an employee was injured while moving a 1" thick plate. 35-40 lbs After grinding the area realized he was in the wrong position to move the plate. He moved from the walkway to the bore level. Employee not anticipate the direction that the plate will move, placed both hands on the plate to move into place and caught his left little finger between the plate and the walkway.
Truck driver was removing a 2.5 inch cable from pull hook. The cable was in a bent position with stored energy. When the cable was removed from the pull hook, the stored energy released and the cable scraped the truck driver's left forearm causing a laceration that required staples.

EE was removing a chock block from the holder mounted on the front of a Cat 789 truck (unit 706) when the chock slipped out of their hand and struck the left leg above the knee.
The injured worker was a driller for a geotechnical/environmental engineering company installing monitoring wells for the mine. The worker was attempting to connect a new drill bit to the rig and it fell on his foot, injuring his toe. Steel toed footwear was in use.
Individual sustained broken left small finger while removing the transmission pump from a CAT D6R dozer. The pump slipped from his hands and smashed his left small finger against the frame. Individual was treated and released from local hospital Emergency Room and placed on Light Duty until 6/29/11. He will be evaluated on 6/29/11 to determine if he can be released to regular duty.

A welder was positioning a 2 inch x 2 inch x ? inch x 8 foot brace using his left thumb. As he lifted the brace on one end, the other end of the brace fell to the ground. As a consequence of the jolt and vibration, the falling brace pinched the welder?s left thumb between brace and the support structure. As a result, the welder suffered an open fracture to the left thumb.
A mechanic was installing the cover plate on a final drive assembly. He placed his right hand on the cover in an attempt to move the plate to align the holes over the studs. When the holes were centered over the studs, the cover plate fell onto the planetary section. His right thumb was caught between the cover and the planetary assembly causing a laceration requiring stitches.
Ee was attempting to remove sticks from the upstream side of a culvert. He was pulling on a partially burned stick approximately 3 inches in diameter and 4 feet long. He had both hands around the top of the stick and was pulling toward his face when the top portion of the stick broke off. As a result, the stick and debris hit him in the face and left eye.
EE WAS REPLACING THE PARKING BRAKE PADS ON EQUIPMENT. THE BRAKE ASSEMBLY WAS REMOVED & PLACED ON A WORK TABLE TO REMOVE THE PADS. EE WAS UNAWARE THAT THE CAGING STUD WAS ALMOST OUT (UNSCREWED) OF THE PISTON, ONLY 2 THREADS HELD THE STUD IN THE PISTON. EE COULD NOT SEE THE PISTON MOVING BACK AS IT SHOULD. AS A RESULT, HE PLACED HIS LTINDEX FINGER INSIDE. PISTON CLAMPED CLOSED.
DURING REMOVAL OF LOCK RING ON FLAT TIRE, LOCK RING SLIPPED OUT OF HIS HAND, STRIKING GROUND CAUSING SPLIT IN LOCK RING TO SPREAD OPEN AND SNAP SHUT WITH LEFT INDEX FINGER BETWEEN ENDS OF LOCK RING.
A D-10 DOZER OPERATOR WAS UNHOOKING A WIRE ROPE CABLE FROM A TOW BALL THAT IS WELDED ON TOP OF THE RIPPERS. THE CABLE WAS BENT IN SUCH A MANNER THAT STORED ENERGY WAS RELEASED WHEN THE OPERATO R REMOVED THE CABLE FROM THE TOW BALL. AS A RESULT, THE UNEXPECTED RELEASE OF STORED ENERGY, THE CABLE SPRANG AND STRUCK THE OPERATOR IN THE UPPER LEFT LIP. CAUSED LACERATION.
TWO WORKERS WERE MOVING A 55 GAL BARREL OR GRREASE INTO POSITION AT THE PUMPING STATION ON DRAGLINE 101. THEY WERE MOVING THE BARREL BY ROLLING IT ON ITS EDGE. AS THEY REACHED THE FINAL POSITI ON WHERE THE BARREL IS STORED, COWORKER LET GO OF THE BARREL BEFORE INJURED WAS PREPARED TO MOVE FROM HARM'S WAY AS THE BARREL RIGHTED ITSELF AGAINST THE BULK TANK WALL. HIS RIGHT MIDDLE FING
EE WAS PLACING A PRY-BAR ON THE RACK OF THE P&H SHOVEL (THIS RACK IS DESIGNED TO STORE THE PRY-BAR.) WHILE PLACING 1 END OF PRY-BAR INTO LEFT SLOT, HE FOCUSED HIS ATTENTION ON PLACING THE OTHE R END IN THE RIGHT SLOT. PRY-BAR FELL OUT OF THE LEFT SLOT WHILE HE LIGHTLY HELD IT IN HIS LEFT HAND. PRYBAR FELL ON HIS LEFT 5TH FINGER (BETWEEN PRYBAR & SUPPORT CHANNEL.)
