IE was removing the boost valve from shield 100 with a pry bar, when the valve freed it swung on the four connected hoses and hit EE in the mouth, breaking EE's front tooth.
IE was using a pipe wrench to loosen a fitting in the prep-plant when IE felt a pain in abdomen. The employee continued to work, took the employee back in for further evaluation on 01/15/2024 where it was determined it would take surgery to correct the hernia. IE is going in for surgery to correct the hernia on 01/18/2024.
IE was cutting a hose with a utility knife when the blade slipped it caused a laceration to the left thumb.
IE was attempting to cut a lifeline cone with a razor knife. The knife slipped off the life line cone and stuck into IE's left hand causing a laceration that required 4 stiches to close the wound.
IE was attempting to change out a top middle roller on a conveyor with another employee. They were prying the roller out with a long screw driver, when the screw driver slipped it struck the injured in the right ring finger causing a laceration and fracture, the laceration required sutures to close the wound.
IE was holding the back up wrench while loosening a bolt on the can setter when EE's wrench slipped causing it to pinch left pinky finger between the wrench and ram on the ASV.
Employee was making a cable splice, while crimping the leads together EE missed stomper with the single jack striking EE's left thumb. The smashed thumb required stitches to close the wound.
During the process of splicing a steel cable belt on an overland conveyor the contractor employee sustained a laceration to his right knee requiring 5 stitches while using a knife to put a bevel edge on a section of 48" steel cable belt which being spliced into the conveyor
Employee was attempting to cut a rock dust hose with a ulility knife, when the knife slipped the employee cut his left thumb. Requiring stiches to close the wound.
Employee was using a come-along to lift up the conveyor belt, while lifting the other side employee felt a sharp pain in his lower back. The incident originally happened on 04/25/2014 he didn't seek medical attention until 05/06/2014.
Setting a motor base into place with a come along, the hook on the come along sprung loose cause the employee to fall face first into a 5 gallon bucket causing loss of consciousness and a laceration that required stitches to close.
While lifting a 4'x8'x1/2" plate of steel into position in the prep plant elevator chute, the plate came dislodged from the chain and hoisting equipment striking the employee in the left foot. Two fractured bones in his left foot.
Employee was chipping clay out of a chute when he got his left pinky finger caught between the axe and the chute, causing a laceration to his pinky which required 3 sutures to close the wound.

Trying to get a pin into place on a Continuous Miner had another employee swing the double jack to get the pin in, he had his left finger in the back of the pin hole with one swing the pin went clear to the back of the pin hole causing it to slam into his finger, causing a fracture to the left index finger.
The employee was using a razor knife to cut a plastic cap off the end of a pipe. He was pulling hard when the knife slipped and cut this left index finger. MSHA reportable injury.
The employee had a ladder set up outside the BC108 building. He was almost to the top disconnecting a conduit with a pipe wrench. The pipe wrench slipped and he lost his balance and fell off the ladder. He first hit a snow cover which was below him and then he hit the ground. MSHA lost time accident***05/28/10.
The EE works at our Rail Road facility. EE was pulling on a release pin with a pair of channel lock pliers. The pliers slipped off the pin and struck the EE's tooth, breaking it in half. The tooth did not need immediate attention, in that the nerve was not exposed.
The employee was underground hammering with a pick hammer when a sliver broke off and struck him in the arm
The employee was helping take off the headgate cover. He was pulling on a stuck bolt while another employee was using a sledge hammer. The sledge hammer glanced off the cover and struck the employee in the cheek. MSHA reportable.
The EE was wrenching on a bolt. The wrench slipped off the bolt, causing him to strike his hand on the frame of the shuttle car.
The employee was replacing foam insulation on the pump piping at the alluvial wells. He was cutting out a new piece with a razor knife. He was pulling the knife through the insulation, when the blade went all the way through his leg. ***MSHA Reportable***
One employee was hammering a roll pin into the connector link on the longwall. A piece of metal flew off the pin and struck the supervisor. The piece of metal imbedded into the bridge of his nose.
The employee was beating on a drill steel with a cable bolt wrench when he experienced a sharp pain in his left elbow.
The employee was hooking up a trailer (in the surface yard) to his hauler. He was using a handy man jack. The employee's hand slipped off the handle of the jack and the handle struck him in the head. A laceration above the right eye needed butterfly sutures. ****MSHA reportable injury.
The employee was removing potting compound from a signal cable. He was using channel locks to hold the cable and a razor knife to cut away the compound. The pliers slipped and he cut his right ring finger with the razor knife.
EE lifting utility trailer off the hitch of the hauler with a handy-man jack. The handel slipped out of his hand and struck him in the right side of the rib cage. EE went hospital on 3/28/06 because he continued to have pain. He fractured rib on the right side.EE returned to regular duty.
The EE was using a come-a-long to lift the center tube on the Peterson Filters. The hook on the come-a-long slipped 1 link and pinched his palm between the hook and the chain it was hooked on. The EE received 8 stitches to close the laceration on the palm of his right hand. Released back to full duty.
THE EMPLOYEE WAS REMOVING A BROKEN FITTING FROM THE ROOF BOLTER. HE WAS PUSHING ON A WRENCH WHEN HE HEARD A POPPING SOUND & FELT A SHARP PAIN IN HIS RT. SHOULDER. THE EE RETURNED TO WORK IN HIS REGULAR JOB & LATER HAD AN MRI DONE & SURGERY ON 01/19/06. FIRST DAY OUT 1/09/06.
The ee was cutting open a box of tuff stuff with a razor knife. The knife slipped out of the container and cut the ee just above his left thumb. Stitches were required to close the laceration. He returned to his normal shift 3-21-05.
