Lifting/pulling screens or hammers and pulled muscle. Diagnosis of Right Inguinal Hernia was made on June 25, 2018. Lost time began July 3, 2018.

The employee was assisting on cutting out the S2 screen support tube and was sliding the pipe out the back and misjudged where the floor was and let go resulting in the pipe striking the fifth digit on EE's left hand. The finger received a small laceration and possible nondisplaced fracture.

Employee was helping install a crusher feed ring and pinched his right thumb between the ring and shaft. Injury was a laceration of the right thumb needing three stitches to close.

A non-employee was reportedly struck by a ricocheted bullet while deer hunting on a non-mining, agricultural area of the property. This location is over a mile from the active mining area. His injury was minor consisting of a soft-tissue injury of the shoulder. He received first aid treatment at a local hospital.

The employee was walking along a string of 5 rail cars when a second employee, operating a pick-up truck in reverse, hit and knocked down the injured employee. The victim received only minor contusions.

This employee was diagnosed with a reportable hearing loss by a audilogical test conducted on 1/17/11. This was reported to the employer on 2/24/11.

Was using a needle scaler to clean metal when a piece of rust got in to his eye. This was reportable because the foreign body had to be removed with tweezers.

Was walking from breakroom to shop when he slipped on ice. He was diagnosed with a hernia on 1/19/09

Was cutting a piece of rubber when the knife slipped and struck his thigh.

Employee sprained his ankle while walking on rough terrain. In doing so, was exposed to poison ivy. Was diagnosed with rhus dermatitus on 9/9/05. Company was notified of diagnosis on 9/20/05.

Employee was driving a pin out of a conveyor support when the conveyor shifted, pinching his left thumb between the drift pin and conveyor frame. Employee received a laceration to his thumb requirig stitches to close.

Lifting elevator cover and felt pain in right should and neck.

Cleat on workboot caught on step of loader as he exited, causing knee to hyper-extend. Note: This accident was not reportable until employee had surgery on 5/3/05. Error in not reporting found during self audit.

Employee was exiting an all-terain forklift when his foot slipped off the rung. The hand that was holding on to the grab-bar was then twisted breaking a bone in his hand.

A leaking natural gas valve allowed gas to accumulate in the Dorr-Oliver dryer control room. The gas was apparently ignited by an electrical device inside the room. The resulting blast severely damaged the walls of the control room and caused a partial roof collapse.

Stepped down off the water truck and twisted left lower back/hip. Note: This incident was not reportable until lost time was incurred on 8/3/04.

Cut hand on impeller while working on pump.

Pinched left hand between driveshaft and loader frame while removing driveshaft.

Was setting the brake on a railcar and felt pain in upper back.

Was using a step ladder to access the hood of a motor grader. ladder slipped out from under him as he was dismounting, causing him to fall to the floor.

Was closing a railroad car door when he felt pain in his lower back. Strained muscle in low back.

HE WAS DISMOUNTING A HAUL TRUCK AND MISJUDGED THE HEIGHTH OF THE LAST STEP. HE LOST HIS BALANCE AND FELL CATCHING HIMSELF WITH HIS LEFT ARM. NO LOST TIME FROM THIS OCCURENCE UNTIL 2/19/04.

WAS WALKING TO HIS CAR AT THE END OF HIS SHIFT AND SLIPPED ON ICE AND FELL.

REPORTED THAT HE WAS REPLACING A PULLEY ON A CONVEYOR. THE PULLEY STARTED TO SLIP OUT OF THE FRAME & HE CAUGHT IT PULLING HIS LEFT SHOULDER. DELAY IN REPORTING DUE TO LATE REPORTING BY EE ON 10/27/03 & DELAY IN RECEIVING MEDICAL INFORMATION ON 11/04/03. THIS CLAIM IS BEING LITIGATED.

TWISTED HIS ANKLE GETTING OUT OF BOBCAT SKID STEER.

WHILE WALKING TO TIME CLOCK, HE SLIPPED IN MUD & FELL ON RAILROAD RAIL. WOUND REQUIRED STITCHES TO CLOSE.

CLEANING UP SPILLAGE WITH SHOVEL. FELT SLIGHT PAIN IN LOW BACK.

BURNED LEFT INDEX FINGER WHILE USING OXYGEN/ACTYLENE CUTTING TORCH TO CUT OFF BOLTS. TORCH REPORTEDLY BACKFIRED AND CAUGHT GLOVE ON FIRE.

PULLED MUSCLE WHILE LIFTING OXYGEN CYLINDER.

CLAIMS BACK STARTED HURTING WHILE SHOVELING. THIS CLAIM IS CONTESTED.

REINSTALLING IMPACT BED ON CONVEYOR. WHILE LIFTING INTO PLACE IT FELL, STRIKING HIM ON THE LEG.

LOWERING GUARD DOWN TO FLOOR ON DIESTER SCREEN, FELT SHARP PAIN IN LOWER BACK. NOTE: ACCIDENT NOT REPORTED UNTIL 7/13. NO LOST TIME UNTIL 7/15.

WAS MOVING TOOLS FROM CONVEYER TO TRUCK ACROSS A PLANK. FELL FROM PLANK BRAKING WRIST.

WAS CUTTING OVERHEAD BOLT, SLAG FELL INTO RIGHT EYE. EE RECEIVED MORE THAN ONE DOSE OF PRESCRIPTION EYE DROPS.

WAS REMOVING BUCKET FROM BOBCAT SKID LOADER, BUCKET FELL STRIKING HIS LEG. EE IS DIABETIC AND DEVELOPED AN INFECTION FROM HIS INJURY. LOST TIME EXPECTED TO 3-7-02.

REACHED AROUND BEHIND AND ABOVE TO ADJUST CB RADIO AND FELT A SHARP PAIN IN NECK AND UPPER BACK. NOT REPORTED AND NO LOST TIME UNTIL 12-20-01.

USING COME-ALONG TO PULL BELT TOGETHER. HIT ELBOW ON CONVEYOR FRAME.

WAS CLEANING AROUND BELT GUARD WHEN GUARD FELL STRIKING LEG.

CHANNEL SLIPPED FROM HOOK UP.

EMPLOYEE WAS STEPPING OFF HAUL TRUCK WHEN HIS RIGHT FOOT LANDED IN A SMALL HOLE ON THE GROUND CAUSING HIM TO STRAIN HIS LOWER BACK.
