The mechanic was working on a skid-mounted pump when EE's right foot slid into an opening, causing EE to twist EE's left ankle.

The employee partially dislocated a shoulder when they fell to the ground.

Employee sustained burns while exiting the cab of employee's haul truck after a fire had ignited on the cab side of the haul truck.

The mechanic stepped onto a vendor's trailer and a board sprung up, making contact with the mechanic's left eye.

Employee was attempting to dislodge a small tree limb that was stuck behind the blade of EE's bulldozer. When EE let go of the limb, stored energy caused it to snap back, pinning the tip of EE's left little finger against the dozer blade and fracturing the bone.

Employee cut palm of left hand and required 8 stitches.

Employee was working on haul truck ladder in the shop. As EE walked past the ladder it moved from the stowed position, knocking EE to the ground in the process. Employee's left fibula was fractured as a result of the accident.

Employee sprained ankle while dismounting motor-grader.

Employee lost balance and fell to the ground while dismounting dozer. Employee suffered a fractured left heel.

Air hose reel malfunctioned and stopped rotating suddenly while employee was pulling hose out. When the hose stopped, the employee felt pain in shoulder.

EE missed step while dismounting excavator and contacted the ground hard on EE's feet. EE began feeling pain in EE's right leg and back days later but kept working through 12/27, the end of EE's rotation. On 12/30, while off work, the employee went to EE's doctor for lingering pain. Doctor ordered MRI and would not release employee back to work until the MRI was completed.

While dismounting equipment, employee stepped on uneven surface of coal seam, turning and spraining ankle.

Employee experienced muscle damage (bicep) while helping another employee lift a shovel dipper tooth.

While removing the bearing retainer on an end dump truck wheel hub, the employee cut thumb on the edge of a metal shim plate. The cut required stitches.

Contractor was shooting shotcrete onto walls. Some of the shotcrete splattered over contractor's eye protection and got into eye.

Employee was in the process of setting up a light plant when a chunk of mud fell from the light fixture striking the employee on the left side of the face between the hard hat and safety glasses. Mud entered the left eye behind the safety glasses.

Employee was working and began to feel weak and tired. A coworker recognized the situation and call the supervisor who is an EMR. They determined that outside assistance was needed and called 911. The local EMS arrived and determined the employee was possibly having a heart attack. The employee was treated at the local hospital for a heart attack.

Employee was installing a pin in the roller frame of a D11 when a chip came off the pin. The chip struck the employee in the back of the right thigh resulting in a laceration that required stitches. The employee thought they were using a soft face hammer, however the hammer was found to be a hardened hammer. The manufacturer was not clear in the marking of the different hammers.

Employee was operating a JLG man lift making repairs to the mast on a Marion 8200 dragline. As EE was letting the boom down on the man lift the basket on the man lift came into contact with the handrail on the mast crushing the tip of the left ring finger.

Employee was tapping a hole in a piece of plate using a starter tap. The tap broke causing the employees hand to hit the broken piece of tap still in the hole resulting in; cutting through the welding glove and into the outer edge of the palm on the right hand. The cut required stitches. The tap has a microscopic crack appearing to have been faulty.

an employee was injured while moving a 1" thick plate. 35-40 lbs After grinding the area realized he was in the wrong position to move the plate. He moved from the walkway to the bore level. Employee not anticipate the direction that the plate will move, placed both hands on the plate to move into place and caught his left little finger between the plate and the walkway.

Employee was dismounting a forklift when the forklift began moving forward catching his leg between the forklift and a chain link fence gate breaking his leg. Employee failed to follow company procedures and policies.

Employee was attempting remove a pin between a dozer push arm and tilt cylinder. The push arm was supported by the crane with the cylinder still pinned. When the pin was driven out the cylinder dropped approximately 2" to the dozer push arm. As it dropped it forced the tool being used to drive the pin out down pinching employee's hand between it and cylinder bore.

