Associate was replacing some rubber with a safety knife. As the associate was scoring the bucking rubber the safety knife kept getting stuck. They squeezed the handle 3 more times, & on the 3rd attempt the blade popped out and was too close to their inner thigh causing the blade to cut through pants leaving a ¾ of an inch cut in their thigh.

While swinging a sledgehammer onto a crusher head, a metal fragment broke off of the crusher & flew into the associate's neck.

While removing a spring in the tunnel feeder with a wrench, associate's hand slipped & the force caused the associate to hit mouth.

While removing hand railing & using a wrench to loosen the bolt on the railing. As the associate with the ratchet loosened the bolt the wrench started to lose contact with the railing. EE reached in to reposition the wrench as the other associate pulled/pushed on the ratchet causing EE's finger to get pinched between the railing and the wrench.

ASSOCIATE WAS REPLACING A TOOTH ON A CONCRETE MUNCHER. WHILE HOLDING THE RETAINING PIN WITH LEFT HAND & SWINGING A HAMMER WITH THE RIGHT, EE GLANCED OFF THE PIN & HIT LEFT HAND.

While using the sledgehammer to tighten the mantle nut on the cone crusher head, employee noticed pain in left forearm.

When cutting a lifting eye in the welding shop, EE knocked the center piece free with a chipping hammer, the hot metal fell on EE's pant leg, burned through the material and fell into the top of EE's boot.

Associate was attempting to dislodge a rock from a conveyor with a 24" bar. The associate's hand got pinned between the bar & the conveyor.

Employee was working from man basket, removing messenger wire for ventilation tube, during this process a piece of metal debris got into eye. Employee noticed some irritation later in shift but didn't start hurting till next day. Employee went to local clinic and got piece of debris removed from eye.

EE was struck in the forearm with a metal fragment from the tip of a grader that became dislodged when another associate was removing the end with a 3 pound hammer. The incident occurred on 8/31/17 & was first aid only. After a few months, the embedded object became infected & resulted in a medical procedure to remove the object at the doctor's office on 12/8/17.

Employee was cutting bolt holes with a utility knife, when changing a rubber curtain underneath a crusher. The knife slipped off & punctured employee's leg.

While using a steel shaft to drive pins from a rock breaker, employee was holding the shaft down on to a jack stand that supported the shaft. Another associate was using a sledge hammer to drive out the pins. The associate missed the shaft & struck employee's left hand which fractured a bone.

WHILE TURNING AROUND TO LOWER THE NEW SHIM INTO THE JAW, THE HOOK TOOL CAME OUT OF THE LIFTING EYE OF THE SHIM & ABRUPTLY STRUCK THE ASSOCIATE IN THE MOUTH.

Employee was working in the basket of the Man lift. He was using a pry bar to force a wing pulley back into the frame. As he was applying force to the pry bar it slipped from the corner of the GTU frame causing the tip of his left hand index figure to get pinched/smashed between the pry bar and the wing pulley.

Employee was using a hammer to remove a bolt that had been cut from a loader bucket. While hitting the bolt with an upward motion he sustained a sharp pain in his right wrist, resulting in a sprain. Associate waited to report the injury because he thought it would stop hurting.

Associate was changing teeth on a 990 loader bucket. While striking the tooth with a sledgehammer, a piece of metal chipped off of the top of the tooth and struck his left leg.

While swinging a sledgehammer with his less dominant hand to knock a pin into the bucket of the excavator, the associate felt a sharp pain in his hand.

Associate was cutting a piece of flashing with a utilty knife(box cutter). When the blade reached the end of the rubber, and with the force the associate was using, the blade slipped causing a puncture wound on the associate's upper left leg near the knee (inside thigh). Causing the associate to receive one(1) suture and three(3) staples. The assocate was cleared to return to work.

Miner using a chain hoist to reposition a feeder pan. Chain slipped and miner fell but was caught by fall arrest system.

Driller was using a hand held hammer and striking the top head of a down the hole hammer to loosen & remove. a mm size piece of steel went into his left wrist.

Associate was using a bar to clear fines from a chute when a significant amount of material broke free forcing his arms downward injuring his shoulder

An associate was attempting to close a valve at the back of a log washer with a 2 lb. hammer. When the associate swung the hammer to set the wedge he felt pain in his back.

