The employee was stepping off a Pelican street sweeper, went to step down noticed a rock and attempted to pull self back up and strained their arm.

The employee was entering a skid steer, lost their footing and fell backwards, catching their left leg between the skid steer and the fork attachment fracturing their leg.

The cone crusher stopped working, the employee traveled out of the plant shack to turn off the belts in the plant. Upon returning back to the plant shack they experienced pain in their right knee. On 3.23.26 the employee received an injection in their knee making the injury reportable.

The employee was attempting to trouble shoot an issue on the 3/4-inch stacker inside the MCC. The employee opened the bucket of the MCC and did not turn off the power to the bucket, while attempting to remove a fuse an arc flash occurred injuring the employees' hands.

Employee stated was stepping out of the anfo rig and missed the step and twisted ankle.

Employee was removing a bearing that had been cut with a torch, using a hammer. A piece of steel came off the bearing and struck the employee in the chest. Two stitches used to close the wound.

Employee stated was cutting a piece of belting with a razor knife when the knife broke loose and cut self in the leg. Employee received two stitches to close the wound.

Employee states that while driving haul truck employee hit a bump in the road that caused the air seat to bottom out, jarring employee's back. Employee started physical therapy on 8/17/2021 which made it a reportable incident.
