Employee diagnosed with Occupational Pneumoconiosis with a 10% pulmonary function impairment after evaluation.

Employee was bolting in the #1 Entry when, while checking hole depth with a 6-ft., 0.804 diameter bolt, it slipped from left hand and smashed right ring finger between the bolt head and drill chuck. Employee self-transported to CAMC General Hospital, received four sutures, and returned to full duty on 11/13/2025.

Employee was scooping block, the scoop canopy struck the rib, dislodging material that fell into the operator's deck and hit the employee's left elbow, causing a laceration. Employee completed the shift, self-transported to the hospital, received six sutures, was fully released, and returned to regular duty on 8/27/2025 performing regular duties. Medical Treatment Only.

The Winifrede Slope Hoist display screen located in the hoist drum building was to be non-operational for over 30 minutes. Electricians are currently troubleshooting on-site and conducting necessary repairs. No accidents or entrapments have resulted from this issue, and miners are not riding the Hoist Car until repairs are made. This is not an emergency escapeway.

The electronic tracking system became inoperative at approximately 8:00 p.m. on 6/16/2025 from 9 North to the #2 Section in the Track and Intake Entries. Manual tracking procedures are in place, and troubleshooting efforts are underway to resolve the issue.

At approximately 9:00 a.m., the Winifrede Slope Hoist was found to be non-operational, displaying a drum fault condition for over 30 minutes. Electricians are currently troubleshooting on-site and conducting necessary repairs. No accidents or entrapments have resulted from this issue, and personnel can safely enter and exit the mine. This is not an emergency escapeway.

EE rolled left ankle while dismounting the Co. #402 roof bolting machine in the #5 entry due to an unseen 4"x4" hole in the mine floor. EE reported the incident later in the shift and continued working. That night they sought medical attention and was diagnosed with a sprained ankle. EE was fully released to work on 02/12/2025.

An unintentional roof fall was discovered in the track entry on 6 North Mains, Entry # 6, Break 15, 2 Breaks outby spad # 4837. MSHA and WVMHST Officials have been notified. No injuries or entrapments occurred. Ventilation is not disrupted, and travel is not impeded. This location is in old works and is not a primary or secondary escapeway.

Employee stated that EE struck EE's right forearm and right elbow on a slate bar while positioning a trickle duster. EE continued working EE's full shift. The employee continued to work, completing EE's normal work duties until 2/13/25, when EE began missing work.

EE was turning 6-inch aluminum waterline on when incident occurred, the waterline unexpectedly burst. The waterline struck EE in the back of the head. EE received six staples in the back of the head. EE was given a full medical release without restrictions, allowing EE to return to EE's regularly scheduled shift. No days lost

Employee was bolting in # 1 Entry when a rock measuring 3 ft. long x 1 1/2 ft. wide x 1-2 inches thick, fell from the mine roof striking employee on the left bicep causing a laceration. Employee received 8 sutures to close the wound and was given a full release with no restrictions to return to work on next regularly scheduled shift. No Days Lost

An unintentional roof fall was discovered in the Primary Escapeway on 7 North Belt Intake, Entry # 6 between breaks 4-5. The crew has withdrawn outby the affected area and is currently dangering off the fall and re-routing the lifeline. MSHA and State Officials have been notified. No injuries or entrapments occurred from this incident. The fall is approx. 10' high and 50' long.

Employee was retrieving a chain from the supply motor. As EE pulled, the chain entangled with other tools. When EE jerked it free, it flew back and struck EE on the bottom lip, causing a laceration. EE received 5 sutures and was given a full release with no restrictions. EE returned to work on 8/7/2024, performing EE's normal duties. No Days Lost

Employee was struck on the left forearm by a falling rock (10" long x 16" wide x 0.25"-1" thick) from the mine roof. The incident caused a laceration, requiring 10 sutures. The employee received a full release with no restrictions and returned to work on the next scheduled shift (7/25/2024) performing normal duties. No Days Lost

An unintentional roof fall was discovered at approx. 9:45 p.m. an called into both MSHA and State Agencies. The fall is in the Right Return on 10 North Belt between 5&6 Entry near spad #10279. MSHA and State Officials have been on site investigation the fall area. No Injuries or entrapments occurred.

Unintentional roof falls were discovered at approx. 6:00 a.m. in the Intake and Right Return of the #1 Section. Adjacent to spad #10241 in the Intake and just inby spad #10284 in the Right Return. Also, in the Intake from spad #10223 to spad #10245. MSHA and State Officials were notified and are on-site investigating. No injuries or entrapments occurred.

An unintentional roof fall was discovered at approx. 8:30 a.m. in the #6 Entry at Break 24 on 10 North Belt Intake just inby spad # 12281 and was reported to MSHA and State Agencies. Both agencies have already been on site and a clean-up plan was submitted and approved. No injuries or entrapments resulted from this incident.

