On 10/23/2015 a roof fall was discovered in the 3 West Mains at break 31 in an intersection. The fall was located in the #5 entry at spad 2196.

On 10/19/2015 a roof fall was discovered on the 3 West Mains at Brk 29 in the #6 entry.

The employee stated that he was operating the continuous miner in the #1 entry when a piece of draw rock fell from between the bolts striking his right shoulder and forearm. The rock caused a laceration that required 7 sutures on his forearm and fractured his acromion process.

On 10/13/2015 a roof fall was discovered at Brk 29 on the 3 West Mains in the #8 entry.

On 10/12/2015 a roof fall was discovered in the 3 West Mains air course where miners do not normally work of travel. The was located at break 31 in a cross cut from #3 to #4 entry.

The employee was operating the 953 mantrip traveling towards the elevator bottom when a piece of draw rock fell and struck his left hand while it was on the brake lever. The rock caused a laceration to his little finger that required sutures. The employee did not return to work on his next scheduled shift.

On 10/8/15 we were informed that the employee had been diagnosed with black lung. His last day of employment was 9/3/2012 at the operation.

The employee stated while he was shoveling the 4 West belt he lost his footing and started to fall, but reached out with his right hand to catch himself. He grabbed a piece of top structure at the wing roller and caused a laceration to the tip of his right middle that required sutures. The incident is a medical reportable injury.

EE stated that his feet got tangled up in a water hose as he walking toward the #1 press to wash it. Fell to the floor straining his left hand and wrist. 10/9/15 incident became a reportable injury because ee sought medical attention and was diagnosed with a possible hairline fracture and prescribed a wrist brace. He returned to work at his normal job his next scheduled shift.

The employee stated that he was operating his shuttle car when he hit a hole in the roadway and bounced his head against the canopy. He sought medical attention due to stiffness and was diagnosed with a strain. He was released without restrictions and returned to work. He was prescribed a brace as needed; therefore this is being reported as medical treatment.

On 9/24/2015 a roof fall was discovered that was not located in active workings of the mine. The fall was located on the 3 East Mains break 30 in the #7 entry. The area was dangered off.

On 9/21/2015 at approximately 9:20 a.m. a roof fall was discovered at break 21 of the 3 West mains between the #6 and #7 entry. The area was dangered off.

On 9/21/2015 at approximately 9:20 a.m. a roof fall was discovered at the mouth of the 3 Right Panel of the 3 West Mains.

A roof fall was found on 3 North #6 Entry inby Spad #558 at Break #51. The roof fall was approximately 25' long and 5.5' high. All approaches to the fall were dangered off.

The employee stated that he was carrying labor saver cinder block when the block suddenly broke; shift the weight to his left hand, which caused pain in his lower back. He continued to work. However the pain did not subside, which prompted him to seek medical attention. He was diagnosed with a back strain and placed on modified duty for 3 days.

On 9/10/15, while the mine was owned and operated by Emerald Processing, LLC, the individual stated while he removing drill steel from a previously drilled hole he experienced pain in his right shoulder. The incident did not become reportable until 11/7/15 when the individual initially sought medical attention, at which time the mine was owned and operated by Kanawha Eagle Mining.

While operating the company No. 405 roof bolter the employee was drilling a 12' hole for a cable bolt in the No. 5 Entry. During the drilling process the operator's Kevlar sleeve was caught between two sections of stack steel where it was bunched up around his elbow and when he engaged the rotation his arm became entangled fracturing his foreman.

On August 26, 2015 we were notified that the employee had been diagnosed with hearing loss.

A roof fall was discovered in the 2 North Right Return at Brk. 16. The fall was approximately 40' long and 10' high extended from brk 16 toward brk 15. All approaches to the fall were breakered off.

EE stated that he was reattaching a wire to the starter of the 910 mantrip at the time of the incident. He stated that he had to reach under the shields during the process and strained his shoulder and the pain intensified after servicing the wheel unit of a shuttle car. EE later sought medical attention and diagnosed with a strain. He was placed on restricted duty.

The employee stated that he was adding coolant to the 507 diesel scoop due to the scoop overheating. When he was adding the coolant to the scoop, the coolant boiled out, contacting his right arm. On 8/17/2015 the employee sought medical attention and was prescribed antibiotics for an infection.

On 8/11/2015 a fall was discovered on the 3 West Mains at Brk. 14 in the number 9 entry.

On 8/11/15 a roof fall was discovered on 3 West Mains at Brk. 28 in the number 9 entry.

The elevator was removed from service due to an electrical issue with the door. The elevator door was repaired and the elevator was returned to service.

On 7/28/15 at approximately 7:30 a.m. a roof fall was discovered that was not located in active workings of the mine. The fall was located in the 2 West Mains, Break 14 in the #6 Entry at Spad 1066. The area was breakered off.

The employee stated that he got out of his scoop to load 6 crib blocks and when he lifted and turned with the last block he felt pain in his lower back. The employee finished his shift, but later sought medical attention at which time he was diagnosed with a strain. He returned on his next shift under restricted duty.

