Employee was losening bolts with a drive ratchet when he felt a burning sensation in his right arm near the bicep/elbow.

Employee was anchoring a piece of equipment with a "come- along" when the lifting mechanism broke causing the item to strike him in the left thigh resulting in a contusion.

Employee was attempting to remove a plastic cable tie from inside a haul unit when his box cutter slipped resulting in a laceration to his right thigh. Employee received 7 stitches as a result of this laceration.

Employee was tightening bolts on the primary plant, when he lost his footing due to muddy conditions. This resulted in him falling onto a conveyor which caused pain to his right knee.

Employee was using a prybar to install a roller on a dozer when he smashed his thumb between the roller and the equipment. At the time, the employee did not seek medical attention. It was not until 10/23/09 when swelling was apparent in his thumb that the employee wished to see a medical professional.

Employee alleges that while removing head bolts on a piece of equipment he hurt his back. Sought medical on 10/12/09 because he thought the pain in his back was a kidney infection.

Employee was using a small sledge hammer on wedges when he missed, stricking his right index finger, resulting in a contusion and a subungual hematoma.

Employee was pulling a drive shaft bolt from under a loader, when his wrench slipped and hit him in the mouth causing a chip to his tooth.

EE was driving a pin out a sheave on a piece of equipment (dredge) when a piece of metal was lodged under the skin.

On 2/06/09 employee was removing bucket teeth with a hammer when he noticed a laceration on his left forearm. Employee used first aid to care for the cut. On 2/16/09 employee realized that laceration had become infected and sought medical treatment, at which point the injury became reportable to MSHA.

EE was using a hammer to loosen and remove "teeth" from a dragline bucket when a piece of metal came up and hit the employee in the neck causing a laceration.

Injured was assisting another employee with replacement of bucket teeth on trackhoe. The teeth were loosened by cuts made by torch. As other employee struck a tooth, a piece of the tooth shot away and embedded into injured's right leg.

Employee was hitting wrench with sledge hammer to loosen bolt on the dredge table when he missed the wrench, hitting his left knee.

While holding wrench on clamp, co-worker was tightening clamp with impact wrench when employee's thumb was caught between wrench and clamp resulting in laceration of thumb requiring stitches.

EE WAS REMOVING PIN FROM TRACK HOE BY HITTING IT WITH BAR SECURED BY CHAIN HOIST. WHEN THE PIN WAS RELEASED EMPLOYEES FINGER WAS HIT BY THE BAR, RESULTING IN FRACTURE OF LEFT LITTLE FINGER

EE WAS ATTEMPTING TO REMOVE TAIL PULLEY WHEN THE BAR SLIPPED AND HET EE IN THE RIGHT SIDE/RIB CAGE AREA

EE was assisting co-workers while tightening bolt on dragline. He was holding wrench on bolt when co-worker missed the bolt and hit EE's left hand with hammer.

Co-worker was hitting part with hammer when a piece of the hammer chipped off and embedded into employee's right hand.

EE was changing out roller on conveyer when the wrench slipped, causing ee to hit thumb against angle iron, resulting in laceration to tip of left thumb.

EE WAS CUTTING A PIECE OF SKIRTBOARD RUBBER WHEN THE UTILITY KNIFE SLIPPED CUTTING EE's LEFT INDEX FINGER REQUIRING 5 STITCHES.

EMPLOYEE WAS DRIVING A WEDGE INTO SLOTTED PIN ON PRIMARY SHAKER WHEN THE PIN POPPED BACK OUT IN A TWISTING MOTION RESULTING IN A LACERATION OF THE LEFT HAND.

AFTER WELDING NUT ONTO BOLT, WRENCH SLIPPED, CAUSING HIM TO TOUCH HOT BOLT.

EE WAS HAMMERING A RIPPER TOOTH ON DOZER WHEN A PIECE OF THE TOOTH (STEEL) BROKE OFF AND EMBEDDED RIGHT THIGH.

EMPLOYEE AND COWORKER WERE REMOVING A SWING SHAFT OFF THE DRAGLINE. WHILE EMPLOYEE WAS HOOKING THE COME-A-LONG, THE SHAFT SLIPPED AND FELL ONTO EMPLOYEE'S FOOT, RESULTING IN CLOSED FRACTURE TO 1ST AND 2ND TOES OF LEFT FOOT.

EMPLOYEE WAS TRYING TO GET THE IMPELLER IN THE CRUSHER TO TURN. A CO-WORKER WAS PRYING ON THE IMPELLER WHEN THE PRY BAR SLIPPED AND HIT HIS KNEE.

EYE INJURY-MECHANIC WAS WORKING ON A MOTOR GRADER TRYING TP GET THE TILT WHEEL GEARS TO ENGAAGE. THE HAD THE COVER OFF THE GEAR BOX & WAS APPLYING PRESSURE BY THE USE OF A PRY BAR ON THE SHAFT TO HELP THE GEARS ENGAGE. WHEN THE GEARS DID ENGAGE A PIECE OF METAL SHERERED OFF ONE OF THE GEARS & HIT HIM IN THE EYE. IT WORKED ITS WAY BEHIND THE EYE & IT HAD TO BE SURGICALLY REMOVED.

EE HAD REACHED DOWN INTO THE JAW CRUSHER TO FREE A ROCK THAT WAS JAMMED, WHILE SWINGING A SLEDGE HAMMER HE STRAINED HIS BACK.

EMPLOYEE WAS SWINGING A SLEDGE HAMMER WHILE CHANGING THE THEETH ON A LOADER & STRAINED HIS BACK

INJ EMPL CHANGING DRILL STEM ON TRUCK -MOUNTED DRILL USING A HYDRAULIC WRENCH,IT SLIPPD CAUSING FINGER /HAND TO STRIKE BACK OF SHAFT HOUSING.

WHILE IN THE PROCESS OF REMOVING THE BARRING FROM THE WATER PUMP AND USING A HAMMER AND A PUNCH TO KNOCK OUT THE BEARING A PIECE OF METAL BROKE OFF. THE PIECE OF METAL LODG3D INTO HIS FINGER.

HE WAS REPLACING SPRINGS & PADS ON SHAKER - WHILE DRIVING PAN IN WITH HAMMER RUST FLAKE WAS POPPED IN LEFT EYE

HE WAS TIGHTENING ADJUSTMENT BOLT ON SPRINGS NO 1 TIPPLE ROLL CRUSHER & GOT RIGHT HAND MASHED BETWEEN WRENCH & CRUSHER FRAME