An unintentional roof fall occurred on the #2 Retreat Mining Section, in the #3 Entry, inby spad #10195, covering a Mobile Roof Support (MRS). The size of the fall is currently undetermined. The affected area has been dangered off for safety. MSHA and West Virginia agencies were notified by phone at approximately 3:35 p.m. No injuries or entrapments resulted from this incident.

Employee was placing tools away in the tool sled when another employee, operating a scoop, had the bucket under the tool sled, accidentally lifting it, causing the sled to fall and injure the employee's right foot. Employee was diagnosed with a fractured foot. This is being claimed as a Lost Workdays Incident.

Employee was building a stopping while using a ladder. When they struck the wedge to secure the stopping, the stopping fell into the ladder, causing them to fall onto a pallet of block. Employee sought medical attention and was diagnosed with a fractured L-1 and L-5 Vertebrae. This is a Lost Work Days Incident.

While changing a scoop tire, employee cut EE's right forearm on a metal burr, sustaining a 4" long, 1/4" wide laceration. EE received 8 staples and was fully cleared to return to work with no restrictions. EE resumed normal duties on EE's next shift, 1/10/2024. This is a Medical Reportable Only incident with no lost workdays.

At approximately 7:15 a.m. it was found that the Winifrede Slope Hoist was not operating and was giving a drum fault for more than 30 minutes. Electricians are on scene to troubleshoot and repair the issues. No accidents or entrapments have occurred from this incident.

At approximately 3:30 p.m., an unintentional roof fall was found in the left return at 3 North Break 12, between spad numbers 0663 and 0662. The roof fall measures approximately 4 ft thick x 18 ft wide but does not impede ventilation. Notifications were made to MSHA and the WVMHST Hotlines. The area has been dangered off, and a ventilation revision is currently being prepared.

Employee was running a cathead through a stopping when two blocks weighing 35 pounds each, fell from above and struck employee's right hand, causing a Fracture of the Distal Phalanx of the Right Thumb. Employee was released and has returned to work. This incident is being reported as Restricted Duty.

During the reattachment of a fork on a CAT 930M Loader, employee lifted up on front of fork to help lock into position and experienced a left bicep tear. Employee sought medical attention, initially received a full release to work, but later required surgery on 10/27/2023 and has been taken off work. Employee missed first shift on 10/27/2023. This is a Lost Workdays Incident.

While hanging miner cable, employee experienced shoulder injury as they lifted the cable. Operator received MRI results on 10/20/2023 stating that employee has a partial glenoid labral tear and rotator cuff strain / sprain. This is now being claimed as Lost Workdays.

Employee was calibrating CO Detectors at the # 1 Section tailpiece, when they stood on the tailpiece to calibrate the CO, they slipped and grabbed a belt chain, jolting their left shoulder. Based on the MRI results received by the operator on 10/11/2023 this is now being claimed as a Lost Workdays Incident.

Employee was attempting to reinstall a rub rail on the continuous miner when it fell, striking employee on the top of right foot. Employee sought medical attention and was diagnosed with a transverse fracture of the right metatarsal bone in the right foot. Employee was given a full release and returned to work on next regular scheduled shift (5/8/2023).

Employee received 5 sutures on left hand after striking against canopy post of rail mounted ride.

MSHA & WVMHST Officials were notified of the potential of a roof fall occurrence at approx 8:30 am when 2 ft of rock was discovered on the track/belt entry of the Winifrede Slope at the 1100 ft mark and the entry was "working" At approx 9:00 am the roof fall was discovered by management and an MSHA Official Areas were dangered off and a clean-up plan has been submitted for approval

Employee was behind the roof bolter to move cable out of the way when EE struck the left side of face against the 16-foot roof straps that overhang off of the back of the bolter causing a laceration to face. Employee received 17 sutures to close the wound and was given a full release to return to work on EE's next regular scheduled shift. No Days Lost.

Employee was attempting to install a roof bolt into the hole EE just drilled. The bolt did not line up perfectly with the chuck of the roof bolting machine so EE bumped the rotation on the machine to try and get the bolt to go into the chuck. As EE bumped the rotation, EE's glove got caught around the bolt, wrapping EE's hand around the bolt, fracturing the left thumb. No Days Lost

Employee was throwing cable out of the way when right wrist came in contact with a roof strap measuring 16' long x 8" wide causing a laceration. Employee received 4 sutures to close the wound on 6/3/2022 but did return to work on scheduled shift that day causing this to become Medical Reportable with No Days Lost

EE was brought to the surface complaining of chest tightness. Upon arrival, Paramedics checked vitals & monitored EE's heart with an Electrocardiogram & found NO issues & vitals were stable. EE was transported to a hospital for further medical attention. On 4/26/2022 at approx. 3:00 pm after further testing, it was found that the EE had a Myocardial Infarction.

EE stated that EE bent over to pick up solid block off the floor when a row of block dislodged from the mine roof, two of the blocks struck EE, one on lower back and the other on right wrist, causing a laceration. EE was transported to the surface, where EE took an ambulance to a medical center and received 5 sutures to close the wound on wrist.

