LATER.

EE WAS HANGING CAR CABLE 8P TO ROOF BOLTS. HE STATED HE FELT SOMETHING POP IN HIS CHEST.

EE WAS HELPING CARRY A 4X8X3/5 PIECE OF METAL USED FOR A BAKC BOARD ON THE TAIL PIECE AND HURT HIS CHEST. A KNOT THE SIZE OF A GOLF BALL POPPED UP IN HIS CHEST. EE HAD TO HAVE SURGERY FOR A RU PTURE AT THE BOTTOM OF HIS BREASTBONE.

EE WAS CHANGING A WET BRAKE ON A SHUTTLE CAR AND FELT PAIN IN HIS RIGHT HIP AND GROIN AREA.

EMPLOYEE DROPPED A METAL LID ON RIGHT INDEX FINGER WHILE REPAIRING CONTINUOUS MINER.

THE EMPLOYEE WAS UNLOADING 5 GAL BUCKETS OF MINE SEALANT OUT OF SCOOP WHEN HE COMPLAINED OF RIGHT SIDE HURTING.
