Employee states they were hand scaling and went to swing the hand bar at a rock when the bar slipped out of their hand and caught the right pinky finger between the hand bar and the top rail of the manlift basket. The pinky finger suffered a fracture.

THE NEXT MORNING WE SENT HIM TO THE DOCTOR TO GET THE WOUND CLEANED. THE DR PUT HIM ON RESTRICTED DUTY EVENTHO HE PERFORMED HIS NORMAL DUTIES.
Employee was removing a liner off separator shaft, while using a porta power hydraulic jack. The jack slipped & hit him in the elbow causing a laceration which required 2 d/t injections and 2 stitches and medication.

EMPLOYEE WAS USING A CLAW HAMMER TO DISMANTLE CONCRETE FORMS WHEN A PIECE OF DEBRIS ENTERED HIS LEFT EYE. HE WAS SENT TO A DOCTOR AND THE DEBRIS WAS REMOVED. A PRESCRIPTION OF ZYMAR WAS PRESCR IBED TO BE USED FOR 3 DAYS.