The elevator was removed from service due to an electrical malfunction. The elevator was repaired and returned to service.

EE stated that he was in #5 entry hanging shuttle car anchor when right arm hyperextended backward. Been attempting to place a pin in a shackle with left hand while trying to hold anchor with right. Incident became reportable 8/30/2015 follow-up diagnosed the ee with a torn tendon. EE returned to work at his normal job class. On 10/14/15 ee began missing work due to the injury

On 7-17-15 a roof fall was discovered in the South Mains intake air course at spad #320. The fall was not located in active workings. The fall was located in an intersection and measured 20'x18'x6 to 7'.

The elevator was removed from service due to an electrical malfunction. The elevator was repaired and returned to service.

EE stated while he was installing roof support in the #4 entry he extended his left hand out to place a bolt on this wrench when a piece of draw rock approx 12"x12"x3-4" T fell striking his hand. When the rock contacted his hand it pushed it down toward the drill pot before he pulled it away striking his finger between the rock & pot. Chipped bone L ring finger. Medical injury.

The employee stated that while operating the 740 truck on the refuse dump he drove over a hole and strained his back. He sought medical attention and placed on restricted duty for two days.

The employee stated that he and another employee were in the process of loading a scoop with rock dust and during the process the employee got rock dust into his eye. The employee later sought medical attention and was prescribed prescription eye drops.

EE operating the 507 Scoop on the West scoop roadway travelling outby, pulling the old West Belt Head toward bottom of the slope. Between break 32 & 31, chain came loose and EE backed up, was reconnecting belt head when another EE who was operating the 509 scoop struck the belt head pushing it towards the EE. EE was caught between the belt head & scoop, fractured both lower legs.

EE stated while using slate bar to pry piece of rock from mine roof, a separate piece of rock came out of roof in front of slate bar. That piece of rock fell down bar toward EE's face. EE used left hand to block rock. Rock that struck EE's hand was approx. 3'x2'x3". EE was wearing metacarpal gloves. EE sought medical attention after his shift & was diagnosed w/bruise.

At 11:35 p.m., the elevator was removed from service due to the elevator motor needing to be replaced.

The employee stated that on 4/20 he was operating the D6 Dozer in the right grain ditch when he ran over a rock and strained his back. The incident became reportable on 5/11/2015 after the employee sought medical attention and removed from work for a back strain.

On 4/14/15 at approximately 4:00 p.m. the operator was notified that the employee had been diagnosed with black lung.

At approximately 11:30 p.m. the Spice Lick elevator went out of service and a motor had to be replaced on the elevator.

On 3/31/2015 at approximately 10:50 a.m. a roof fall was discovered that was not located in active workings of the mine. The fall was located in the previously mined 6 West Panel in the #4 entry, in the intersection 1 break inby spad 3129. The area was dangered off and cribbed.

On 3/30/2015 at approximately 11:00 p.m. a roof fall was discovered on the 1 South Mains at break 31 in the #7 entry. The fall was dangered off and all approaches breakered.

The employee stated that he had just completed dumping the metal from the slope belt magnet and as he was raising the metal door back into position, he felt pain in his left shoulder. The incident became reportable on 3/16/2015 when the employee did not return to work.

The hoist car was removed from service due to a malfunction of its magnet. The hoist car was repaired and placed back into operation.

The employee stated that he was attempting to cut a 200' piece of CO cable from a 500' spool with a utility knife. He was holding the knife in his right hand and using his left palm to apply pressure when the knife flipped over cutting the palm of his left hand. The employee sought medical attention and received 6 sutures due to the laceration & returned to finish his shift.

The employee stated that he and another employee were lifting the clean-up pump guard from the pump and did not realize the guard contained mud. As they lifted it, the additional weight caught him off guard causing a strain to his back. The employee was diagnosed with a strain and did not return to work on his next shift.

EE stated that he was cleaning the #3 entry & had made passes up the right side & middle. As they were hanging the cable, the weight of the cable bent him backwards straining his back. He completed his shift & returned to work on the 17th, but due to discomfort sought medical attention after shift. Reportable due to restricted work activity on 2/17/15.

On 2/3/2015 a roof fall was discovered in the 2 West Mains air course where miners do not normally work or travel. The fall was located in an intersection at break 16.

The employee stated that he was exiting the 507 scoop and as he pushed himself out with his arms, his right foot slipped out and he fell towards his right leg. This caused his right knee to bend inwards toward his left leg. The incident became a medical reportable on 9/17/2015 due to the employee receiving physical therapy.

At approximately 7:40 a.m. the elevator was removed from service for more than 30 minutes due to a mechanical issue.

The employee stated he had carried supplies to a roof bolt machine and on his way back stepped on a rock twisting his left ankle. The employee continued to work, but sought medical attention on 1/27/15 and was diagnosed with a strained ankle. The employee returned to work at his normal job class, but this incident is being reported due to medical treatment.
