Employee states they were hand scaling and went to swing the hand bar at a rock when the bar slipped out of their hand and caught the right pinky finger between the hand bar and the top rail of the manlift basket. The pinky finger suffered a fracture.

Injured was using a wrench to tighten a bolt on a crusher. The wrench slipped off and injured's right palm struck the bolt head. The resulting laceration required 6 stitches.

Injured was using a sledge hammer to loosen wear parts on a VSI rotor. While striking a cast bar, a piece of metal separated from the bar and entered into the injured's abdomen. Injured went to urgent care and received 2 stitches.

Employee stated was using a pry bar to clear material from a chute when the bar slipped and employee caught a finger between the chute and the pry bar.

Employee stated ee was scaling in a man lift when the rock ee was prying on forced the pry bar down and caught finger between the pry bar and the top railing on the man lift basket.

Employee stated was cutting a rubber spacer with a utility knife when EE slipped and cut hand.

Employee stated was using a wrench to loosen bolts on a flange when the employee slipped and cut finger on a bolt head.

Employee stated was using a razor knife to cut piece of belting and cut leg with the knife.

Employee stated was scaling the highwall, pulled down a piece of rock, the rock struck the bottom of the man-lift basket, but the bar was still stuck in behind the rock which caused EE to strike ribs on the man-lift railing.

Employee stated was prying on a rock while hand scaling and felt a pain in back.

Employee stated was lifting steel plate into place with chain hoist, tied the plate off to re-hook, released the chain hoist, and the chain slipped causing the chain hook to strike the employee on the cheek.

The employee was performing maintenance at the jaw with a 24 inch crescent wrench and felt burning sensation in left bicep.

The employee was replacing teeth on a loader bucket when a small sliver of steel from the tooth struck the employee in the stomach.

The employee was using a bar to move a tail piece, tail piece slipped and fell on his foot.

Reports was releasing a stuck pin on the bucket of the bob cat with a scaling bar, pin released and the bucket swung forward, the bucket detached and fell, scrapping the front of his legs. Deep abrasion to his right leg approx 18" in length, and superficial abrasions to the left leg.

EE was aligning holes for bolts in a slide bed on a seperator bypass conveyor. While fitting steel plate EE struck left thumb with sledge hammer.

EE ALLEGES TO HAVE CUT HIS LEFT LONG FINGER, WHILE USING A PRY BAR.

Working on Stedman crusher - using a sledge hammer when it broke getting a splinter in palm of hand.

EE was using a 2lb sledge hammer attempting to remove a come-along safety latch that was outside the hook. He swung hammer and elbow hit guard rail causing hammer to miss latch and hit left hand causing laceration of left little finger.

Employee was cutting belt skirting with box knife when it slipped and cut left arm.

EMPLOYEE WAS PRYING ON LOOSE MATERIAL WHEN THE BAR SLIPPED, HITTING HIS HAND ON THE TRUCK FRAME.

Employee was prying on loose material that was clogged in one of the rotolocks when he felt a pull in his lower back.

EMPLOYEE WAS USING A SCALING BAR ON THE FACE WHEN THE TOP SECTION FELL ON THE SCALING BAR SPRAINING HIS RIGHT WRIST AND SHOULDER.

EE WAS PRYING OPEN A J-HOOK ON A BLASTING CAP WHEN THE KNIFE SLIPPED, CUTTING HIS LEFT INDEX FINGER (SURGERY REQUIRED).

EE WAS USING A 16 LB SLEDGE HAMMER TO TAKE HAMMERS OUT OF THE MILL WHEN HE STRAINED HIS BACK.

Employee was shoveling spilled material and throwing it out the hole that had been cut in the wall when the shovel hit the conveyor roller jamming his thumb.

A ROCK BECAME LODGED IN THE CONVEYOR BELT OF THE HEAD PULLEY OF THE 4"CONVEYOR. WHILE TRYING TO DISLODGE THE ROCK THE PRYBAR SLIPPED HITTING THE EMPLOYEE IN THE CHEEK, BELOW THE RT. EYE CAUSING A 1" LACERATION.

EMPLOYEE WAS PEELING THE PLASTIC WRAP OFF A CARDBOARD BOX WHEN THE TIP OF HIS FINGER CAUGHT THE OPEN BLADE OF THE UTILITY KNIFE. THE EMERGENCY ROOM DOCTOR GLUED THE LACERATION. THE NEXT MORNING WE SENT HIM TO THE DOCTOR TO GET THE WOUND CLEANED. THE DR PUT HIM ON RESTRICTED DUTY EVENTHO HE PERFORMED HIS NORMAL DUTIES.

Employee was removing a liner off separator shaft, while using a porta power hydraulic jack. The jack slipped & hit him in the elbow causing a laceration which required 2 d/t injections and 2 stitches and medication.

EMPLOYEE WAS STRIKING THE EDGE OF A BROKEN COUPLING, WHEN A SMALL PIECE BROKE OFF AND ENTERED EMPLOYEE'S LEFT LEG ABOVE THE KNEE. EMPLOYEE WENT TO THE DOCTOR WHERE HE RECEIVED SUTURES.

EMPLOYEE WAS USING A CLAW HAMMER TO DISMANTLE CONCRETE FORMS WHEN A PIECE OF DEBRIS ENTERED HIS LEFT EYE. HE WAS SENT TO A DOCTOR AND THE DEBRIS WAS REMOVED. A PRESCRIPTION OF ZYMAR WAS PRESCR IBED TO BE USED FOR 3 DAYS.
