Employee was helping change battery cells out of the #523 scoop on the 18 headgate longwall section. In the process of loading one of the battery cells into the #765 Duckbill Mantrip, the employee felt pain in his right arm.

Employee was removing a worn out piece of metal liner from the raw coal feeder. The employee removed the metal from the feeder and went to throw the piece of metal out of the way. When the employee threw the metal it cut his left index finger.

EE was attempting to repair the column feed pump that was located in the basement of the prep plant. Another EE pushed the large sprocket & belt while EE pulled the belt toward him. While doing this, he looked away from the belt & got his finger caught in between the cogged V belt & drive pulley. He immediately jerked his hand out when he felt his finger become pinched.

Employee was removing top roller structure when he lost his grip and mashed his left index finger between top roller & discharge boom

Employee picked up a rock out of the walkway on the Longwall face and injured his back.

Employee was struck by a piece of rock that fly out of the conveyor chain on the longwall face.

Employee was loading JennMar Monster Mats on to roof bolter and felt a pain in his groin area. *Employee did not start missing work until 7/8/2014.

Employee was pulling a 1 1/2" diameter by 120' cable during a power move and said he pulled something in his lower back.

Employee was attempting to slide the forks manually on the IT28 Caterpillar loader. He tilted the forks down to slide the forks over with his hands. While doing so the hydraulic pin released the forks smashing employees right middle finger between the fork and main frame. NOTE: This incident occurred off the mine ID property.

Employee was helping other employees move a PPE vending machine when his right index finger was caught between the PPE vending machine and a 2X4 board.

Employee was handling scoop charger and strained his back.

Pulled bottom roller off of the shield car and one end of the roller came off striking the employee on the right little toe.

EE was shoveling belt when he strained his right shoulder.

Employee was taking down supply doors at Brk 87 on the long wall. Employee laid the first door against the rib. As they were dismembering the second door, the door against the rib fell, striking employee on top of his right foot.

EE was lifting the end of a piece of sheet metal to slide it out of the way. When he picked up on the metal he felt a sharp pain in his lower back.

EE was changing a bearing on a tail roller when his right middle finger got caught between the bearing housing and tailpiece frame.

Employee was helping load a continuous miner on a low car when the cat pad made contact with the track rail and a piece of the cat pad slivered off and struck employee in upper left chest.

Employee was putting sand in the sanders on a diesel locomotive when he strained his back.

Employee was manually loading monster mats in to scoop bucket when he twisted his right knee.

The crew was in the process of making a belt move. Employee was climbing up onto 9HG belt line and felt a strain in groin area.

EE was puling a oxygen bottle across the long wall face. The bottle became lodged between the relay bars. The EE pulled on the bottle catching his left ankle between the relay bar and oxygen bottle.

EE was removing a crib block out from under a Brookville 15 ton battery powered locomotive when the locomotive came down smashing his right thumb between locomotive and crib block.

Employee was placing a skirt board lid back on the side of the tail piece when the skirt board slipped and fell on the tip of his left fingers.

While working to fix the fouled longwall face chain the EE's hand was smashed between a face conveyor flight and the longwall pan line.

Employee was pushing against the monorail due to the dollies were fouled up. When the dollies broke loose the wheel on the monorail ran over his left hand fingertips.

Employee was taking buggy anchor down from where it was hung and it slipped. When he caught it something popped in his back.

EE states that he was spraying off belt structure with a water hose; hose behind him got caught in belt causing hose to be pulled into belt conveyor. EE did not release hose and his right arm got pulled into belt between belt roller and belt.

Employee was lifting 60" top belt structure and strained lower back.

Employee was loading a drive onto a flat car to be transported to another location. They had the turn buckle pinned up with a rope hanger. As they were loading the drive on the flat car, rope hanger broke causing the turn buckle to hit his left thumb.

EE was replacing damaged bolts near the section shelter. He pushed the bolt up and got his right thumb between the steel guide and roof bolt plate.

Employee was installing a water ring in the drum on the long wall shearer. Employee pick up the water ring and felt pain in his lower back.

Employee was picking up one end of a piece of 72 inch top belt roller when he felt something in his right ankle "pop". *Employee continued to work until 2/13/12.

EE was getting ready to get on the elevator to start his shift. There was a speed reducer sitting on the outside of the elevator on the surface that needed transported underground. EE along with another EE picked up the speed reducer to load it on the elevator when EE felt pain in his lower back. EE finished his shift, went to the Dr, diagnosed EE with a pulled muscle.

Employee was installing a shear pin in the sheeps head on the relay bar at shield #152. The relay bar pushed out shoving the shear pin into the pan line. Three fingers on employees right hand were caught between the shear pin and pan line.

EE was unloading portable rock duster off of supply car and he lost his balance the tongue of the duster came down and mashed his right hand ring finger and right hand middle finger.

Employee was operating the Joy 7LS long wall shearer. A piece of rock approximately 2' long X 1' Wide X 6" thick had become lodged in the bretby trough. EE lifted the rock out of the bretby trough and attempted to shove it over into the panline. While doing so the rock slipped and fell back onto his hand amputating the tip of his left ring finger.

While removing a mandoor from an intake stepping on the longwall section ee cut his right hand causing a laceration that required 8 stitches to close.

Employee alleged he was lifting a piece of steel off of the mule. *Filed under protest due to part 50 audit.

EE was operating a scoop with a 3/8" and 1/2" chain connected to it and the other end of the chain connected to the conveyor chain on a continuous miner. He was pulling in the conveyor chain on the continuous miner when the 3/8" chain broke and a link from it struck him on the left cheek.

EE was plastering stopping when a few of the cinder blocks fell on him.

Employee was removing a piece of track rail from between the tracks. He smashed his left index finger between the piece of rail and the tongue on a locomotive.

EE was closing a set of supply doors and the doors slammed shut on him causing contusions to his back and ribs.

Employee was installing water line when he smashed his left middle and ring fingers between a crib block and the water line.

EE was moving a rub rail and it slipped and smashed his hand between the rub rail and scoop insert causing a fracture to his left index finger.

Over excertion while turning 800 ft. belt over and moving heavy tool boxes.

EE was removing cover from pump compartment his hand became caught between the cover and the pan line on the longwall resulting in a laceration to the palm of his hand.
