Miner was cutting a piece of belting off of a roll when the knife slipped and the Miner cut EE's left hand in two places. The laceration resulted in two sutures.

Miner was in the process of using a pocketknife to cut EE's food and cut the palm of EE's left hand just below the index finger. This resulted in the Miner receiving three sutures.

Employee was working on a front end loader and while pulling on a wrench employee felt a pop in left shoulder

Employee was using a wrench to rotate a drive shaft on the dredge. The wrench slipped from the employee's hand and the employee lost balance, hitting EE's shoulder on the pump.

Around 3:30pm, an employee was attempting to clear off material buildup on a stairway. Due to the cold weather, material had frozen solid and could not be removed just by shoveling. The employee made the decision to grab a sledgehammer and utilize that to break loose the debris. About the third or fourth swing, the employee felt a sharp pain in their neck.

EE reported that EE was trying to remove rear tow pin from loader. EE was using another pin to dislodge the stuck pin and got EE's pinky finger cut/smashed in between the loader counterweight and the pin. This resulted in the tip of EE's left pinky finger getting cut.

Employee was cutting a dried silicone caulk tube out of caulk gun with a utility knife, when the knife slipped at the end and cut EE's left ring finger requiring stitches.

Employee injured left index finger while attempting to bend a piece of metal with a chipping hammer. EE missed and struck the end of finger. This resulted in EE losing the fingernail.

Using come-along to hang pulley on conveyor, additional chain hanging inside come-along needed to be moved. When chain was moved it tightened come-along and caught middle finger between chains.

EE was using a long drift pin, to remove bolts that were stuck. As they hit the pin with a hammer, they instantly felt something and saw that thumb was bleeding. EE didn't believe they struck themselves and couldn't see anything in the thumb. EE believes a piece of metal from either the drift pin or the hammer entered left thumb at the top, just behind the knuckle.

While completing maintenance activities, miner smashed EE's middle finger between the head of a sledge hammer and the cone crusher body.

During installation of a gear reducer sheave, employee was using a pry bar to position the taper lock and align bolt holes. The pry bar slipped and stuck employee in the face.

Employees were trying to remove a crusher breaker plate by welding a lifting eye on the plate and attaching a come-a-long to pull it out. They put tension on the lifting eye and after a few minutes the eye released from the plate and struck one of the employees on the left cheek.

While cutting a piece of belting for a patch with a utility knife, the knife slipped resulting in a laceration to the right thigh. **It was found during internal audit that this report was not submitted within 10 working days of accident and is being submitted now on 1/28/2020.

Employee was cutting a piece of rubber belting with a utility knife when the knife slipped striking left forearm. Small laceration of left forearm. Wound required one suture to close making it reportable.

Employee was tightening a jam nut on the crusher when the wrench slipped causing EE to fall backwards striking back on the steps behind EE.

Employee was prying on a conveyor leg and felt pain in lower back when EE got up.

While changing screen cloth he was using a bar to position the screen panels. The screen above slid down and struck the bar bending his wrist backward. Original diagnosis was a sprain/strain of the wrist. The employee was diagnosed with a fracture on November 27, resulting in lost time.

Employee was cutting a piece of skirt rubber with a utility knife when it slipped, cutting his leg.

Employee was replacing a feeder plate. He had a 3/4 electric impact gun on a shelf above were he was working. When he reached over to put a crescent wrench on a bolt he caught the cord of the impact which pulled it off the shelf. It struck his left thumb.

Employee was driving a pin out of a conveyor when he struck his left thumb with the hammer. Injury was a fracture of the left thumb.

Was using a needle scaler to clean metal when a piece of rust got in to his eye. This was reportable because the foreign body had to be removed with tweezers.

Was holding a bolt with a wrench while another tightened it. The other employee's wrench slipped off and struck him in the mouth, chipping a tooth.

Was cutting a piece of rubber when the knife slipped and struck his thigh.

Employee was trimming a piece of skirt rubber when the knife slipped, cutting his left hand. Thw wound required 4 stitches to close.

Employee was cutting a piece of rubber belting with a utility knife when the knife slipped, cutting his leg. Laceration required stitches to close.