Weather conditions were cold and raining. Employee was splicing de-energized dewatering pump cable. Cable stripper slipped off cable cutting through glove, cutting the end of his left index finger.

Truck driver was removing a 2.5 inch cable from pull hook. The cable was in a bent position with stored energy. When the cable was removed from the pull hook, the stored energy released and the cable scraped the truck driver's left forearm causing a laceration that required staples.

Employee pinched hand in gate.

Electrician was removing tape from drag line trail cable conductor with folding box cutter. While making cut, cable moved causing knife to come into contact with their left index finger resulting in a laceration.

Mechanic was removing roller frame from bulldozer in shop with shop crane. While setting frame on floor their left fingers were caught in a pinch point under track rollers.

Mechanic was climbing down after troubleshooting a maintenance issued in the engine compartment of a bulldozer. Their foot slipped on the bottom step causing them to fall against service truck resulting in a small laceration on the right side of head.

EE was removing a chock block from the holder mounted on the front of a Cat 789 truck (unit 706) when the chock slipped out of their hand and struck the left leg above the knee.

The injured worker was a driller for a geotechnical/environmental engineering company installing monitoring wells for the mine. The worker was attempting to connect a new drill bit to the rig and it fell on his foot, injuring his toe. Steel toed footwear was in use.

Individual sustained broken left small finger while removing the transmission pump from a CAT D6R dozer. The pump slipped from his hands and smashed his left small finger against the frame. Individual was treated and released from local hospital Emergency Room and placed on Light Duty until 6/29/11. He will be evaluated on 6/29/11 to determine if he can be released to regular duty.

Welder was drilling a piece of 2" steel plate. He attempted to clean back drill shaving with he hand. A piece of shaving caught his finger cutting through his glove and into right index finger.

Employee was making repairs to a hydraulic cylinder hose; armour jacketing on hose cut through leather glove resulting in a laceration to left thumb.

EE was crawling under a piece of equipment to make repairs. Shop fan blew a piece of debris behind his safety glasses into his right eye.

Moving bearing heater cart across shop floor. Top cross-bar of heater fell from cart onto his left foot.

On a windy day EE was removing bottom plate of a DL bucket. Wind blew foreign object into lt eye. EE rubbed eye to clear it & scratched eye & lid.

Welder was using a hand grinder to grind out frame cracks. Attempted to adjust grinding wheel guard and wheel caught his glove pulling his left index finger into wheel resulting in a laceration to the left index finger.

Welder was removing a 2'x 2'x 1/2" steel plate for cleaning before installing a bearing assembly inside of the feeder/breaker drive.

Using a C clamp to line up bolt holes in a truck suspension. Clamp slipped striking mechanic in the face causing a laceration to the left eyebrow requiring stitches.

Employee was loosening a bolt with a pull handle and a cheater pipe. When the bolt broke free the pipe slipped and hit him on the right side of the face.

An employee was unloading a 3x3x1 section of gap grating from the bed of a pickup. A sudden gust of wind caused loose dust (dirt) to be blown into his face and eyes. Attempts to flush the dust from his left eye were unsuccessful. A doctor flushed the dust from the eye and administered an antibiotic prescription medication as a precaution.

EE, a drill tractor operator, was replacing a drill stem in the storage rack. While sliding the drill stem into the rack, EE's finger was pinched between the front of the rack and the drill screw. As a result, EE suffered a laceration to the tip of his right middle finger.

A mechanic was removing an electric starting motor from a motor grader. Using a 5/8 inch boxed end wrench, downward pressure was applied to the outermost bolt. When the bolt broke loose, the mechanic?s hand hit the machine frame and lacerated the right middle finger at the middle knuckle.

A welder was positioning a 2 inch x 2 inch x ? inch x 8 foot brace using his left thumb. As he lifted the brace on one end, the other end of the brace fell to the ground. As a consequence of the jolt and vibration, the falling brace pinched the welder?s left thumb between brace and the support structure. As a result, the welder suffered an open fracture to the left thumb.

