Employees were rotating the blow bars in the HSI. This requires hitting with blow bar wedges to loosen them up to remove the blow bar. While hitting the blow bar wedges with a hammer, the wedge came loose and hit injured employee's left middle finger and nail.

EE was using a bar to rotate conveyor belt. Bar slipped, EE stumbled and felt EE's knee pop. EE moved aside, within a minute or two, EE indicated to other employees EE might pass out. EE did pass out for 15-30 seconds. Other employees caught EE before EE fell. EE was taken to Emergency Room. Diagnosed with a sprained knee.

Employee was using a pocket knife to cut a zip tie from underneath a wheel loader, while performing an oil change, and the knife slipped and cut EE's left index finger.

Employee was working in the shop on lining a chute with polyurethane sheet material. While fitting a piece of the polyurethane into the chute, the employee was using a utility knife to trim one of the edges of sheet when the utility knife slipped and cut the employee's right index finger and part of EE's palm.

The employee was tightening a bolt on a dredge pump when the wrench EE was using slipped, causing EE to hit their hand on the pump and / or was struck on the wrist by the wrench.

In work boat tied off to the dredge, pulling on 3/4" ratchet wrench to tighten 7/8" diameter bolt. On 9/30/15 went to clinic because of low back pain.

A hydraulic ram was being used to push a shaft out of a pulley when the ram slipped off the shaft, as a result of this the spacer separated from the ram bounced off pulley then struck the employee under his chine, requiring three stitches.

EE was holding a steel tubular spring with a apair of vise grips while another EE attempted to cut the steel with a hacksaw. The hacksaw slipped and cut the EE's hand resulting in four stitches. Both EE's were drug tested per co. policy. The injured EE tested positive for illegal drugs and was suspended until a 2nd and conclusive drug test is completed in approx. a week.

Employee was pumping on jackhandle when the jack kicked out & metal stand hit him in the head.

Employee cut the base of his left thumb with a utility knife when cutting a conveyot belt.The employee pulled the knife towards his body when the knife cut loose from the belting & came in contact with his left hand and cut through his leather glove. Five stitches were required and the employee returne to work the same day.

Tightening bolts on a piece of pipe using a ratchet. Right hand turned red and was sore. Sprained right hand from stretching or tearing ligaments while tightening bolts.

A 1 1/2 TON CHAIN COME-ALONG MALFUNCTIONED WHEN SWITCHING IT TO DOWN POSITION WHILE LOWERING IMPELLER ON DREDGE. FINGERTIPS WERE CAUGHT BETWEEN SUPPORT & POLE CAUSING BROKEN TIPS ON RING & MID DLE FINGERS OF RIGHT HAND.

WRENCH SLIPPED FROM NUT HE FELL BACK AND HIT HIS TAIL BONE ON A BRACKET. HE HAD PAIN IN LOWER BACK NEAR TAIL BONE.

PLACING BEAM UNDER THE DREDGE LADDER USING A CABLE COMEALONG. BEAM BECAME STUCK. HE PULLED UP ON THE CABLE TO FREE UP BEAM. INJURED LOWER BACK. 4TH & 5TH DISC IN BACK STRAINED.

REPLACING DIGGER LINK CHAIN ON DREDGE WAS DRIVING PIN OUT WITH HAMMER WHEN HAMMER HIT ROLLER CUASING ROLLER TO BOUNCE & HIT THUMB LEFT THUMB WAS SPLIT OPEN & BRUISED

EMPLOYEE WAS INSTALLING A SCREEN IN A ROTARY TUB, WHEN TIGHTENING A BOLT, THE SCREEN PANEL SLIPED AND POKED A WIRE IN HIS LEFT FOREARM.

EMPLOYEE WAS CUTTING A PIECE OF ANGLE IRON, THE ANGLE IRON WAS RESTING ON A SAWHORSE AND IT FELL OFF HITTING HIS LEFT FOOT AND TOES, THE LITTLE TOE IS BORKEN AND THE OTHER TOES ARE CRACKED AND BRUISED.

EMPLOYEE WAS HELPING REPLACE WIRES IN A HARP SCREEN, WHEN TIGHTENING A WIRE DOWN, IT SNAPPED UP AND HIT HIM ON ONE OF HIS FRONT TEETH, BREAKING OFF A PORITION OF THE TOOTH.

CHANGING 12" PI1E WHEN CLAMP WAS LOOSENED PIPE JAW SUDDENLY CAUSING BOAT TO PITCH. INJURED GRABBED PIPE TO STEADY HIMSELF CUTTING HAND ON PIPE FLANGE.

EMP WAS UNLOADI3G BOTTOM HOPPER RAIL CARS CONTGROL CHAIN ON CAR WAS TANGLED ON THE ROLLER WHEN HOPPER DOOR OPENED THE CHAIN RELEASED SUDDENLY CAUSING THE WINDING BAR TO SPIN STRIKING EMP ON RT ARM 3

INJURED EMPLOYE9 WAS TIGHTENING A BOLT WHEN THE WRENCH SLIPPED AND HE FELL AGAINST GUARD RAIL AND CRACKED A RIB ON LEFT SIDE

EMPLOYEE WAS TIGHTENING A PIPE LINE BOLT USING A 3/4" DRIVE RATCHET WHEN RATCHET SLIPED STRIKING HIM ON FORHEAD.

PRYING ON BEARING WITH PRYBAR WHEN BAR SLIPPED AND STRUCK HIM ON FOREHEAD 3 SUTURES TO CLOSE WOUND
