Mechanics were removing a hub assembly on a piece of equipment. Employee slipped and EE's hand got in the path of the striking hammer. The hammer contacted the tip of EE's middle finger on EE's right hand.

Mostrando los 14Employee was preparing to change a roller on a 345 Track hoe by removing dried mud from the roller and bracket. A dead blow hammer was used to knock off the dried mud. In doing so, the hammer came in contact with the roller and a metal shard come off the roller and punctured the mechanics stomach.
A mechanic was working in the shop on a CAT triple 7 end dump. A mechanic was removing plastic zip ties from a transmission cooler hose with a folding knife with a locking blade. As the mechanic cut the last zip tie the pressured applied carried the knife toward EE's other hand, cutting the glove and EE's hand. The employee received stitches.
THE EE WAS CHANGING A BIT ON A BLAST HOLE DRILL. HE USED A 32" PIPE WRENCH ON HAMMER TO BREAK BIT LOOSE AND HIT HIS HAND WITH THE WRENCH.
EE WAS USING A WRENCH TO REMOVE BELLY PAN BOLTS ON DOZER. WRENCH BROKE.
THE EMPLOYEE AND THE WITNESS WERE REMOVING THE BELLY PAN FROM A CAT D10N DOZER. HE WAS USING A PRYBAR TO HOLD THE BELLY PAN IN PLACE SO HE COULD REMOVE A HINGE PIN. THE BELLY PAN SLIPPED OFF T HE PRY-BAR AND HIS LITTLE FINGER (RIGHT) WAS CAUGHT BETWEEN THE BELLY PAN AND THE PRY BAR.
THE EE WAS WORKING WITH A BAR AND SHOVEL CLEARING FROZEN MUD FROM DOZER TRACK FRAMES. THIS PRODURE RESULTED IN A BLISTER ON HIS LAND BETWEEN HIS THUMB AND FOREFINGER. TWO DAYS ENTER 11-17-94 T HE EE EXPERIENCED SIGN OF INFECTION AND WENT TO THE LOCAL E.R. FOR TREATMENT.
INJURED WAS REMOVING A STEERING VALVE ON A D10N WHEN HIS WRENCH SLIPPED. HE JERKED HSI FACE TO ONE SIDE AND WHEN HE DID HE HIT THE FLOOR PLATE CUTTING HIS FACE JUST ABOVE HIS LEFT EYE.
EMPLOYEE WAS WORKING WITH A DRILL STEEL & PIPE WRENCH SLIPPED & CAUGHT RT HAND BETWEEN WRENCH & DRILL DECK SMASHING & CUTTING RT RING FINGER
THE EMPLOYEE WAS DOING MAINTENANCE ON A CAT DOZER THE WRENCH (END) HE WAS USING SLIPPED OFF THE BOLT HE WAS TURNING & HE BANGED HIS ELBOW ON THE DOZER FRAME THE ACCIDENT WAS REPORTED ON 06-07- 94
EMPLOYEE WAS INSTALLING A BEARING ON A DRIVE SHAFT. HE WAS USING A HAMMER AND PUNCH AND APPARENTLY GOT A SLIVER OF METAL FROM THE PUNCH IN HIS WRIST. HE REMOVED THE SLIVER, BUT AWAKE TH ENE XT MORNIGN WITH REDNESS AND SWELLING.
EMPLOYEE WAS REMOVING BEARING WITH A PRY BAR FROM THE FRONT AXLE OF A HAYES ORE TRUCK (146.001) PRY BAR SLIPPED AND EMPLOYEE SPRAINED HIS BACK
THE EMPLOYEE WAS CLEANING A CHUTE ON THE CRUSHER WITH A BAR. HIS HAND (RIGHT) HIT THE EDGE OF THE CHUTE, WHICH CUT THE BACK OF HIS HAND.
EMPLOYEE WAS INSTALLING AN AXLE ON A HAUL TRUCK A PRY BAR BEING USED TO PRY THE AXLE INTO THE DIFFERENTIAL RECEIVED A BURN ON THE BOTTOM PORTION OF THE BAR AS THE BAR WAS PULLED OUT WITH THE N IGHT HAND THE BURR LACERATED THE LEFT RING FINGER WHICH WAS GUIDING THE BAR