A mechanic was installing the cover plate on a final drive assembly. He placed his right hand on the cover in an attempt to move the plate to align the holes over the studs. When the holes were centered over the studs, the cover plate fell onto the planetary section. His right thumb was caught between the cover and the planetary assembly causing a laceration requiring stitches.

Ee was attempting to remove sticks from the upstream side of a culvert. He was pulling on a partially burned stick approximately 3 inches in diameter and 4 feet long. He had both hands around the top of the stick and was pulling toward his face when the top portion of the stick broke off. As a result, the stick and debris hit him in the face and left eye.

SEDIMENTATION POND SP-4 LEVEE DEVELOPED A SIGNIFICANT LEAK AROUND THE 36 INCH DECANT PIPE. AS A CONSEQUENCE, THE LEADK ERODED THE LEVEE CAUSING FAILURE IN THIS AREA OF THE DAM. THE ERODED AREA WAS APPROX. NINETEEN FEET WIDE BY SIXTY FEET LONG. THE WATER WAS RELEASED INTO MIDDLE BYWY CREEK AND THE RESULTING FLOW WAS CONTAINED? NO DOWNSTREAM FLOODING OR DAMAGE OCCURRED.

EE WAS REPLACING THE PARKING BRAKE PADS ON EQUIPMENT. THE BRAKE ASSEMBLY WAS REMOVED & PLACED ON A WORK TABLE TO REMOVE THE PADS. EE WAS UNAWARE THAT THE CAGING STUD WAS ALMOST OUT (UNSCREWED) OF THE PISTON, ONLY 2 THREADS HELD THE STUD IN THE PISTON. EE COULD NOT SEE THE PISTON MOVING BACK AS IT SHOULD. AS A RESULT, HE PLACED HIS LTINDEX FINGER INSIDE. PISTON CLAMPED CLOSED.

DURING REMOVAL OF LOCK RING ON FLAT TIRE, LOCK RING SLIPPED OUT OF HIS HAND, STRIKING GROUND CAUSING SPLIT IN LOCK RING TO SPREAD OPEN AND SNAP SHUT WITH LEFT INDEX FINGER BETWEEN ENDS OF LOCK RING.

PRESSURE BUILT IN COMPRESSOR WHEN REACHED MAX - HOSE BLEW AND RADIATOR CLAMP HIT EE IN FOREARM.

EMPLOYEE SAYS THAT HE BENT OVER TO PICK UP PIPE,WHEN DRILLER DROPPED A PIPE ON THE BACK OF HIS NECK. SAYS IT STRUCK SO HARD HE WENT TO HIS KNEES. DRILLER SAYS HE HAD LIFTED THE PIPE WHEN THE E E WALKED UNDER IT AND IT DROPPED.

A D-10 DOZER OPERATOR WAS UNHOOKING A WIRE ROPE CABLE FROM A TOW BALL THAT IS WELDED ON TOP OF THE RIPPERS. THE CABLE WAS BENT IN SUCH A MANNER THAT STORED ENERGY WAS RELEASED WHEN THE OPERATO R REMOVED THE CABLE FROM THE TOW BALL. AS A RESULT, THE UNEXPECTED RELEASE OF STORED ENERGY, THE CABLE SPRANG AND STRUCK THE OPERATOR IN THE UPPER LEFT LIP. CAUSED LACERATION.