THE EMPLOYEE WAS CUTTING A DUST STRAP WITH A RAZOR KNIFE. THE KNIFE SLIPPED & HE CUT HIS LEFT RING FINGER. THE EE FINISHED OUT HIS SHIFT & THEN HAD FIVE STITCHES TO CLOSE THE LACERATION IN HIS FINGER.
Two employees were working to remove an inspection cover in the panline. An employee was hammering on the door and caused a piece of metal to fly off the door and hit the other employee. The metal went through his glove and cut his right 3rd finger. Sutures were required to close the laceration. Reportable injury.
The employee was cutting PVC pipe with a razor knife when it slipped and cut his left thumb open. Stitches were required to close the laceration.
THE EE WAS REMOVING A BIT FROM THE DRILL STEEL BY HITTING IT WITH A HAMMER. A SMALL PIECE BROKE OFF AND HIT HIM IN THE LEFT BICEPS, GOING THROUGH HIS SKIN INTO THE MUSCLE. THE EE CONTINUED TO WORK AND WENT TO THE DOCTOR ON 7/16/04 AT WHICH TIME A SMALL PIECE OF METAL WAS REMOVED FROM HIS ARM. THE EE RESUMED NORMAL DUTIES 7/17/04. REPORTABLE INJURY.
THE EMPLOYEE WAS LIFTING UP A BELT WITH A COME-A-LONG TO CHANGE OUT A ROLLER. THE HANDLE OF THE COME-A-LONG CAME BACK AND HIT HIM IN THE JAW/CHIN. SUTURES WERE REQUIRED TO CLOSE THE LACERATION .
THE EE WAS CUTTING A BELT WITH A UTILITY KNIFE. THE KNIFE SLIPPED OFF THE BELT AND HIT HIS LEFT LEG, RIGHT ABOVE THE KNEE. SUTURES WERE REQUIRED. THE EE RETURNED TO WORK THE NEXT DAY.
THE EE WAS LOWERING A WATER JACK. A CRIB WAS ON TOP OF THE JACK. THE EE HAD HIS LEFT HAND HOLDING THE JACK WHILE LOWERING IT. THE CRIB FELL OFF THE JACK AND STRUCK THE EE'S LEFT RING FINGER. M EDICAL ATTENTION WAS REQUIRED. 7 STITCHES. THE EE THEN RETURNED TO WORK.
THE EE WAS CUTTING INSULATION OFF OF A TRAM MOTOR LEAD. HIS KNIFE SLIPPED AND CUT HIS RIGHT THUMB. THE LACERATION REQUIRED STITCHES TO CLOSE.
THE EE WAS HELPING ANOTHER EE REMOVE A SIMPLEX JACK FROM UNDER THE BELT. HE WAS STRIKING IT WITH A DRILL STEEL AND WHEN HE STRUCK THE JACK WITH THE STEEL IT CAUSED PAIN AND DISCOMFORT TO HIS L EFT THUMB. HE CONTINUED TO HAVE A SORE THUMB ANDSAW A SPECIALIST ON 3-19-01. IT WAS DECIDED THAT THE THUMB WAS DISLOCATED. HE WAS RELEASED TO RESTRICTED DUTY 3-20-01.
THE EMPLOYEE WAS RIDING IN THE BACK OF A MANTRIP WHEN HE NOTICED THAT THE BLADE TO HIS KNIFE WAS NOT CLOSED. WHEN HE ATTEMPTED TO CLOSE IT THE BLADE CAUGHT HIS THUMB. STITCHES WERE REQUIRED TO CLOSE THE LACERATION. HE RETURNED TO FULL DUTY WORK THE NEXT DAY 11/17/00.
THE EE WAS SITTING ON THE FLOOR CROSS LEGGEDCUTTING A BOLTER CABLE FOR REPAIR. HE WAS USING A UTILITY KNIFE AND PULLING IT TOWARD HIMSELF. IT SLIPPED AND GLANCED OFF HIS RIGHT KNEE. HE REQUIRE D 6 STITCHES TO CLOSE THE LACERATION. HE WAS RETURNED TO HIS NORMAL JOB.
THE EMPLOYEE WAS PULLING ON A WRENCH TO TIGHTEN A CONVEYER CHAIN. HE FELT A SNAP IN HIS WRIST AND HE IS NOT ABLE TO STRAIGHTEN HIS FINGER ALL THE WAY.
EMPLOYEE WAS REMOVING SOME DATED PAINT FROM THE PAINT CABINET ON THE 4TH FLOOR OF THE PREP PLANT. SOME OF THE CANS WERE STUCK DOWN AND SHE WAS USING A HAMMER AND A 14 INCH FLAT FILE TO LOOSEN THE CANS. AS SHE TAPPED ON A PAINT CAN OF ACTIVATOR THE CAN PUNCTURED AND SHE WAS SPRAYED IN THE EYES. SHE WAS RETURNED TO WORK WITHOUT RESTRICTIONS AND GIVEN EYE PRESCRIPTION.
THE EE WAS STRIPPING THE JACKET OFF OF A 1/0 CABLE TO PUT A NIP ON IT. THE RAZOR KNIFE CUT THROUGH THE JACKET AND CUT HIS LEFT LITTLE FINGER REQUIRING 4 STITCHES TO CLOSE THE LACERATION. THE E E WAS RETURNED TO WORK WITH NO RESTRICTIONS.
THE EE WAS WORKING WITH A CREW WHO WERE LOWERING AN UNDERGROUND BELT. A JACK HAD BEEN SET UNDER THE SPREADER BAR. THE JACK SLIPPED OUR FROM UNDER THE SPREADER BAR AND FELL OVER ONTO THE EE S L EFT FOOT.