EE was operating a SC on the #1 Section. EE stated that EE was in the process of re-hanging the #212 C/M cable that had come unhooked from two roof bolt plates in the #4 Entry at Break 7 intersection. EE stated as EE was in the process of hanging the cable, EE felt a sharp pain in EE's left lower back with pain radiating down EE's left leg to EE's ankle

Employee was prying on stub shaft with a slate bar while another employee was tapping on the stub shaft with a sledge hammer. The employee missed the stub shaft and struck the foot shaft which resulted in a piece of the foot shaft breaking off and entering the employee's left bicep. The employee later had the metal removed and received 3 sutures to close the wound. Medical Only.

Injured employee was moving C/M cable out of the roadway when a rock broke loose from the mine roof. As EE turned to run away, employee felt pain in the right knee. The operator received the results of an MRI on 11/8/2021 which reported a fibular ligament tear. This is now being reported as a Lost Time Incident.

Employee was in the process of installing a .804 diameter bolt and while inserting the bolt into the drilled hole the bolt came to an abrupt stop which resulted in employee feeling a pop and a pain in EE's right shoulder. Employee later sought medical treatment and the results of an MRI were received on 10/5/2021 that reflected a torn longhead bicep muscle.

Employee was bending a .804 diameter roof bolt when EE felt pain in EE's lower back. The employee saw a specialist on 6/21/2021 and was ordered 6-8 weeks of physical therapy. Corrective surgery still might be required.

Employee was shoveling spillage into a wheelbarrow when EE felt a pain in the naval area. Employee was diagnosed with a hernia, employee was placed on restricted duty and a procedure will be scheduled to repair the hernia.

Employee was installing a 16' roof mat when it slid off the ATRS and cut EE's left wrist which resulted in a laceration that required sutures. No lost workdays.

Employee was installing a bit insert on a continuous miner and was striking the insert with a pick hammer when a piece of the hammer face broke off and embedded itself in EE's left pointer finger. Metal was removed from employee's hand by a surgeon on 12/29 and sutures were used after the procedure. No workdays were lost.

An unplanned roof fall occurred in the No. 5 entry above the anchorage zone of the primary roof support at break No. 48. The fall was reported as being an estimated 20' in width X 20' in length 7' in height.

The operator is submitting this 7000-1 due to citation number 9248260 issued on 10/22/2020. The operator had determined that this condition does not meet the definition of an occupational injury. Employee was simply walking across a graveled area at the time EE identified pain and discomfort to EE's knee. An immediate contest has also been filed. (See attached documentation).

Employee was installing belt hangers when EE's drill steel became stuck in the hole, the employee was in the process of freeing the drill steel when it fell from the hole and struck EE's left ring finger resulting in a fracture and laceration. Employee was given a full release without restrictions and returned to work on EE's next scheduled shift.

Injured employee was adjusting a hose when the employee was struck on right leg by a rock measuring approximately 20" in length by 15" in width by 6" in thickness. Based on the results of an MRI that the operator received on 9/10/2020 it was determined that the employee will need surgery to repair ACL. This is now being reported as a lost time incident.

Letting boss pass through gate and reached to close gate and wind caught gate and hit wrist.

An unintentional roof fall was found at approximately 10:00PM in the face of the #4 entry. Approximately 20'x 20'.

The lilly controller needed adjusted and the hoist was removed from service.

Injured employee was in the process of installing roof support and was bending a roof bolt to install in the mine roof when they felt a pop in right shoulder. Employee was referred to a specialist for possible rotator cuff injury. Incident is being reported as a Lost Time incident.

Injured employee was installing a metal roof strap when drill canopy came into contact with rib loosening the rib causing it to strike EE's back and lower left leg. EE finished the shift but sought medical treatment on 8/5/2019 and was diagnosed with a left fibula fracture and placed off work.

An unintentional roof fall was found in the #4 entry BRK 20 on the 1B Belt corridor, the dimensions of the fall where approximately 25' in width by 25' in length x 6-8' in thickness. Miners were withdrawn outby the area, lifeline was re-routed, area was dangered off, and training was conducted.

A roof fall approximately 100' in length X 20' in width X 8-9' in thickness was found in the primary escapeway. Crews were immediately withdrawn outby area, lifeline was rerouted.

A roof fall approximately 25' in length X 20' in width X 8-9' in thickness was found in the primary escapeway. Crews were immediately withdrawn outby area, lifeline was rerouted.

A roof fall was found at approximately 1:50PM in the intake escapeway between spad number 05247 and spad number 05249 approximately 18'-20' in width by 25'-30' in length by 6'-8' in thickness. The lifeline was rerouted and additional standing supports were added in the area.

Employee was installing roof support in the number 5 entry when the employee was struck in the left shoulder with a rock measuring 76" in length by 24" in width by 2" in thickness. Employee sought medical treatment and was referred to a specialist. MRI results were made available to the operator on 7/9/2019 at which time this became a LOST TIME incident.

Employee was repairing an intake stopping when a solid block fell approximately three feet onto their left ring finger. Needed five sutures to close laceration. No lost or restricted workdays.

