EE was adding knife blades in storage compartment on knife. A blade slipped out when closing compartment, cutting through glove and into ring finger on right hand. EE received four stiches in finger.

Working on #6 Scoop. Employee was pulling cable leads through rubber grommet with channel locks when they slipped loose and channel locks struck above right eye. Employee was taken to ER were EE received 3 stitches.

EE WAS RECOVERING RAIL. LEFT MIDDLE FINGER WAS STRUCK BY A HAMMER CAUSING A FRACTURE.

Installing belt clamps to make belt splice, hammering two wedges between clamp and belt. Top wedge broke causing hammer to deflect and struck injured in right cheek below corner of right eye approximately 1 inch, causing a 1 inch laceration. Taken to a medical center requiring 2 stitches to close laceration.

A bundle of roof mesh was leaning against the rib, the miner positioned self between the mesh and the Roof Bolter. When the miner cut the bands on the mesh, the bundle of mesh rolled over pinning the miner against the Roof Bolter.

Employee was tightening lug nuts on roof bolter and ratchet slipped off and EE fell into roof bolter, busting EE's lip and cut EE's thumb.

Employee was using a hammer to change a tooth on an end loader bucket. A Small piece of metal chipped off the tooth and lacerated their right thigh. Two stitches were needed to close the wound.

While loading Rail on car, the rail slipped out of the clamps. When the rail hit the ground the rail bounced striking EE on the foot.

EE was splitting a roof bolter tram chain, using a hammer and a chisel. After striking the chisel with a hammer, the chisel flew up striking the employee in the mouth. This resulted in a broken tooth.

Employee tightening a ratchet strap securing a slinger duster and hook came loose striking EE in the mouth breaking a tooth in middle top of EE's mouth and loosening three more.

IE beating dust out of pinner steels with a 1' wrench when wrench bounced back and hit IE in forehead resulting in laceration needing 3 sutures to close.

EE's were putting inserts in the lugs on the miner, they were using a sledge hammer to drive the inserts in when an EE hit the lug the sledge hammer glanced off the lug and hit this EE in the mouth. This accident chipped 1 tooth and knocked the cap off of another tooth.

Employee states while hooking up a pull rope up to miner cable to pull forward to working face the knot was caught as employee jerked felt a pain in back

EE and a co-worker were changing a motor mount on a Chevy pickup. EE was underneath the truck and the co-worker was using a pry bar to remove the mount. The co-worker dropped the bar and it struck EE in the mouth. The impact chipped one of EE's front teeth and caused a small laceration to the underside of EE's tongue.

Employee was helping mechanic on a 777 rock truck. Using a pry bar and bar slipped causing employee's right wrist to bend backward too far

On 2/6/2017 employee was making repairs on a diesel man trip. Pushed up on a ratchet and felt left shoulder pop. Worked on full duty until 4/24/2017, when EE was scheduled for surgery.

While resetting the underground feeder a crib block was being removed. EE reached to pull the crib block out at the same time another worker struck the block with a hammer. The hammer struck EE's left ring finger and left middle finger causing fractures to both fingers.

Employee was changing a tire on a buggy. EE was breaking lug nuts loose using a breaker bar, when EE felt a sharp pain in back.

Section foreman was helping repair a damaged shuttle car cable. The belt knife blade slipped out of the cable and cut through EE's glove and resulted in a laceration to EE's left hand that was holding the cable. EE received 5 stitches.

Injured was swinging a pipe to remove a roof jack support for the header during reclamation and hurt right shoulder. EE had been treated for the illness since 2006 and was supposed to have surgery on it in January 2016 and injured elected not to have the surgery to treat condition. Employee was holding timber, another employee was using an axe to wedge timber in, the other EE hit the cap piece and the axe veered off and hit injured EE in the mouth.

Injured was repairing a damaged miner cable. He was cutting the outer jacket when then belt knife slipped and cut his inner left knee. He received a laceration that required 7 staples.

