EE was installing inspection cover plates on a propel case. EE was tightening bolts with a socket/ratchet wrench and the socket slipped off of the bolt as they were tightening. Felt pain in right shoulder. EE was instructed to report the incident to supervisor but did not do that. The incident was not reported until 08/27/2018 when treatment was sought.

Employee strained shoulder while pulling on an alignment bar. *The incident occurred 6/23/16 but did not become a recordable until 7/18/16.

PM tech was attempting to tighten a transmission filter on a 797F truck. While hitting a hammer wrench with a hammer he hit his left hand causing a contusion between the thumb and index finger which required two stitches.

While pulling pin from the frame of a truck, individual pinched his hand between the ram and the backing plate, requiring sutures.

While installing a minor bogie pin on a D-10 dozer EE was striking the bogie with a sledgehammer and his fifth digit on his right hand was struck between side frame and the hammer handle.

While swinging a hammer, employee felt pain in abdominal region, which was later diagnosed as a hernia.

EE, mining tech. was attempting to cut a piece of 1/2" nylon rope with a utility knife. He was pulling on the knife when it slipped causing a laceration to the middle finger on his left hand. The laceration required 10 stitches to close.

EE WAS LOWERING A SECTION (BED) USING A COME-A-LONG. THE COME-A-LONG SLIPED GEARS CAUSING THE LOAD TO SHIFT OUTWARD. THE EMPLOYEE WAS HIT BY EDGE OF THE TRUCK BED ON THE BACK OF HIS RIGHT LEG PUNCTURING THE SKIN.

EE WAS CHANING OUT A HOIST CYLINDER ON AN R100 TRUCK. THE CYLINDER WAS SUSPENDED WITH A CHAIN FALL. WHEN ATTEMPTING TO REMOVE THE BOTTOM OF THE CLYLINDER FROM THE STUB SHAFT WITH A PRY BAR, TH E EE REACHED AROUND THE CYLINDER WITH HIS LEFT HAND TO POSITION THE PRY BAR. IT SLIPPED OFF & PUNCTURED HIS LEFT HAND. WHEN EE JERKED HIS HAND BACK HE HIT THE BACK OF IT ON THE HYDRAULIC TANK

THE DRAGLINE BUCKET DRAG SHACKLE WAS BEING POUNDED OUT WHEN A SMALL PIECE OF METAL CHIPPED OFF FLYING APPROXIMATELY 30 FEET EMBEDDING IN THE INJURED'S LEFT SHOULDER. REQUIRED AN INCISION AND S TITCHES TO REMOVE.

PART FROM TIRE TOOL UNDER PRESSURE DISENGAGED AND STRUCK EMPLOYEE IN LEG WHILE HE WAS REPAIRING TIRE.

WHILE MANUALLYR EELING ROPE OFF ROPE REEL STAND, INJURED GOT HIS FIGNER CAUGHT BETWEEN REEL HANDLE AND REEL FRAME WHICH LACERATED AND FRACTURED HIS LEFT INDEX FINGER.

EMPLOYEE WAS STRIPPING WIRE WHEN HIS KNIFE SLIPPED AND CUT HIS HAND.

EMPLOYEE WAS USING A WEDGE PIN TO SPREAD FORMS TO SPREAD FORMS TO GET THE TIE IN.ANOTHER EMPLOYEE ON OTHER SIDE OF FROM HIT THE TIE THE WEDGE PIN HIT EMPLOYEES TOOTH.

EMPLOYEE WAS TA1PING IN A 480V LIGHT CIRCUIT FROM AN ELEVATED MANBASKET. HIS POCKET KNIFE SLIPPED WHILE HE WAS STRIPPING INSULTATION FROM THE #1 QUADRAPLEX CUTTING HIS LEFT INDEX FINGER. FOUR STITCHES WERE 9EQUIRED TO CLOSE THE WOUND.

EMPLOYEE HAD BE9N ASSIGNED TO REPLACE SOME BROKEN BOLTS IN A FEEDER IN OUR WEST PLANT.AS HE WAS TIGHTENING THE NEW BOLTS.HIS RATCHET SLIPPED OFF OF THE HEAD OF THE BOLT.ALLOWING HIS HAND TO S STRIKE A NEAR-B2 PIECE OF ANGLE-IRON REQUIRING ((2)STITCHES TO CLOSE.

ONE OF OUR ENGINEERS WAS FOLDING A MINE MAP AND CUT HIS PALM ON RT HAND ON XACTO KNIFE BLADE THAT WAS LEFT LAYING ON MAP TABLE THE TENDON IN THE LITTLE FINGER ON RT HAND WAS SEVERED AND REQU REQUIRED SURGERY