The Winifrede Supply Hoist slope car derailed and a panel lid was torn off damaging switches.

FOOT CAUGHT ON CENTER BLOCK AND FELL ON KNEES STOMACH AND CHEST KNEES AND ELBOWS SCRAPPED AND BACK HURTING

The Winifrede Slope supply hoist was removed from service due to a bad cell in the battery. The battery was repaired and the hoist was returned to service.

During the weekly examination of the Toney's Branch hoist it was found the hoist failed the required brake test, the hoist was removed from service and the brakes were adjusted.

Employee was busting up rock in the walkway using a pick hammer when a smaller piece of rock broke off when EE hit the rock striking EE's left forearm which resulted in a laceration that required two sutures.

The employee stated EE was clearing out some loose coal and rock from a rib roll to install timbers at break 27 on the 2-South belt when the rock EE was carrying slipped out of EE's hands causing a laceration to EE's left ring finger. The employee sought medical attention and received six sutures. This incident is being reported as medical due to the stitches.

The employee stated EE's right hand was resting on the ground and was struck by a lifting jack that had been lifting a rover to place back on the track. The employee was diagnosed with a fractured thumb, index, and middle finger. This incident is being reported as a lost time incident.

Employee was cutting a band from a bundle of blocks with a utility knife and cut left forearm in the process. Employee received sutures to close the laceration.

The employee stated EE was cleaning out the back of the 551 charger when a 20lb fire extinguisher fell approximately 43 inches striking the employees left ring finger causing a laceration and a chipped intermediate phalange area. The employee return to work EE's next shift. This incident is being reported at medical reported due to chipped bone and laceration.

The Winifrede Slope hoist was removed from service due to a knot being found in the hoist cable. The cable was repaired and the hoist was returned to service.

The employee stated as EE was trying to get the 4-North belt back under the skirting when the bar EE was using slipped out and struck EE across the face. EE sought medical treatment and was diagnosed with a fractured left zygomatic bone and a chipped left front tooth and was taken off work. This incident is being reported as a Lost Time Accident.

Stepped out of guard shack and tripped over a rock and hurt hand and side

The employee stated was walking from the number seven entry to the number six entry when EE slipped/tripped on a rock and twisted left knee. The employee was transported via ambulance to the hospital and was taken off work. The incident is being reported as a Lost Time Accident.

The employee was unhooking cable from a scoop when the operator of the scoop placed the scoop bucket in float, causing the scoop bucket to lower on to the injured employee's right foot. The employee was diagnosed with a fractured big right toe. The scoop was shut off at the time of the incident.

The employee was trying to tighten up the already hung miner cable and waterline when the waterline burst hitting the employee on the head. The employee sought medical treatment and diagnosed with a strained neck and shoulder. The employee sought medical treatment again on 8/6/2018 and was taken off work this incident is being reported as a lost time injury.

The employee was walking behind a moving scoop when the scoop batteries struck the top of a broken area of track rail and caused the rail to raise and shift onto the employees left foot. This caused the employee to fall and twist ankle. The employee was diagnosed with a fractured metatarsal and a dislocated metatarsophalangeal joint (big toe).

The employee was standing on the bottom belt at the top of the slope trying to feed the new slope belt through a set of roller assemblies used to pull the belt up the slope. A clamp slipped on the belt causing the employee to fall and strike sternum against a piece of metal. The employee went to the hospital and was diagnosed with a fractured sternum.

On May 30, 2018 at approximately 2:44 pm the operator was notified that the employee had been diagnosed occupational pneumoconiosis.

The ee stated ee was in the # 8 entry inby spad 4297 when the incident occurred. The employee reported ee was in the process of hanging and pushing the miner cable up to hang it when ee felt pain in lower right side of back. The employee sought medical attention and was given a full release but on 5/18/2018 employee sought medical care and was taken off work.

The employee stated employee stopped at break 27 travelway to get an A/C Miller welder when employee bent over to get the excess cable when employee's back locked up. The employee sought medical attention and was diagnosed with a strain back and was placed on restricted duty.

The employee stated the ee was returning from the south seals when ee stepped on a rock that was laying in a water hole. The employee immediately came outside to seek medical attention and was placed on restricted duty and on 3/16/2018 employee went back to the hospital and was take off work. The incident is being reported as a lost time.

The employee stated EE was a dropping miner and waterline cable together when the rope hanger swung back and caught the employee in the left nostril causing a laceration. The laceration required three sutures.

The employee stated ee was in the process of opening a set of 6-foot airlock doors when the door ee was opening slammed shut causing the employee right to get smashed between the doors. The employee sought medical attention and was given restricted duty. This incident is being reported as restricted duty.

On 1/6/2016 Employee was helping drop miner cable when something got into left eye. On 1/09/2018 employee sought medical attention and was prescribed medical eye drops. This incident is being reported a medical due to the medicated eye drops.

The employee was in the process of help hanging highline when their foot got caught between the pressure pump sled and mine floor. The employee was transported to CAMC where they recieved 18 stitches and was placed on restricted duty. This incident is being reported a restricted duty.

