Shop employee was attempting to remove a trailer tire from the front axel of a tanker truck with a sledge hammer. The sledge hammer bounced back and pinched the employees hand against the fender of the truck causing a 1/2" laceration that required 2 stiches.

Mostrando 50 de 51While removing parts from a cone crusher, a pry bar was being used to spin a part, the bar slipped causing the employee to slip landing on their left knee causing injury.
Employee was cleaning out a chute with a bar and slipped and hit arm on a piece of angle iron resulting in a cut on left forearm that needed stitches.
While unjamming jaw crusher employee was using a digging bar to try and knock the rocks down out of the chute. EE made a swing at a rock with the bar and missed, the momentum caused the employee hand to come in contact with the chute. EE's hand got crushed between the bar and the chute.
Another company came to tighten a bolt on the quick coupler of a CX350. They tried with a swivel head ratchet on the nut end & an air impact on the head of the bolt. It didn't work. Then switched places & our EE was holding the ratchet with a hand on the head of the bolt & tried to loosen it, the pivot pin in the ratchet broke & smashed EE's left thumb.
EE was attempting to remove a pin out of a snow plow assembly on a triaxle with a 5 lb hammer and punch. EE struck the punch twice and the pin did not move. On the third attempt, EE swung the hammer with more force and missed the punch and the hammer struck the top of EE's left thumb. Left thumb is swollen, but fortunately no broken bones.
While attempting to unsecured a de-engerized electrical cord that was held by zip-ties, employee cut towards himself with the knife feature of the Leatherman that he was using. After the zip-tie was cut, the momentum of the knife took the blade accidentally into employees's left forearm. The laceration required 7 stitches.
They where changing the impact bars in our secondary crusher and a piece of metal flew off of the shoe that holds the bar in flying into the EE's leg on his right hip. Fine material will get packed in around the shoes and the EE was hitting the shoe with a 4 pound hammer to knock the fines out when a sliver flew off the side and was imbedded into his leg.
EE was in the process of removing a motor base bolt. The 24" crescent wrench slipped off the nut and struck EE's right hand. EE went to receive medical attention due to the amount of pain he was in. Luckily there was not broken bones or stitches required. The Doctor put EE on restricted duty; no lifting items greater than 5 pounds.
Employee was replacing a broken U-bolt on the rear suspension of a Triaxel. As he was hammering on the broken U-bolt after using a torch, a "bb" size piece of slag struck his left wrist and became embedded in his arm.
Installing rubber disc around spray bars on screen deck. Had to trim disc. Holding disc in place with right hand on the top scribbing line with knife in left hand resulting in a laceration to pointer finger on right hand. The laceration resulted in six stitches.

Employee missed his target with sledge hammer and his finger was pinched between sledge hammer and slider bed frame.
employee was standing cleaning out a chute with a bar. as he brought the bar down with some force to remove material he pinched his finger between the bar and the edge of the chute cutting the the end of his left little finger about three quarters of an inch.
A CHUTE WAS PLUGGED UP GOING INTO A 4 1/4 CRUSHER. I WAS POKING AT THE ROCKS WITH A BAR 4' LONG. WHEN THE LAST LITTLE BIT OF STONE CAME OUT 2 ROCKS HIT EACH OTHER AND ONE OF THEM FLEW OVER & H IT ME ON THE LEFT SIDE OF MY FACE. IT WAS A 10" ROCK. IT BROKE MY SAFETY GLASSES AND CUT ME JUST BELOW MY EYE.

