EE was pulling on a wrench with both hands when he felt pain in both shoulders. Pain was more pronounced in his left shoulder. Pain has progressed where EE cannot do his regular job duties so he was placed out of the plant on 12/19/2007. EE's first lost workday was 12/20/2007. EE scheduled for left shoulder surgery on 01/03/2008.

EE was using an 8lb sledgehammer to close a header valve and experienced pain in his right shoulder. EE placed on restricted duty.

EE states that he experienced pain in his right shoulder after using an 8lb sledgehammer to open a valve. On 3/28/07, EE was removed from work due to work limitations placed by his treating physician. First lost work day was 03/29/07. Surgery is pending.

On 01/29/07, EE reported to his supervisor that on 01/21/2007, he had been using an 8lb sledgehammer to operate a valve. While swinging the hammer, EE alleges that he felt a "pop" in his left chest region. On 06/27/2007, EE elected to have surgery on his left shoulder.

EE was using an 8lb sledge hammer to drive a rod out through a drain valve. EE experienced a sharp pain in his upper back. EE was transported to a local hospital for evaluation. ER doctor gave EE prescription strength medication and placed on restricted duty. Unable to work within his restrictions, EE placed out of the plant.

EE was opening a valve using a 8lb sledgehammer. The EE missed the valve he was trying to open and struck his right ring finger on an adjacent valve. Thie resulted in a laceration that required sutures to repair. EE is on restricted duty.

EE felt a pain in his right shoulder when pulling on a scaling bar in an attempt to open a hopper door on a reclaim conveyor. On 04/02/07, surgery was performed on his right shoulder to repair a rotator cuff tear.

EE was using a "wobble-joint" with a socket to loosen a nut off of a bolt. The wobble joint pin broke, striking the EE on the nose. EE sustained a broken nose as a result of the impact. No lost work days, EE placed on restricted duty.

EE was using a swivel impact socket on an impact gun to remove a nut off of a bolt. The swivel socket slipped off of the nut and became entangled with the EE's gloved left hand. The result was a fractured fifth finger on his left hand. EE did not lose work days. Placed on restricted duty on 8/01/06.

EE was using a valve wrench to operate a wheel valve. The valve wrench slipped and the fourth finger of his left hand was caught between the wrench and the valve wheel handle. Result was a crush fracture of the fourth finger of his left hand. EE was placed on restricted duty. No lost work days.

EE was using a sledgehammer to clear a pluggage on a process line. His right index finger got caught between the hammer handle and a process pipe. X-ray of the finger revealed a fracture. EE place on restricted duty.

EE was using a 10 lb. sledgehammer to open a valve. The valve opened quicker than the EE expected, causing him to overextended his upper body. He felt a pain in his neck, upper back and shoulders. Plant doctor placed EE on restrictions that resulted in EE being placed off work.

EE was using a sledgehammer to operate valve. The hammer slipped out of his hand and landed in a puddle of caustic. Caustic splashed up and got into left eye. Opthomologist gave prescription-strength medication. No lost time or restrictions.

EE was holding a blunt-end chisel while a coworker struck it with a hammer. The coworker missed the chisel head and struck the EE on his right hand, fourth finger. Result was a laceration and a fractured finger. Placed on restricted duty.

On 08/17/2005, EE had to have an small piece of metal surgically removed from his right wrist that had gotten imbedded on 07/24/2005. EE had been holding a hand tool while another EE struck it with a hammer. After several weeks the EE noticed that he had something in his right wrist.

On 08/01/2005, EE reported to his supervisor that he had injured his hand on the job on 07/24/05. He thought he had a metal splinter in his right hand. He tried to dig the splinter out with his pocket knife without success. X-ray & CT scan reveal no metal splinter, however, an absess is present.Treating physcian prescribed antibotics. Placed on restricted duty on 08/01/05.

EE was using a wheel wrench to operate a wheel valve. The wheel wrench slipped and struck the EE on the mouth. On 07/29/2005, x-ray revealed a cracked tooth. No lost time or restricted duty.

EE was using a hacksaw to cut a piece of 5/8" bolt stock. As EE was making the cut, his left hand, 3rd finger came in contact with the bolt stock and sustained a laceration that required sutures to repaired.

EE was using a spline drive to operate a valve. In the process, he felt a pain in his back. On 04/21/2005, plant doctor prescribed medications and EE was given restrictions.

Employee was using a valve wrench to open a six inch plug valve when he felt a pain in his back. On 03/09/2005, plant doctor placed employee on work restrictions; no lifting > 25#, no ladder climbing, no pulling/pushing >25#

EE was using an eight pound sledgehammer to knock scale out of a pump screen box when he felt a pain in his lower back. On 3/31/05, EE was placed on restricted duty and taken out of his assigned work area.

Employee was using a pry bar to tighten a pump impeller. The pry bar slipped causing the employee's right hand to strike an adjacent flange. The impact caused a fracture of his right hand index finger.

Employee was using an 8lb hammer to open a hammer type valve. When he struck the valve for the 3rd time the hammer bounced back and struck the ee on the right side of his face. The EE was wearing safety glasses & goggles, but the impact caused both items to shift and the safety glass frame cut the employee's right eyelid. Sutures were required to close the wound.

Bauxite Transfer Attendent was using a scaling bar to descale a conveyor discharge chute. He pinched his left hand, fifth finger between the scaling bar and the chute causing a laceration that required sutures to repair.

Scaler was using a 5 foot scaling bar to break apart a piece of hydrate scale. He missed the scale and struck his left foot causing a laceration on his big toe that required suctures to repair.

A mechanic was standing on a 5'x7' scaffold and was using an old pipe wrench to remove a nipple and valve from a tank. The wrench slipped and the employee went backwards over the top rail and onto the ground below. He sustained a laceration to the back of his head which require sutures to repair. No lost time.

Employee was using an 8 pound sledgehammer to remove bolts from a tank flange. The hammer handle knocked the employee's hard hat and monogoggles off and at that moment a drop of caustic got into his left eye. This resulted in irritation of his left eye. An opthomologist prescribed pain medication and an antibiotic for the prevention of infection.

Employee was using a pipe wrench to remove a vertical drill bit. His right hand got caught between the wrench and the drill bit causing a laceration on his right hand. The laceration required five sutures to repair.