The Employee stated was standing in a cross cut as the bolter was backing up EE went to reach for the cable and toes of left foot were ran over by the bolter. Employee finished the shift, and on 11/13/2017, Employee sought medical treatment and was taken off work. This incident is reported as a Lost Time Accident.

Break 17 on 1 South Mains Return a roof fall was discovered in the examiners travelway. The area was dangered off by Jacks and beams.

Employee reported ee place hand on the canopy of the S/C when a piece of rock fell striking the employee on the left ring finger causing a laceration. Employee sought medical attention after shift where ee received two stitches. This incident being reported as medical reportable due to getting sutures.

The employee stated ee was hanging tracking cable and stepped on some uneven bottom and twisted left knee. Employee finished shift and the next day went to med express and was diagnosed with a strained knee and was placed on restricted duty. The incident is being reported as a restricted duty.

EE stated was taking a hydraulic fitting apart on the 206 miner when trying to loosen the fitting; EE stated wrench slipped off causing EE to strike right elbow on the frame of the miner. EE sought medical attention on 10/9/17 & was placed on restricted duty. *EE sought medical attention again on 10/31/17 & was diagnosed with a chipped bone in elbow.

The employee stated EE was in the number 5 entry at spad 3592 when the incident occurred. The employee said EE was in the process of dropping down miner cable when EE's shoulder makes a "popping" sound. The following day the employee sought medical treatment and was placed on restricted duty. This incident is being reported as a restricted duty.

Employee stated they went through man door at 1A near spad # 445 into the intake and slipped on a small pile of rocks and twisted the left knee. Employee finished the shift and later sought medical treatment and was prescribed crutches. This incident is being reported as medical reportable due to crutches.

The employee stated EE was bolting in the number 6 entry on 2-Section when a piece of steel fell out of the hole causing a laceration to EE's left middle finger. The employee was transported to CAMC Memorial where EE receives two sutures to EE's middle finger and returns to work next scheduled shift. This is a reportable medical incident.

The employee stated they went to open a bag of rock dust in the # 1 Entry when approximately a cap full of rock dust got in the employees' right eye. The employee sought medical attention on 08/02/2017 and was prescribed medicated eye drops. This incident is being reported at medical reportable.

The employee stated they stepped into a hole with their right foot after dismounting the belt at 33 Break on 4 North Belt and felt pain in the right side of their lower back. The employee sought medical attention on 5-6-17 but returned to work their following shift. The employee again sought medical attention on 5-30-17 and was placed on restricted duty.

A roof fall was discovered at Break 14, 1-North, inby spad 205. The fall is approximately 20' x 20' 8' long. All approaches to fall is breakered off.

Employee stated was using a battery powered leaf blower to blow dust out of #3 D-Box and felt a small piece of dust or unknown object enter right corner of right eye. The employee went to optometrist after work and was prescribed prescription eye drops. This incident is being reported as a medical reportable due to the prescription eye drops.

The employee stated that EE was in the process of exiting EE's scoop to hang a piece of cable and slip on a rock causing EE to fall. Employee sought medical attention and it was determined that EE dislocated EE's right shoulder. Employee was placed of restricted duty. This incident is being reported as restricted duty.

A roof fall was discovered on the 3 North Panel in the track entry from breaks 2 to 3. The fall was approximately 80' in length, 5-8' thick, and 20' wide. All approaches to the fall were breakered off.

On November 16, 2016 at 9:40 am we were notified that the employee had been diagnosed with Occupational Pneumoconiosis.

At approximately 11:30 p.m., the Spice Lick Elevator was taken out of service due to a malfunctioning door switch.

Employee stated was operating the # 954 diesel mantrip when a rock struck employee measuring 3.5" wide x 1/2" thick and 6" long in the nose area causing a laceration. The employee finished shift, but later sought medical attention where the laceration took 6 sutures. This is being reported as a medical reportable incident.

SECURITY GUARD WAS WALKING OUT OF GUARD SHACK LOCATED AT BOTTOM OF HILL AND EE STEPPED THROUGH THE DOOR THRESHOLD TO THE PORCH AND TWISTED ANKLE.

On 10/10/2016 employee stated was working on the tracking and communication system in the # 6 entry, while walking out of the # 6 entry employee stated a rock measuring 1' 6" x 1' 6" x 3" thick fell on right shoulder. Employee sought medical attention and was taken off work. This incident is being reported as a lost time injury.

At approximately 5:10 a.m., the Spice Lick elevator was taken out of service due an issue with the elevator's E-stop.

On 09/30/2016 at approximately at 2:45 p.m. a roof fall was discovered in the # 4 entry intersection of the Parallel South Mains at spad # 122. The fall is approximately 15' high, 30' long and 20' foot wide. All approaches to the fall were breakered off.

Employee stated that EE was in the process of replacing a planetary on Co No. 203 miner, in that process employee felt pain in left shoulder. On 09/12/2016 employee sought medical attention and was placed on restricted duty. This incident is being reported restricted duty.

