The employee was performing maintenance using a wrench. Wrench slipped off a bolt, causing the employee to lose balance and fall backwards into a shelf then onto the ground. The employee tried to brace themselves with their left arm injuring it.

In doing so a small piece of steel splintered away from the roller and struck the employee in the sternum and embedded below the skin.
