On 8/27/12 the employee was cut on his elbow when the nut he was trying to loosen broke free & his arm hit the frame of a machine. He received 1st aid & returned to work. On 8/28/12, while using a wrench to turn a thread-tapper & compensating for the pain in his elbow, the wrench slipped, glanced off his safety glasses & struck him in the forehead, causing another laceration.

Two employees were shoveling trash into the back of a garbage truck. One employee lifted his shovel as the other was leaning down and struck him in the forehead with the shovel.

Employee was chipping slag off of a weld. A piece flew up under his safety glasses and lodged in his eye.

The employee was using a hammer to dislodge a rock from a grate. A rock fragment broke off and stuck into his hand between his index and middle finger.

Employee was using a bar to lift a feeder into place. He strained too hard and injured his groin.

Employee was attaching a chain to a piece of equipment, preparing to move it. He pinched his thumb between a pulley and the lifting chain.

While using a utility knife to cut rubber conveyor belt, the knife slipped out of a groove in the rubber causing a laceration to the employee's left thumb. The wound required three stitches, x-ray and tetanus shot.

EMPLOYEE WAS SWINING A 10-16 LB SLEDGEHAMMER IN A BENT OVER POSITION WHILE HELPING TO CHANGE A BEARING ON THE WASH PLANT.