Employee was cutting a Joy nip off a pump cable, under excessive force went through cable and leather glove cutting left thumb, requiring 3 stitches.

Employee was using a razor knife to cut belt skirting rubber. His knife slipped and cut the side of his left pinky finger. Required 4 stitches.

EE was using a chain ratchet to make a belt splice in the #3 belt. As he was ratcheting the belt together he felt pain in his lower back. EE continued to work until 6/1/15 when he sought medical attention was taken off work.

While trimming a belt splice with a utility knife, the knife slipped and the employee lacerated his right thumb requiring 8 stitches. The employee was wearing gloves at the time of the accident.

Employee was changing a cutter head motor on a continues miner. He was prying the new motor into place with a 3' pry bar. He felt a sharp pain in his abdomen. He continued working until 5/8/2015. He seen the doctor on 5/8/2015 and had surgery on 5/9/2015.

Injured cut right hand thumb with a belt knife while cutting a piece of suction hose for a Fletcher roof bolter. He received 5 stitches.

The injured was taking apart a water line fitting with 2 pipe wrenches on 2/06/2015. He felt a pain in his shoulder. He reported accident on 2/10/2015. He had not missed work and continued to do his normal job but required surgery on his right shoulder on 5/29/2015. He has not been released to come back to work since surgery on 5/29/2015.

While cutting a brattice board with a buck saw, the saw jumped and cut the top of the employees left hand requiring 5 stitches. The employee was wearing gloves at the time of injury.

Injured employee was assisting in installing track. A rail had been jacked up to line the other end up to bolt to another rail. While lowering the rail the jack slipped and the rail fell on the injured employees right foot.

Employee was assisting in changing a hydraulic pump on a face scoop. One employee was using a hammer and chisel to break the pump loose. When he struck the chisel with the hammer the chisel shot out hitting injured employee in the left eye and side of head.

While raising one end of a pipe with a come-a-long, the opposite end of the pipe fell striking the employee on the right hand causing a laceration requiring 7 stitches.

He was hitting a chisel with a hammer, missed the chisel and hit his thumb. Fractured his left thumb.

While trimming the leads on a battery operated motor to install a new plug, the knife slipped and lacerated the employees left hand between the thumb and forefinger requiring stitches.

Mechanic was cutting a mud flap with a utility knife and it slipped causing the blade to puncture is left hand between his thumb and finger requiring 2 stitches.

Employee was using a slate bar to move the track when the bar slipped and came back hitting him in the side of the face causing a fracture of the zeugmatic arch.

Employee states "I was attempting to remove fan drive assembly from loader, and fan assembly would not come out. Removed channel holding assembly, still would not come out. Used come along to level assembly and removed last bolt with impact. I sit the impact and bolt down. After that I don't know exactly what happened. Fan assembly cut my thumb."

While cutting wire ties with a utility knife he cut through tie while holding other end of tie and cut his left hand which required 3 stitches to close the cut.

Helping make a shuttle car splice. Cut his left index finger.

While releasing the rope come-a-long it flew back and hit him on the (L) cheek bone causing a laceration which he received 5 stitches.

Victim was cutting a clip off the belt when he slipped and the knife cut his hand.

EE was removing a bearing race from a spindle with a hammer and chisel, a piece of race broke off and went into his left bicep.

Victim hit his left hand with a hammer. His safety glasses were fogged up

EE was using a hammer to loosen sprocket from excavator when the sprocket chipped and a fragment of metal entered his right thigh requiring medical attention to remove it.

EE was making a splice on the shuttle car cable and cut his index finger on his left hand requiring 6 stitches.

While cutting a new conveyor belt to length, the mechanic's foot slipped, causing him to jerk the razor blade knife and cut his ring finger on his left hand. His finger required 9 stitches.

Employee was trimming belt skirting, when the knife went through material and cut his right index finger, requiring 18 stitches.