At approximately, 1:20 a.m., the Spice Lick elevator was taken out of service due to an issue with the door latch switch.

At approximately 7:15 a.m., the Spice Lick Elevator was taken out of service due to an issue with a bearing.

At approximately 11:25 p.m., the Spice Lick Elevator was taken out of service due to an electrical malfunction of the elevator door.

On 08/22/2016 Employee stated EE was in the process of fire bossing when EE was crossing a over cast. In that process, employee went to walk down the steps and slip and twisted EE's left knee. Employee finished EE's shift. On 10/15/2016 Employee missed a day of work due to EE's knee hurting. This incident is being reported as a Lost Time Incident due to the employee missing.

The employee stated they were building cribs and strained their forearm. The employee sought medical attention after their shift and was placed on restricted duty. This incident is reported at restricted duty.

The employee was sitting on the bolter eating when a piece of rock fell out between the bolts striking right hand causing a laceration to right index finger. The laceration required 6 sutures and the employee returned to work on next scheduled shift.

The employee stated they were checking charge on a 500 scoop and in that process they wiggled the plug and caused an arc flash burning their right arm. The employee sought medical attention and was placed on restricted duty. The incident is reported as a restricted duty.

Employee stated that EE was trying to pull cable out from around rock bottom that busted up and felt a sharp pain in EE's right shoulder. The incident became reportable on 08/10/2016 when the employee sought medical attention and did not report to work. This incident is being reported as a lost time injury.

The employee stated that EE was exiting # 126 scoop charger to put it on charge. As EE exited the scoop EE felt a sharp pain in lower back. The employee later sought medical attention on 08/01/206 and was diagnosed with a lumbar strain and was taken off work. This incident is being reported as a lost time injury.

On 07/26/2016, the employee stated EE was removing the wheel/hub from 410 roof bolter. Later in the shift employee stated EE was feeling pain in EE's upper stomach. This incident didn't become reportable until the employee sought medical attention and was placed on restricted duty on 10/10/2016. This incident is being reported at a restricted duty.

The employee stated that EE was closing the lid of a power center when EE pinched the tip of finger causing a laceration. The laceration required 3 sutures.

The employee stated was loading bolt supplies when EE felt pain in groin. The employee later sought medical attention and was diagnosed with a hernia. The incident became a lost time injury on 7/15/2016.

The employee was installing an insert on the miner when a sliver of metal imbedded in right thumb. This incident is being reported as a medical injury due to removing the imbedded metal.

The workers compensation carrier was notified on 5/12/2016 that the employee had been diagnosed with black lung.

On 4/27/2016 the tip of the employee's left little finger was struck by a bottom belt roller that fell over while it was leaned against the rib. On 5/12/16 employee was diagnosed with a fracture to little finger deeming it a medically reportable incident.

On 4/15/2016 at approx. 2:22 p.m. we were notified that the employee had been diagnosed with Black Lung.

A roof fall was discovered in the 3 Right Panel common intake off 1 Left Submains at Break 18. The fall was approximately 40' long, 10' high, and extended from the number 4 to number 5 entry in the panel. All approaches to the fall were breakered off.

The employee stated EE was installing belt hangers when EE reached for EE's wrench that had fell over on the drill pot when EE's drill steel fell down striking the tip of right ring finger. The steel fractured the tip of EE's finger and caused a laceration that required sutures. EE returned to work EE's next shift. This is a medical reportable incident.

The employee stated that EE was working on a broken belt and as EE crossed the #2 section tailpiece EE stepped down and twisted EE's ankle. EE sought medical attention and was prescribed crutches and walking boot if needed, but no restrictions. EE returned to work the next shift at full capacity. This is being reported as a medical due to the prescription.

On 3/14/2016 at approximately 1:45 p.m. a roof fall was discovered in the 3 Right Panel of the 1st Left Submains at brk 20. The fall was located in the intersection of the #6 entry in the return travelway.

The employee stated that EE was spot bolting on the 2 South track at break 40. While trying to position EE's plate and pizza pan over the tracking cables a piece of draw rock fell and pinched EE's left thumb against the mast. The laceration required 4 sutures and EE returned to work at EE's normal occupation.

On 1/30/16 a roof fall was discovered at break 12 on the parallel south mains at spad number 104. The fall occurred in an area that is not normally traveled, and the area dangered off.

On 01/28/2016 employee stated was carrying a bag of rock dust on right shoulder, the employee was walking and stepped into a rut causing EE to twist right knee. On 10/19/2016 employee had to have surgery on right knee to repair a torn meniscus. This incident didn't become reportable until 10/19/2016. This incident is being report Lost Time Incident.

At approximately 9:15 a.m. the slope hoist malfunctioned and had to be corrected by repairing a cooling fan in the housing of the electrical components.

A roof fall was discovered on 1 South Parallel Mains at break 13. The fall was approximately 20 feet long. Ventilation was not impaired and no travelways were impeded. All approaches to the fall were breakered off.

