EE was trying to pry a rock out of jaw unit and smashed EE's finger with pry bar and the rock.
EE was trimming conveyor belt using a utility knife. The belt cut easier than EE expected, resulting in the knife travelling past the conveyor belt and making contact with the EE's leg. The contact resulted in a laceration requiring sutures.
During Teardown of the loadout bunker one of the legs got hung up. They were using a pry bar to release the tension, the bar slid off the leg and it slid down on the pedestal. EE had put hand on the pedestal to move it as the bar slid off crushing EE's 4 fingers.
Employee was trying to remove a drill steel from a 988H loader bucker which was wedged in the bucket. He hit it with a hammer and it sprung out hitting him in the ankle. The doctor did an x-ray 11-12-2013 and gave him time off until follow up appt. on 11/18/13. At that time, the Dr returned ee to light duty with anther follow up appt in 10 days.
Miner was tightening a loose bolt. The ratchet slipped off the bolt causing the miner's right finger to be smashed.
EE was using a pry bar to put a screen in place. The bar slipped and cut his lip.
Employee was welding liner in the truck bed of 769 cat wagon. He was using a hydraulic porta-power to wedge metal in place to get metal tight to box when porta-power became unlodged and sprang up hitting his upper right lip dislocating a tooth.
EMPLOYEE WAS PREPARING A CONVEYOR BELT FOR SPLICING & WHILE CUTTING THE EDGE HE SLIPPED, CUTTING HIS LT. INDEX FINGER.
EE WAS CLEANING THE RETURN ROLLERS ON THE JAW UNDER CONVEYOR WITH A HAMMER
USING CHOP SAW TO CUT A PIECE OF KEY WAY STOCK. WHEN BROKE OFF KEY WAY, SHARP EDGE CAUGHT RIGHT PINKIE AND TORE OFF NAIL AND RIPPED END OF FINGER.
EE WAS USING A TORQUE MULTIPLIER TO TIGHTEN THE BLOW BARS WHEN IT FELL OFF THE HEAD OF THE BOLT AND HE TRIED TO CATCH IT BUT IT FELL ON HIS LEFT LITTLE FINGER.
EMPLOYEE WAS HOLDING A STEEL POST WHILE OTHER EMPLOYEE WAS DRIVING IT WITH A SLEDGE HAMMER. HAMMER MISSED POST AND STRUCK FINGER OF EMPLOYEE HOLDING POST.
EE WAS TRYING TO REMOVE A ROCK WEDGED BETWEEN BELT AND CONVEYER USING A SMALL PRY BAR. THE BAR LOST ITS BIT ON ROCK AND SLIPPED FROM HIS HANDS HITTING HIM IN THE CHIN. HAD SMALL CUT ON RIGHT C HIN NEEDING 4 STITCHES. RETURNED TO WORK TO FINISH THE DAY OUT.
EMPLOYEE WAS DRIVING A BONDING GROUND ROD INTO GROUND AND SMASHED LEFT THUMB WITH HAMMER.
EE WAS SHOVELING BY THE JAW UNDER CONV. & PULLEDA MUSCLE IN HIS R SIDE OF HIS BACK
