Employee was attempting to place a bearing onto a shaft by hammering bearing onto shaft. This resulted in a piece of metal shard embedding itself into the left wrist area of the employee.

Mostrando los 17Employee was removing a carrier roller and support block from a 336 Caterpillar Machine. To remove the block the employee was using a metal punch and hammer. When the hammer stuck the punch a shard of metal chipped off striking the employee's chest.
EE was removing a head pulley bearing at 103 VSI plant. After removing the bearing housing and shaft sleeve, EE was hitting it out with short handle anvil hammer. While tapping it out, a piece of shrapnel from the hammer struck EE, penetrating EE's glove and right index finger. This caused a laceration and fracture of EE's finger.
Supervisor was using a hammer to remove bearings for the wash plant shaker screen, when the hammer struck a bearing directly. The bearing shattered and some of the splintered pieces struck EE as EE was standing approximately 12' behind the supervisor. One piece struck EE's chin and another embedded in right forearm.
Cutting rubber to form a conveyor belt skirt, the knife slipped and cut employee's left hand.
Employee was trimming a belt wiper with a utility knife. The knife slipped lacerating EE's left wrist. 3 stitches required to close wound.
Employee was using utility knife to cut notches in skirt board rubber for a conveyor. Employee was cutting towards body when knife slipped and hit employee in the leg cutting it. Cut required 4 stitches.
Opening crusher door on VSI crucher, employee was pushing up from under side of door while other employee was lifting from top side with rock bar. The bar slipped, the door started to come down, employee tried to pull hand out of the way but the door caught pinkie of right hand pinching and lacerating finger.

EE WAS REPLACING AIR CLEANER ON D8N DOZER. SCREWDRIVER SLIPPED AS HE PRIED ON GASKET AROUND AIR CLEANER. SCREW DRIVIER STURKC HIM THE RIGHT EYE.
INJURED WAS PRYING A ROCK OUT OF A CONVEYOR BELT WITH A STEEL BAR. THE ROCK SUDDENLY CAME FREE AND THE BAR AND HIS HAND STRUCK THE CONVEYOR FRAME PINCHING HIS FINGER.
INJURED STRUCK 5 HAMMERMILL HAMMER WITHA H AMMER. A SMALL PIECE OF STEEL CHIPPED OFF THE HAMMERMILL HAMMER AND ENTERED THE FLESH OF HIS LEFT LOWER LEG.
