Employee was attempting to remove a 2" nipple that was frozen into a valve with two pipe wrenches. He was using great force and felt pain in his abdomen. Was not a reportable incident at the time but now is scheduled for surgery to repair umbilical hernia.

Fracture - Left Thumb - Struck with 8 lb beater
Cold conditions caused the outside layer of cable to break while the employee was stripping the cable. Possible handling of the cable.
Employee using a tool called (Klever Kutter) to remove electrical tape that was used to hold together 3 wires being pulled through a conduit. Employee was cutting the tape towards his left hand and the cutter slipped and caught his glove, cut the glove and consequently cut the top of his hand between the left forefinger and thumb area.
IW was striping wire using a recessed blade cutting device (Klever Kutters) and sustained a 2" laceration to the left index finger when the cutting device slipped and contacted finger.
Employee cut the left thumb with a pocket knife while cutting a zip tie.
Tightening bolts with a spud wrench that had a sleeve bar attached. Wrench slipped off the nut. Finger was pinched between sleeve bar and girder beam.
Employee was using a pneumatic chisel to install seal ropes on a filter cloth plate. He did not note any specific event but stated that his wrist began to hurt later. He was seen by a physician.