TWO WORKERS WERE MOVING A 55 GAL BARREL OR GRREASE INTO POSITION AT THE PUMPING STATION ON DRAGLINE 101. THEY WERE MOVING THE BARREL BY ROLLING IT ON ITS EDGE. AS THEY REACHED THE FINAL POSITI ON WHERE THE BARREL IS STORED, COWORKER LET GO OF THE BARREL BEFORE INJURED WAS PREPARED TO MOVE FROM HARM'S WAY AS THE BARREL RIGHTED ITSELF AGAINST THE BULK TANK WALL. HIS RIGHT MIDDLE FING

EE WAS PLACING A PRY-BAR ON THE RACK OF THE P&H SHOVEL (THIS RACK IS DESIGNED TO STORE THE PRY-BAR.) WHILE PLACING 1 END OF PRY-BAR INTO LEFT SLOT, HE FOCUSED HIS ATTENTION ON PLACING THE OTHE R END IN THE RIGHT SLOT. PRY-BAR FELL OUT OF THE LEFT SLOT WHILE HE LIGHTLY HELD IT IN HIS LEFT HAND. PRYBAR FELL ON HIS LEFT 5TH FINGER (BETWEEN PRYBAR & SUPPORT CHANNEL.)

EE WAS DISMOUNTING A CATERPILLER D-11 DOZER ON UNEVEN GROUND IN NIGHT TIME CONDITIONS. WHEN HIS RIGHT FOOT STEPPED TO THE GROUND, HIS FOOT WAS ON TOP OF A CLOD OF DIRT. WHEN HE SHIFTED HIS WEI GHT TO THE RIGHT FOOT TO CONTINUE THE DISMOUNT, HIS FOOT SLIPPED OFF THE CLOD, WHICH CAUSED HIS KNEE TO "POP".

A 300 ' SECTION OF 6" POLY PIPE WAS BEING TOWED AROUND A LEFT HAND TURN WITH A MINER ON THE GROUND, STANDING BETWEEN A BERM AND A CABLE TREE. WHILE MAKING THE TURN, THE PIPE FOLLOWED ALONG THE BERM UNTIL THE TENSION WAS SUCH THAT THE PIPE BEGAN TO RIDE UP AND OVER THE BERM. WHEN THE POLY PIPE CLEARED THE BERM, THE TENSION ON THE PIPE WAS RELEASED. PIPE HIT THE MINER BELOW LEFT KNEE

EMPLOYEE HAD STOPPED DRILLING HOLES, REMOVED SAFETY GLASSES AND WIPED HIS EYE WITH HIS HAND. METAL SHAVINGS FROM HAND GOT INTO HIS EYE.

EE WAS STEPPING DOWN FROM A DOZER BY STEPPING ON THE TRACK PAD. HSI FOOT SLIPPED AND EE FELL BACKWARD ABOUT TWO FEET, HTTIN GHIS ELBOW (LEFT) ON THE CONCRETE SHOP FLOOR. EE WENT TO THEDR THE NEXT DAY AND WAS DIAGNOSED WITH A BRUISED ELBOW. HOWEVER, EE RETURNED TOT HE DR ON 8-18-00 AND WAS DIAGNOSED WITH A BRUISED ELBOW. HOWEVER, EE RETURNED TO THE D RON 8-18-00 AND WAS DIAGNO

EMPLOYEE WAS HOLDING A HYDRAULIC TORQUE WRENCH. HE PLACED HIS FINGERS IN A PINCH POINT AND WHEN PRESSURE WAS APPLIED, HIS FINGERS WERE INJURED.

A MECHANIC WAS PULLING THE WINCH LINE OF THE D-7 DOZER. THE CABLE SLID THROUGH HIS HAND AND A LOOSE WIRE CUT THE PALM OF HIS RIGHT HAND. THE RESULTING INJURY REQUIRED 6 STITCHES.

EE HAD STEPPED DOWN FROM WHERE HE HAD BEEN WORKING AND BEGAN TO WALK AWAY. AS HE TURNED HIS BODY, HIS LEG AND FOOT WERE NOT IN SYNC WITH BODY CAUSING PAIN TO KNEE.

PIECE OF HOT SLAG FELL ON EE BURNING THROUGH. HIS JEANS AND FALLING INTO HIS BOOT BURNING HIS FOOT.

EMPLOYEE WAS LIFTING PIPE ONTO A CUTTING TABLE WHEN HE FELT PAIN IN HIS BACK.