At approximately 7:15 p.m., the Spice Lick elevator was taken out of service due to an electrical malfunction.

The employee stated that he was operating the continuous miner in the #1 entry when a piece of draw rock fell from between the bolts striking his right shoulder and forearm. The rock caused a laceration that required 7 sutures on his forearm and fractured his acromion process.

The employee was operating the 953 mantrip traveling towards the elevator bottom when a piece of draw rock fell and struck his left hand while it was on the brake lever. The rock caused a laceration to his little finger that required sutures. The employee did not return to work on his next scheduled shift.

The employee stated that he was operating his shuttle car when he hit a hole in the roadway and bounced his head against the canopy. He sought medical attention due to stiffness and was diagnosed with a strain. He was released without restrictions and returned to work. He was prescribed a brace as needed; therefore this is being reported as medical treatment.

A roof fall was found on 3 North #6 Entry inby Spad #558 at Break #51. The roof fall was approximately 25' long and 5.5' high. All approaches to the fall were dangered off.

The employee stated that he was carrying labor saver cinder block when the block suddenly broke; shift the weight to his left hand, which caused pain in his lower back. He continued to work. However the pain did not subside, which prompted him to seek medical attention. He was diagnosed with a back strain and placed on modified duty for 3 days.

A roof fall was discovered in the 2 North Right Return at Brk. 16. The fall was approximately 40' long and 10' high extended from brk 16 toward brk 15. All approaches to the fall were breakered off.

The elevator was removed from service due to an electrical issue with the door. The elevator door was repaired and the elevator was returned to service.

EE stated that he was in #5 entry hanging shuttle car anchor when right arm hyperextended backward. Been attempting to place a pin in a shackle with left hand while trying to hold anchor with right. Incident became reportable 8/30/2015 follow-up diagnosed the ee with a torn tendon. EE returned to work at his normal job class. On 10/14/15 ee began missing work due to the injury

The elevator was removed from service due to an electrical malfunction. The elevator was repaired and returned to service.

At 11:35 p.m., the elevator was removed from service due to the elevator motor needing to be replaced.

On 4/14/15 at approximately 4:00 p.m. the operator was notified that the employee had been diagnosed with black lung.

At approximately 11:30 p.m. the Spice Lick elevator went out of service and a motor had to be replaced on the elevator.

On 3/30/2015 at approximately 11:00 p.m. a roof fall was discovered on the 1 South Mains at break 31 in the #7 entry. The fall was dangered off and all approaches breakered.

The hoist car was removed from service due to a malfunction of its magnet. The hoist car was repaired and placed back into operation.

The employee stated that he was attempting to cut a 200' piece of CO cable from a 500' spool with a utility knife. He was holding the knife in his right hand and using his left palm to apply pressure when the knife flipped over cutting the palm of his left hand. The employee sought medical attention and received 6 sutures due to the laceration & returned to finish his shift.

At approximately 7:40 a.m. the elevator was removed from service for more than 30 minutes due to a mechanical issue.

The EE was mining in #5 heading. He pulled the cable away from the miner and walked towards the rear of the miner. Through the investigation, it was determined that he walked past the standoff while the pump motor was still energized & accidentally pressed the tram lever resulting in the miner pinning his midsection against the rib.

The employee stated he twisted his right knee while he was dismounting a 988 loader. The employee initially sought medical attention but did not receive any medical treatment and returned to work. On 1/22/15 the incident became reportable due to follow-up medical care. This is being reported as a medical incident. *On 5/13/15 the EE began missing work.

ON 12/1/14 at approximately 8:00 a.m. the operator was notified that the employee had been diagnosed with pneumoconiosis.

The employee stated that he was performing a pre-operational inspection of the 20 ton diesel motor and while dropping a side panel to the motor compartment caught the tip of his finger between the panel and a 10 ton jack that was secured in a side tray. The incident resulted in a fracture to the tip of the finger and avulsion that required sutures.

The elevator was removed from service due to an electrical malfunction. The elevator was repaired and returned to service.

The employee stated that while plastering a stopping a piece of plaster fell onto his glasses and contacted his left eye causing irritation. He sought medical attention and was given prescription medication for the eye. This is being reported as medical treatment injury.

The operator was notified on 6/20/14 at approximately 3:00pm that this employee which was last employee in 2009 had been diagnosed with black lung. This is Being reported as an illness.

EE stated that the 1 North section belt head had went off on gob and he stepped up on the side of the flow-thru to see inside of the rock box. As he was stepping down he grabbed a cinder block that was sitting on the flow-thru that gave way and caused him to loose his balance and fall against the mine floor and onto his lower R leg. Diagnosed w/fractured leg, on restricted duty.

The employee stated that he had been dusting on the 1 East Belt and while walking along the belt his hard hat and light contacted the lifeline flipping it off his head and onto the belt. The employee stated he then turned and stepped towards his hat when his head struck an overcast causing a laceration to the top of his head that required 3 staples.

