Mostrando los 8Employee was attempting to loosen a nut on the jaw crusher. When the nut broke loose, the 17/8 inch wrench fell out of his hand and struck his head above his left eye, which caused a small cut above his eye which required a few stitches.
EE was using a hammer to repair a roller pin and was putting the bearing back onto the pin. When he went to strike the pin with the hammer he missed the pin and hit his left left hand that was holding the pin.
EMPLOYEE WAS ASSISTING ANOTHER EMPLOYTEE WELDING ON A PIECE OF EQUIPMENT (LOADER). AS HE WAS TRYYING TO KNOCK OFF THE WELDING SLAG, A PIECE OF SLAG OR METAL WENT INTO HIS LEFT EYE. HE WAS WEAR ING EYE PROTECTION WHEN THIS INCIDENT OCCURRED. HE WENT TO AN OPHTHAMOLOGIST FOR TREATMENT. HE WAS TREATED AND RELEASED.
EE WAS SHOVELING MATERIAL UNDER THE #3 SCREEN DECK WHEN A ONE FOOT DIAMETER ROCK FELL OFF AND STRUCK THE EE ON HIS LEFT SHOULDER AND BACK.
ROCKS BECAME JAMMED IN CRUSHER. CRUSHER SHUT DOWN. FOUR MEN WERE WORKING TO LOOSEN THE ROCKS, SEVERAL PIECES FLEW OUT AFTER BECOMING DISLODGED, ONE STRIKING EMPLOYEE ON END OF HIS FINGER. WAS WEARING GLOVES.
WORKING AT CRUSHER, TRYING TO UNJAM THE JAW, USING A STEEL PRY BAR TOLOOSEN A PIECE OF ROCK, BAR CAME BACK HITTING HIS CHIN.
THE TOGGLE PLATE WAS BEING LIFTED OUT OF CRUSHER WHEN IT CAUGHT ON SIDE OF CRUSHER. IT CAME FREE CAUSING CHAIN IT WAS BEING LIFTED BY TO GO SLACK COMING UNHOOKED AND THE PLATE FELL TO THE CATW ALK STRIKING THE MAN ON HIS LEFT ARM CAUSING BRUISES.
