Contractor Employee injured left elbow when trying to rotate feed cylinder (still in crate) with pry bar in the mine yard. Contractor Employee was rotating cylinder crate to do a proper hoist with telehandler.

The employee was assisting shop personnel in preparation to test 24017 drill before releasing to Ops. The miner was putting new steel onto the boom and was attempting to cut off orange plastic cap from the end of steel. The miner's utility knife slipped, contacting their leg resulting in a laceration.

The employee was using a fin-hoe to clean out lifter holes after drilling them, when EE felt pain in the lower back/left hip.

The employee was using a safety knife to strip 00 electrical cable when they sustained a laceration to the Left thumb.

Employee was tightening a centerlizer and the wrench slipped cause a laceration to the left hand.

While removing bucket off loader employee struck dog bone pin with pipe, the pin slipped out and the pipe moved, resulting in pinching fingers between pipe and bucket. Employee crushed 3rd and 4th digit finger and distal fracture of ring finger.

Employee was jacking up a derailed rail car when the jack handle did not catch springing back up striking the employee in the face. The jack handle struck the employee around the left eye area. Employee suffered a zygoma fracture and received 3 stitches.

EE used a ratchet strap to set bead on small utility tire, upon releasing ratchet strap, it whipped & struck employee on back of left hand.

Employees were unscrewing a bowl from the crusher mainframe. Employees were using bars to unscrew the bowl. The injured employee's bar slipped, causing the employee's right hand to come in contact with a hand rail. Pinching the employee's finger between the bar and hand rail.

Laceration to left hand palm. Cutting an avocado during coffee with a razorblade knife.

Employee was using a scaling bar to dislodge a rock from the #56 chute when the rock fell against the scaling bar crushing right 5th finger between the scaling bar and the back. The employee's finger sustained a full thickness laceration severing the tendon and dislocating the appendage.

Employee was scaling heading when employee got out of claw, employee noticed when employee raised arm employee had shoulder pain. Employee did not know of anything employee did that would have hurt it. Employee was carried out for a drug screen and returned to work. Employee came in 9/6/18 about shoulder and was taken to the clinic for evaluation.

A blaster was using a box cutter to cut the debt cord on a round loaded in 9664 and lacerated left hand below the thumb area. Safety was notified and transported the employee to the local ER for evaluation and had 14 stitches put in the laceration.

EE was cutting a chain, footing slipped causing hand to slide down the handle of the bolt cutters into the closing stop as they closed. The tip of L ring finger was crushed in the closing stop. The bone was broken and a laceration on finger. EE was transported to Medical Center where EE received treatment that included 5 sutures and shots for tetanus and antibiotics.

Using pry bar and finger caught between bar and feeder, fracturing little finger.

Using jack to rerail train car when handle kicked out and hit ee in throat causing a fx to the hyoid bone with possible vocal cord involvement and also a broken tooth.

hit in face with scaling bar. no med treatment until 2/26. Jaw began to hurt after violent sneeze. Went to dr and was dx with a jaw fracture. saw specialist on 2-29. No treatment needed as it is healing on its own. l

Greasing truck bearing when felt sharp pain in rt shoulder and arm

Trying to get rail car back on track with jack. jack kicked out and ee lost balance, fell forward and jack hit rt ring finger amputating the end

Using a wrench to tighten a bolt on the test drill when he felt a sting in the back of his right shoulder.

Opening box with knife; hand slipped when he contacted a staple; cut thigh

Using shovel to un-jam a conveyor and chute when he felt pain in his shoulder. Case did not become recordable until 2/12/14 after his first medical evaluation.

Using a pry bar to assist installation of a boom extension cylinder pin on mechanical scaler felt pain in back

The miner was using a come along to secure door on skip pocket when the clevis slipped out of hand and hit the miner's knee.

Using torque wrench to loosen lug nut when miner slipped, rolled ankle and fx his fibula

Utilities worker was rebuilding a flushing head for the drill jumbo when he hit his finger with a hammer

using a pry bar to straighten the over travel bracket on the shop crane when the pry bar came loose and he twisted his knee

Miner was replacing the wear liner plate on the #3 shaft. While using a spud wrench, his hand slipped and he struck his left forearm on the adjacent vertical steel member.

Tightening bolts on an oscillation weldment on DUX truck; hyper extended thumb.

Employee was bending parts and he was repositioning the part with a 4 lb hammer. He delivered a glancing blow with the hammer causing him to strike his left ring finger.

Miner was using a razor blade to open a PM kit when he cut his opposing hand between his index finger and thumb.

Miner was using a sledge hammer to break bearings loose on a pulley when a fragment of metal broke loose and struck miner's right shoulder.

Another mechanic using sledge hammer when it slipped out his hand and hit ee in face causing lacerations and contusions. On 11-21, after a CT scan, dx with a fracture of the nose

High scaling and when rock gave way, employee lunged forward, straining his rt shoulder

Using pry bar to get rail car back on track and rt arm was hit by bar. Rt forearm fracture

Working on loose bolt on feeder. Finger caught between come-a-long. Cut finger

using jacks to slew a 15 ton rail car back onto the tracks when one jack became stuck beneath the car. Miner stood on jack handle trying to get it to release when the handle released and struck him in the rt leg and pinned him against the coupling. Placed on restricted duty until MRI completed

Miner was using a wrench to tighten the drill slide when the wrench slipped and mashed two fingers.

Miner was using a large pipe wrench to loosen a nut when he twisted his back.

Miner was using a hammer to drive a pin in a claw pick when he missed the pen and hit his left right finger causing a fracture.

While opening a box injured hand slipped causing him to cut his left fore arm. He recieved five stiches.

While re- railing a wrecked ore car the injured was using a bar 5 feet long to pry the car. The car slewed over to set down on the track pushing the bar in a downward motion, striking the injured on the left leg, he also held on to the bar causing strain to his right shoulder. Injured was placed on (restricted duty on 7-7-10 and is estimated to be on light duty for 4 days.)

While scaling a rib, injured stated he felt sharp pain in his left arm. The injured worked two more shifts then went to the Dr. on 6/28/10. Was examined and released to return to work with no restrictions. On 7/2/10 injured had follow up and was placed on restricted duty.

Employee was helping clear dump chute. Rock came loose, mashing right thumb. Partial amputation to first joint.
