The employee was removing a ventilation device with a hammer. As he pulled a spad out of the curtain board his elbow hit a coal rib. He started having pain on 10/7/11 and was seen by a Dr. on 10/10/11 who put him on restricted duty.

The employee cut a steel band on a skid of block. The band slung towards the employee hitting his arm causing a laceration.

The employee was using a slate bar to straighten up a cable horn on a continuous miner. The bar slipped, he lost his balance and twisted his ankle.

The employee was using a Bit Puller to remove a bit from a continuous miner. As he struck the puller a piece of the hammer broke off imbedding in his hand.

The employee was installing floor to roof support. As he was securing a cap piece in the support his hammer slipped and hit his thumb.

The employee was attempting to make a splice on a continuous miner cable. The knife slipped cutting the employee in the leg.

The employee was changing a boom lift hose. A wrench slipped smashing the employee's finger.

The employee was changing a bushing on the continuous miner. A piece of the bushing came off and struck the employee in the upper arm.

Two employees were engaged in hosresplay. One employee swung a belt knife thinking it was closed. The belt knife was open and cut the employees forearm.

The employee was building a steel lagging overcast. He attempted to raise one side of overcast with a pry-bar to remove crib blocking. The pry-bar slipped causing the steel lagging to drop onto the employee's thumb. The employee sustained an amputation of the tip of his thumb.

The employee was changing bits on the miner. He struck a bit with a metal hammer causing an object to become airborne. The object struck the employee's arm puncturing the skin. On 11/29/07 the wound had become infected. The employee was sent to have the object removed from his arm and the wound cleaned.

The employee ran a razor belt knife into his knee while attempting to cut suction hose for the roof bolter. He was holding the hose with one hand & cutting with the other. He did state that he was cutting away from his body. The pressure needed to cut through the hose allowed the knife blade to continue into the employee's knee. The employee received six stitches.

The employee was changing bits on the continuous miner. He was using a metal hammer and a metal bit changing fork type tool. The rubber strike guard was missing. The act of striking metal against metal caused the employee's hammer to splinter. A piece of the hammer entered the back of the employee's thumb.

The ee was ratcheing a ratchet strap to secure supplies on a trailer. The buckle of the strap was above his head. The ratchet buckle came apart during pressurition striking the ee on the left sheek below the exe. the laceration was cleaned and glued at the er.

EMPL WAS TRYING TO REPAIR ELECTRIC PUMP AS HE HIT PUMP WITH HAMMER IT BROKE OIL SEAL CAUING HOT OIL TO SPRAYED IN FACE.

EE WAS TRIMMING RUBBER FROM A CONVEYOR BELT WITH A POCKET KNIFE. THE KNIFE SLIPPED CUTTING HIS THUMB.

HE WAS CHANGING BITS ON THE CONTINUOUS MINER. AS HE WAS REMOVING A BIT, HE STRUCK HIS THUMB WITH A HAMMER CAUSING A LACERATION TO HIS LEFT THUMB WHICH HAD TO BE TREATED BY A DOCTOR.

WHILE HOLDING ASHEAR JACK PIN IN HIS RIGHT HAND, HE WA SHITTING A BIT BLOCK ATTEMPTING TO LOOSEN A BROKEN BIT, WHEN HIS LETTLE FINGER WAS STRUCK BETWEEN THE BIT BLOCK AND SHEAR JACK PIN.

USING KIFE TO SPLIT PC OF FILAMENT SUCTION HOSE, HOLDING THE HOSE ON FLOOR PULLING KNIFE WITH R HAND HOSE TWISTED , KNIFE PULLED ACROSS L INDEX FINGER.

I NAME WAS BENT OVER THE ROPPER CHAIN OF THE MINER DRIVING A PIN OUT WITH A POUNCH AND HAMMER A CHIP OF STEEL CAME OFF THE HEAD OF THE PUNCH LODGING IN HIS R THIGH HE HAD IT REMOVED AFTER WORK THERE WAS NO LOST TIME

I,NAME PLASTERING A BRATTICE ON THE MAIN EAST UNIT WITH PREMIX MINE SEALANT WHEN SOME SPLATTERED IN HIS RIGHT EYE. THE EYE WAS NOT DAMAGED BUT WAS SEVERELY IRRITATED.

MECHANIC WAS USING A PORT-A-POWER HYDRAULIC JACK TO REMOVE A SHAFT BEARING FROM THE FEEDER BREAKER. THE PORT-A-POWER RAN OUT OF STROKE, SO HE PUT A CAP WEDGE BETWEEN THE FRAME AND THE JACK. WH EN HE PRESSURIZED THE JACK THE CAP WEDGE FLEW OOUT STRIKING HIM IN THE MOUTH IT REQUIRED STITCHES IN HIS LIP. HE FINISHED HIS SHIFT.

ELECTRICIAN WAS DRIVING A GROUND ROD WITH A POST DRIVER,HE RAISED THE DRIVER TOO HIGH,ALLOWING HIS AHND TO GET UNDER THE DRIVER,WHEN HE BROUGHT THE DRIVER DOWN,THE GROUND ROD CUT HIS RIGHT HAN D RESULTING IN 12 STITCHES HE RETURNED TO WROK AND FINISHED THE SHIFT.

I NAME WAS TAKING APART PVC PIPE FOR RECOVERY ONE END OF THE PIPE WAS WIRED TO THE ROOF AND HE WAS STANDING ON THE OTHER END WITH ONE FOOT WHEN HE CUT THE PIPE INTO THE END THAT WAS TIED TO TH E ROOF FLEW UP AND HIT HIM IN THE NOSE.

I,NAME WAS WORK3NG ON THE #1 SHUTTLE CAR AT THE TIME OF THE INJURY. HE WAS LAYING ON HIS SIDE PULLING ON AN END WRENCH TRYING TO BREAK A BOLT LOOSE WHEN HE PULLED A MUSCLE IN HIS RIBS. HE WORK ED THE REST OF 7HE WEEK BEFORE REPORTING THE INJURY ON 4-13-87.