Employee was tightening the jam nuts on the Stedman crusher. He twisted his body to reach the second nut and felt a sharp pain in his back.

Cut his thumb while using a razor knife.

Employee was cutting a piece of screen rib rubber when the utility knife slipped and cut his hand.

WAS SPLICING CONVEYOR USING A UTILITY KNIFE TO SQUARE BELT. IT SLIPPED AND HE CUT HIS FINGER. INJURY OCCURRED ON 1/5, BUT HE DID NOT SEEK TREATMENT UNTIL 1/6. WE WERE NOT AWARE THAT TREATMENT INCLUDED "STERI-STRIP" CLOSURES UNTIL MEDICAL REPORT WAS RECEIVED ON 1/12/04.

UNPLUGGING HYDRATED LIME CHUTE WHEN THE BIN FLUSHED. GOT LIME IN EYES. EMPLOYEE RECEIVED PRESCRIPTION EYE DROPS AT E/R.

WAS USING HAMMER TO CLEAN OUT BIN. FELT POP IN WRIST.

USING A RAZOR KNIFE TO CUT THE SILICONE FROM THE FEEDER GATE FLANGE. KNIFE SLIPPED AND CUT ARM. NEEDED 4 STITCHES TO CLOSE WOUND.

USING COME-ALONG TO PULL BELT TOGETHER. HIT ELBOW ON CONVEYOR FRAME.

EMPLOYEE WAS REMOVING TUBE ASSEMBLY FROM SCREEN WITH A COME-ALONG. TUBE CAUGTH ON FLOOR, CAUSING TO TO JUMP, STRIKING HIM IN THE ARM.

HIT THUMB WITH HAMMER WHEN DRIVING BUSHING INTO ELEVATOR STRAP.

USING UTILITY KNIFE TO CUT PACKAGE BANDING, KNIFE SLIPPED AND LACERATED LEFT HAND, REQUIRING 4 STITCHES.

WAS UNPLUGGING TRANSFER BOX WITH PRY BAR, ROCK CAME DOWN PINCHING FINGER BETWEEN BAR AND HOPPER FRAME.

USING BAR TO OPEN BOTTOM OF RAILROAD CAR. SPRAINED LEFT WRIST.

PIECE OF STEEL SLAG ENTERED EY WHILE HE WAS BENDING STEEL.

WAS HOLDING LARGE WRENCH SO ANOTHER EMPLOYEE COULD HIT IT WITH A HAMMER. PINCHED FINGER BETWEEN WRENCH AND CRUSHER.

USING BAR TO CLEAR PLUGGED FEEDER, FELT PAIN IN BACK.

WAS DUMPING LIME IN 5 GALLON BUCKET WITH SHOVEL. LIME FLEW UPOUT OF BUCKET AND GOT IN EYE.

WAS RE SPLICING A CONVEYOR BELT WHEN HE WENT TO REPOSITION HIMSELF, HE PUT HIS KNEE DOWN ON THE UTILITY KNIFE.

BEATING ON BOTTOM OF BIN, EMPLOYEE FELT POP IN LEFT ARM.

EE STRUCK A CHIPPING HAMMER WITH ANOTHER HAMMER CAUSING A PIECE OF THE CHIPPING HAMMER TO BREAK OFF & LODGE IN LEFT HAND

EE WAS USING A CHIPPER HAMMER TO PUNCH HOLE IN CONVEYOR BELT WHEN A PIECE OF METAL FLEW INTO WRIT PUNCTURING THE SKIN

CLEANING OUT CRUSHER WHICH WAS SHUT DOWN. BAR I WAS USING TO REMOVE STONE GLANCED OFF A ROCK THROWING RIGH HAND AGAINST CORNER OF HAMMER MILL CAUSING IUNJURY TO 2ND & 3RD FINGERS.

REMOVE A BIG ROCK OUT OF JAW & EE USED A CRUSHERHOOK TO MOVE ROCK & THE ROCK HUNG UP ON THE HOOKROCK FELL WITH HOOK FELL AGAINST LEFT HAND

EMPLOYEE WAS OPENING THE DOOR ON A RAIL CAR WITHA PRYBAR. HE PINCHED HIS FINGER BETWEEN THE PRYBAR AND THE BRAKE ROD.
