Employee's right thumb was caught between a post hole driver and a ground rod while driving rod into ground. Employee received a laceration which required medical attention and stitches.

Mostrando los 5Employee was conducting scheduled cleaning of Plant. EE was using a general purpose hammer to clear dried material off the conveyor frame. As EE hit the frame, the hammer ricocheted off striking EE's opposite (left) pinky finger. EE received a laceration and was taken to the clinic. EE did not receive sutures however, EE was prescribed an antibiotics.
Heavy lifting may have caused a hernia while performing maintenance tasks at the Rock Plant. The employee did not report the injury to his supervisor until 8/20/12, at which time he was taken immediately to Simi Health Center, where he is currently being referred to a surgeon for treatment. Until then, the employee is on restricted duty with no lifting over 15 lb.
While permorming maintenance work on belt press #2, the employee reached out to place a come-along around a pulley and strained his back.
THE OPERATOR WAS REPAIRING THE BAGHOUSE SCREW CONVEYOR. THE WRENCH SLIPPED, STRIKING HIM ABOVE THE LEFT EYE.