The EE stated he was ballasting track outby the 1 South Section track with the ballast car. He stated he had already dropped 2 of the 4 compartments and as he dropped the 3rd one he pushed the wrench down with his right hand to release the locking mechanism with his left hand. Hand slipped with the lock disengaged causing wrench to strike EE on R side of face, sutures required.

On March 28, 2014 we were notified that EE had been diagnosed with occupational pneumoconiosis.

The slope hoist was removed from service due to the speed sensor malfunctioning. The sensor was repaired and the hoist was placed into service.

The employee stated he was pulling a hand dolly across the track at the Spice Lick Shaft Bottom and tripped and fell. While falling, he tried to catch himself and fell onto his right shoulder causing a strain. This incident became a medical reportable on 2-14-14 because the employee was seen for a follow-up medical visit that resulted in physical therapy. There were no lost days.

The employee stated that he bent over to lift the handle of the tracking and communication cart and when he felt pain in his lower back. On 1-15-14 this incident was deemed a medical reportable because the employee received medical attention in the form of an injection and diagnosed with a sprain.

The employee stated that he was helping to construct a stopping when he struck his right index finger with a hammer. The finger was splinted as a result of the incident.

EE was setting up to sheave belt to lap in preparation for a belt move. During this process, he was pulling the sheave cable up the entry beside a parked scoop. As he pulled the cable, he twisted & struck his left elbow on the parked scoop's canopy. The EE sought medical attention & was given antibiotics for infection because he had struck the bursitis sack in his elbow.

The slope hoist was removed from service due to a coupling needing replaced. Once repaired, the hoist was placed back into operation.

A roof fall was discovered in the return air course between the East Mains Seals and Return Shaft 1 break outby spad 211. The fall was located in an intersection and was approximately 50' long x 20' wide x 8' thick. The fall will be dangered off.

The employee stated that while he was in the process of dropping miner cable and water line that was hung from the mine roof by a rope hanger. The hanger hook caught between his fingers as he tossed the cable to the mine floor. The hook contacted the webbing between his index and middle finger causing a laceration The employee sought medical attention and received 6 sutures.

The employee stated he was standing in front of the RBM cable when the reel engaged which caused him to fall backwards onto his back. After further consideration and because the insurer found the injury compensable, this is being reported as a lost time injury.

At approximately 7:00 p.m. the slope hoist experienced an electrical malfunction which prevented it from functioning normally.

The employee stated he was attempting to dislodge a rock that was fouled in the chain of a shuttle car, and while pushing the rock over the bumper of the car the rock cantilevered upwards striking his right index finger between the rock and mine roof. This resulted in an avulsion to the tip of the employees right index finger that required 11 sutures.

Employee was awarded 30% permanent disability for occupational pneumoconiosis.

A roof fall occurred at break 10 of the 1 South Mains. The fall occurred in the track entry. The area was dangered off.

During the preshift examination of all underground workings after a 7 day power outage a roof fall was discovered in the 1 South Mains track entry from break 28 to 29. The fall was 5 ft thick and 95' long. The fall was reported at 9:15 a.m.

Employee was installing a set of doors with 2 other employees at the bottom of the slope. 1 door was in place & they were working on the 2nd door. Door was blocked in place & 2 EEs raised 1 side causing the door to creel pinching employee's finger. The accident resulted in a laceration requiring sutures. He returned to work his next shift. Further investigation is ongoing.

At 4:00 am the slope hoist was removed from service due to an electrical issue in the hoist panel that occurred after a power outage. The hoist remained out of service until repairs could be made.

At 7:15 pm the Spice Lick elevator was taken out of service due to mechanical issues.

At 11:57 a.m., the slope hoist began having issues with the electrical system and was taken out of service and the mine was evacuated.

A roof fall occurred on the #1 Section #4 entry break 15 while retreat mining. The area affected included the intersection to the push-out that had just been mined.

At the 3:38pm the spice lick elevator was taken out of service due to an electrical issue.

The employee stated he was hanging curtain in 4 cross cut left when a piece of rock fell from between the first bolt and the rib striking the employee in the top of the head. The employee did not begin missing work until 3-14-12.

Injured was walking down steps when he slipped landing on back

Roof fall between belt and track at Brk. 29 on 1 south belt. Fall is approximately 19 foot wide. There is a permanent stopping on belt side of fall and the intersection is cribbed on both sides.

Injured was operating scoop when it was struck by shuttle car causing injured to complain with back pain

Injured stepped on loose material and twisted right leg.

Injured was riding on mantrip when rock flipped up hitting left foot and knee

Injured was trying to square hook back together when it slipped hitting right little finger.

Injured was taking slack out of miner conveyor chain and when he stood up he felt pain in right knee. (reported 1/7/12)

Injured was moving crib blocks and strained stomach. Did not report until 1/7/12.

Employee was opening gate, tripped on gravel, fell to ground, hit left ribs, hip, leg

Employee stated that he was putting together 4 inch waterline, and as he was clamping a coupling together the clamp sprung back and hit the index finger knuckle and middle finger knuckle of his left hand, employee stated that he was wearing his gloves.
