On 8/27/12 the employee was cut on his elbow when the nut he was trying to loosen broke free & his arm hit the frame of a machine. He received 1st aid & returned to work. On 8/28/12, while using a wrench to turn a thread-tapper & compensating for the pain in his elbow, the wrench slipped, glanced off his safety glasses & struck him in the forehead, causing another laceration.
