EE smashed left ring finger.
Doing routine maintenance, wrench slipped & caused laceration to top of hand.

Worker was using a utility knife to cut packaging on a cord drive over. Was pulling knife toward himself instead of pushing away. Cut right leg, mid thigh about 1/2in long. Received 2 stitches.
Pry bar contacted right forearm of employee.
While using a pry bar on unit 14-731, the bar slipped and hit the IW in lip. Injury resulted in stitches.
Employee cut thumb with a utility knife while cutting a piece of belting, the laceration resulted in sutures.
Employee was cutting a piece of rubber with a box cutter and cut his left index finger, cut resulted in employee getting stitches.
Hammer fell on EE's head (EE was wearing a hard hat and safety glasses), needed 8 staples on top of his head. Bridge of nose and eyebrow were cut. EE works on crew that works 8 days on 6 days off.
EE was using a sledge to break loose a dozer tooth. The tooth spintered and the fragment went into the fleshy part of his calf,
Cutting belt, utility knife slipped, causing laceration to left thumb.
Cleaning out the HSI to adjust liners with pry bar. He pryed up with pry bar and hurt right elbow.
Pry bar hit employee in the face.
Accident happened on 8-24-08, however, he did not got to the doctor until 9-2-08. He was tightening up bolt when wrench slipped & he fell to the ground straining right shoulder muscle.
EE was prying bar & pinched left pinky finger.
Unplugging crusher with bar and struck elbow on bar.
While using pinch bar to clear mud build-up, the bar slipped and pinched thumb between the bar and framework.
He was standing on the ground, prying frozen chunks out of the box, when one chunk of frozen dirt hit him on his left ankle.

Rocks plugged the chute on the spokan, stuck bar in it & the bar came back at the employee & hit his hard hat & top of head. It broke the hard hat & cut the top of his head.
Barring material, bar slipped and struck his mouth knocking out one tooth and chipping another.
EMPLOYEE WAS 5 FT AWAY FROM LIFT AREA WHEN LIFTING EYE ON CONE WEAR LINER BROKE FREE & CHAIN USED TO LIFT WITH STRUCK THE EMPLOYEE KNOCKING OFF HIS HARD HAT & CAUSING A LACERATION ON THE SIDE OF HIS HEAD & SKULL FRACTURE.
EE WAS REBOLTING BELLY PAN ON DOZER. THE PAN WAS SECURED WITH A COME ALONG. EE MOVED PAN TO ALIGN BOLTS AND PAN SLIPPED AND FELL ON FINGER.