LIFTING A PIECE OF METAL WITH COME ALONG. HOOK SLIPPED OFF OF METAL. HOOK STRUCK RIGHT RING FINGER, CAUSING A CUT TO RIGHT RING FINGER.
CUTTING HOSE WITH A UTILITY KNIFE, WHEN EMPLOYEEGOT TO END OF HOSE, THE KNIFE SLIPPED, AND CUT HIS LEFT HAND.
THE EE WAS REMOVING BOLTS FROM THE 966D, WHEN THE WRENCH WOULDN'T MOVE HE PUSHED ON IT WITH HIS FOOT. THAT IS WHEN HE FELT PAIN IN HIS LEFT LEG. HE DIDN'T SEEK MEDICAL ATTENTION UNTIL 2/23/98.
VICTIM WAS CHIPPING FROZEN MATERIAL OFF A PRIMARY FEEDER HOPPER FLOOR, WHEN THE BAR HE WAS USING, BOUNCED BACK STRIKING THE VICTIM IN THE CHIN, HE WENT TO THE HOSPITAL WHERE HE RECEIVED FOUR S TITCHES.
THE EE WAS SCALING THE RIB IN 16E CLOSE TO THE ROOF WHEN HE PULLED ON A PIECE AN UNSEEN PIECE FROM ABOVE LEFT LOOSE HITTING HIS RIGHT INDEX FINGER BETWEEN THE BAR AND THE STONE
THE EE WAS REPLACING BOLTS IN 1 STONE ELEVATOR WHEN HE WAS BURNING NUTS OFF OF THE BUCKET AND WAS KNOCKING BOLTS OUT WITH A SLAG HAMMER A PIECE FLEW OFF AND HIT HIM ON THE LEFT HAND BETWEEN TH E THUMB AND WRIST.
WHILE PUNCHING A 3" HOLE IN A MEATL BOX OVERHEAD THE KNOCK OUT PUNCH DROPPED OUT OF THE HOLE WHEN IT BROKE THRU AND FELL DOWN STRIKING EE IN THE MOUTH. THE KNOCK OUT PUNCH WEIGHED ABOUT 5 LBS.
WHILE BARRING DOWN ROOF FROM JLG MANLIFT, EE FELT A SHARP PAIN IN HIS LEFT SIDE OF HIS RIBS & BACK.
WHILE BARRING MUD OUT OF THE BACK OF THE PUG MILL, THE EMPLOYEE FELT SOMETHING PULL IN HIS RIGHT SHOULDER.
EMPLOYEE WAS CLEANING OUT THE STONE BIN WHEN HE STRUCK HIS FINGER WITH THE HAMMER TO BREAK THE STONE. HE CUT HIS FINGER, REQUIRING A FEW SUTURES.
EMPLOYEE WAS CLEANING CHUTE AT PULVERIZING PLANT WHEN HE FELT PAIN IN HIS NECK. HE HAS BEEN UNDER THE CARE OF A CHIROPRACTOR [4 VISITS].

REPLACING A BEARING IN CRUSHER. HOLDING A PUNCH THAT WAS TO PUSH THE BEARING IN WHILE HAMMERING WITH A SLEDGE HAMMER. THE HAMMER SLIPPED AND HIT HIS FINGER.
EE WAS PULLING ON A WRENCH TO TIGHTEN BOLTS IN EXHAUSTER DOOR LINERS, WHEN HE FELT SOMETHING PULL IN HIS RIGHT WRIST. HE IS PRESENTLY RECEIVING PHYSICAL THERAPY AND MEDICATION FOR HIS HAND WRI ST.
INJ WAS STRAIGHTENING CHUT WITH CLAMP. FOOT OF CLAMP SNAPPED OFF HITTING FOREHEAD HE REQUIRED 14 STITCHES.
EMPLOYEE WAS CUTTING INSULATION OFF OF WIRE. HIS KNIFE SLIPPED AND HE CUT HIS LEFT HAND.
PRYING ROCK OUT OF FEEDER, USING PRY BAR AND PULLED MUSCLE ON SHOULDER.
EMPLOYEE WAS USING A PIPEWRENCH ON SHAFT TO TURN FEEDER VALVE. THERE WAS A 1" PIECE OF METAL STICKING OUT ON GUARD. WHEN VALVE BROKE LOOSE, HE HIT HIS HAND ON PIECE OF METAL.
EMPLOYEE WAS SWEEPING THE FLOOR UNDER THE HYDRATOR, WHEN HE LIFTED HIS HEAD HE HIT AN ELECTRICAL SWITCH BOX AND JOLTED HIS NECK CAUSING PAIN IN HIS NECK AND BACK.
INJURED WAS WORKING ON #3 COOLER WIND BOX WHEN HE FELT PAIN IN LOWER BACK HE WAS POUNDING WITH A HAMMER INSIDE WIND BOX AT THE TIME
HE WAS CLEANING HOPPER WITH CLEANING TOOL WHICH IS A PIPE 10' X 4" WITH A BLADE ON END. THE HOPPER IS WORKING WITH THE SCREEN. TOOL IS USED TO CLEAN OFF DIRT AND MUD ON STEEL SIDES. HE WAS PUS HING WITH BOTH HANDS AND THE TOOL SLIPPED. HE SPRAINED HIS LOWER BACK.
EMPL WAS WORKING ON 25 TON CRANE TAKING OFF SNAP RING USING SCREWDRIVER. SCREWDRIVER SLIPPED CUTTING RIGHT INDEX FINGER.