Two associates were handling a handgun when it accidentally discharged injuring both associates. No serious injuries occurred. Both associates were transported to the hospital and subsequently released.

Associate was removing a liner from the crusher bowl using a sledge hammer. Pulled muscle in his back on the upswing of the hammer.

Tightening the head nut on the 1560 Omni Cone crusher (P01-0273) at the Bull Run Plant. This procedure requires the use of a sledge hammer. He was swinging the sledge hammer against the tightening tool and when he made contact, the sledge hammer bounced back, hurting his wrist.

Tightening the 5 1/2 shorthead crusher and lid that covers the adjustment controls fell closed and contacted his wrist.

Employee was trying to remove a piece of metal that he had cut from the old jaw structure. He was moving it with a digging bar. He turned at the waist and pulled up on the bar and felt his lower back pull. The metal was 4'x 6' x 3/4".

Associate pulled a muscle in his back while operating a chain jack. His torso was twisted slightly and his back was misaligned, while he was pulling on the chain jack handle.

Employee was hammering on a rock (upper) jaw when dirt fell out of the jaw hitting him in the face. Employee was wearing safety glasses but dirt still got into the employee's eye. The crew was removing the spacer plate from the jaw.

Associate, replacing broken spring on FMC feeder in Tunnel A at 3M secondary, was using chain come-a-long. While pushing comealong, pulled muscle in back.

Associate changing teeth on bucket of machine using small sledgehammer, when struck metal against metal, shard broke loose and lodged in his left arm.

While fabricating steel column. Plate that was tack welded on wrong location was being removed with a 2 pound hammer. The hammer slipped out of one man's hand and went about 10 feet and struck another man on the forehead.

Employee was using long bar to dislodge a slab of material from Lokolink 1, rock rolled down conveyor, pinching right pinkie finger between bar and rock, 2 stitches required.

Cleaning chunks of ammonium nitrate out of the hopper on a repump unit, cut right arm and hand on the edges of the hopper.

Associate using sledgehammer to release a bearing on the 1311 Jaw crusher, missed smashed finger.

Associate was cutting a plastic strap from a box of blasting caps. The knife had a new blade and cut thru the strap with more force than expected. Sliced thru associates pants into leg requiring 4 stitches.

Assoc. trying to remove socket from bolt which was stuck on bolt. Was holding bolt with hand and hitting socket with hammer, missed struck left pinky finger

Associate was removing a bolt on tool box from the bed of an out of service vehicle. There was a great deal of pressure on the bolt and when the wrench slipped off, associate lost his balance and fell to the ground breaking his left wrist

Associate was cutting leadline to hook up a toe shot to a production shot and was not cutting away from his body.

Associate changing tooth on 988G loader bucket, was having trouble getting tooth on so hit it with hammer, piece of metal broke off and lodged in associates abdomen.

Associate assisting with install of tailgate on 65 ton haul truck, was connecting the chain link assembly, was striking with 2lb hammer to close link and struck left thumb behind knuckle

EE WAS USING A 1 1/2" WRENCH TO TIGHTEN A MUD HOG (BELTSCRAPER) (EE WAS WORKING OVERHEAD) THE WRENCH SLIPPED OFF AND CUT EE ON UPPER LIP ON RIGHT SIDE.

EE WAS CUTTING A WEAR PLATE IN PRIMARY FEEDER. ANOTHER EE WAS APPROX. 4' AWAY USING A 3LB. HAMMER TO DRIVE BAG UNDER PLATE, WHEN THE SWING HAMMER SLIPPED FROM GRIP STRIKING EE OVER HIS EYE.

ASSOC INSTALLING IDLER ON CONVEYOR USING 6 FT PRY BAR, BAR SLIPPED, MASHED PINKY FINGER BETWEEN PRY BAR & BEAM UNDER JAW CRUSHER.

EMPLOYEE WAS WATCHING ANOTHER EMPLOYEE DRIVE A PIECE OF STEEL WITH A SLEDGEHAMMER. PIECE CHIPPEDOFF STRIKING EMPLOYEE.

EE WAS REMOVING A WEAR EDGE FROM LOADER BY STRIKING EDGE WITH A HAMMER. WHEN THE EE STRUCK THE WEAR EDGE A SMALL PIECE OF METAL FLEW OFF & IMBEDDED ITSELF IN THE EE'S LEG.
