The employee stated that he was carrying labor saver cinder block when the block suddenly broke; shift the weight to his left hand, which caused pain in his lower back. He continued to work. However the pain did not subside, which prompted him to seek medical attention. He was diagnosed with a back strain and placed on modified duty for 3 days.

On 9/10/15, while the mine was owned and operated by Emerald Processing, LLC, the individual stated while he removing drill steel from a previously drilled hole he experienced pain in his right shoulder. The incident did not become reportable until 11/7/15 when the individual initially sought medical attention, at which time the mine was owned and operated by Kanawha Eagle Mining.

EE stated that he was reattaching a wire to the starter of the 910 mantrip at the time of the incident. He stated that he had to reach under the shields during the process and strained his shoulder and the pain intensified after servicing the wheel unit of a shuttle car. EE later sought medical attention and diagnosed with a strain. He was placed on restricted duty.

The employee stated that he was adding coolant to the 507 diesel scoop due to the scoop overheating. When he was adding the coolant to the scoop, the coolant boiled out, contacting his right arm. On 8/17/2015 the employee sought medical attention and was prescribed antibiotics for an infection.

The employee stated that he got out of his scoop to load 6 crib blocks and when he lifted and turned with the last block he felt pain in his lower back. The employee finished his shift, but later sought medical attention at which time he was diagnosed with a strain. He returned on his next shift under restricted duty.

EE stated that he was in #5 entry hanging shuttle car anchor when right arm hyperextended backward. Been attempting to place a pin in a shackle with left hand while trying to hold anchor with right. Incident became reportable 8/30/2015 follow-up diagnosed the ee with a torn tendon. EE returned to work at his normal job class. On 10/14/15 ee began missing work due to the injury

The employee stated that he and another employee were in the process of loading a scoop with rock dust and during the process the employee got rock dust into his eye. The employee later sought medical attention and was prescribed prescription eye drops.

The employee stated that he had just completed dumping the metal from the slope belt magnet and as he was raising the metal door back into position, he felt pain in his left shoulder. The incident became reportable on 3/16/2015 when the employee did not return to work.

The employee stated that he and another employee were lifting the clean-up pump guard from the pump and did not realize the guard contained mud. As they lifted it, the additional weight caught him off guard causing a strain to his back. The employee was diagnosed with a strain and did not return to work on his next shift.

EE stated that he was cleaning the #3 entry & had made passes up the right side & middle. As they were hanging the cable, the weight of the cable bent him backwards straining his back. He completed his shift & returned to work on the 17th, but due to discomfort sought medical attention after shift. Reportable due to restricted work activity on 2/17/15.

The employee stated he had just cut a piece of metal for an overflow box. As he turned the piece of metal over, he slipped and dropped the metal onto the top of his right foot. He continued to work and sought medical attention on 1/12/15, at which time he was diagnosed with a fracture and place on modified duty. This incident is being report as restricted duty.

The employee stated he was attempting to empty a belt magnet when he caught his ring finger between the magnet and the chute. The employee began missing work on 12/29/14 due to the injury.

Employee was helping move a 10" section of pipe that had gotten caught on an adjacent pipe and sump, when the pipe shifted and pinched his finger between the pipe and sump. The employee was wearing metacarpal gloves at the time.

The employee stated that he was performing a pre-operational inspection of the 20 ton diesel motor and while dropping a side panel to the motor compartment caught the tip of his finger between the panel and a 10 ton jack that was secured in a side tray. The incident resulted in a fracture to the tip of the finger and avulsion that required sutures.

The employee stated that the crusher had become clogged requiring the chute access door to be opened. While observing the chute the access door swung closed striking the back of his right hand. The employee continued working, but sought medical treatment on 10/18/14 due to continued swelling and was diagnosed with a fracture. The incident became medically reportable on 10/18/14.

The employee was positioned at the corner of an intersection to stay clear of a shuttle car that was in motion, but as the cable real tensioned to the cable anchor that the employee was standing beside of it caused the cable to strike his lower lip. The employee received a laceration that required sutures and returned his next shift.

The employee stated that he had backed the MRS out of the last lift of the block and a rock was laying in the MRS over the plow. the employee was trying to push to rock off the plow when his hand slid down a sharp edge of the rock cutting through his glove and lacerating the heel of his hand. The laceration required 6 sutures.

The employee stated that while plastering a stopping a piece of plaster fell onto his glasses and contacted his left eye causing irritation. He sought medical attention and was given prescription medication for the eye. This is being reported as medical treatment injury.

The employee stated that he was constructing a set of overcast steps and while holding one leg of the stairs the other side shifted contacting his right middle finger. This resulted in a fractured right middle finger. This is being reported as a medical reportable.

The employee stated that the 1 West tailpiece had gobbed out and after cleaning the tailpiece he bent down, picked up a rock, and tossed it onto the belt when he felt pain in his neck. The employee sought medical treatment for the strain and was placed on restricted duty for three days.

While laying track in the track entry of 1 North, employee attempted to split a half header on the rail when he smashed his middle finger on his left hand between the half header and rail

The employee stated that another employee and himself were carrying a P-50 pump approximately 200 feet. While carrying the pump toward the water hole he strained his lower back. The employee later sought medical attention and this is being reported as a medical due to scheduling follow-up visits. On 7-29-14 the EE began to miss work due to the injury.

On 4/25/14 the employee reported that he had a strained groin that was suspected to have occurred on 4/23/14 when pulled cable slack from a shuttle car reel. This is being reported as a lost time injury.

The employee stated that he was attempting to push a rock into the throat of a CM and as the back of the rock tipped into the throat it caused the end the employees hand was on to tip up towards the mine roof. The tip of the employees right ring finger was caught between the mine roof and the rock. The employee received a fracture to the tip of his ring finger and five sutures.

The employee was changing a bad bearing on a roller for the #1 belt press when the roller popped out of place causing his left ring finger to be caught between the roller and belly pan resulted in a laceration that required sutures to close.

Employee was taking a 4" valve out of a pipe when the pipe came loose and caught his hand between the 4" pipe and an adjacent sump.

Injured was transporting chemical tank when hyd. hose came off causing pond treatment to splash out hitting face and eye area.

Cut right ring finger removing old skirt boards from plant cart.

EE WAS ATTEMPTING TO HANG SHEETING GRIT, LEFT ONE END UNSECURED AND WHEN HE RAISED THE OTHER END THIS CAUSED THE UNSECURED END TO SLIP OFF. THIS REACTION CAUSED THE SHEETING TO STRIKE EE ON TH E NECK AND SHOULDER.

EE WAS RIGGING I-BEAMS FOR CRANE. WHILE MOVING BEAMS, ONE ROLLED OVER ON EE'S HAND.

EMPLOYEE WAS BOLTING STEEL 100 FT IN THE AIR AND LOST HIS HARD HAT. WHEN HE COMPLETED HIS JOB HE STOOD UP AND HIT HIS HEAD ON A PIECE OF STEEL ANGLE.

EMPLOYEE WAS LIFTING A CHAINFALL TO PUT INTO A SLING AND FELT A BURNING IN HIS GROIN AREA.

PULLING A SMALL SET OF STAIRS, EMPLOYEE STRAINED KNEE.
