Lube mechanic was striking shovel bucket dig tooth with steel hammer to drive the tooth off. A small shard of steel from tooth was sheared off & imbedded into the EE's leg. After the injury, only 1st aid & medical exam was provided. Multiple Drs advised EE against removal of the small steel splinter. At a much later date (5-3-11) EE had it removed as elective procedure.
Employee was using a pipe hook to adjust some pipe on the pipe truck. When he pulled on the pipe his right shoulder popped.

EE was performing electrical connections at generator set when tightning the fitting with wrench the wrench slipped. EE injured left index finger as it was pinched between wrench and steel frame.
WHILE USING A 5'PRY BAR, TO MOVE A TIRE ON A GRADER, THE PRY BAR SLIPPED , EE LOST HIS BALANCE & TWISTED HIS LEFT ANKLE
WHILE DRIVING A BEARING ONTO A PUMP SHAFT WITH A HAMMER AND PUNCH EE MISSED THE PUNCH AND HIT HIS LEFT HAND.
HE WAS TAKING OFF TRANNY FILERS ON A DOZER.HE WAS USING A RATCHET WITH A CHEATER BAR HOOK TO IT. CHEATER BAR SLIPPED AND CAUGHT HIS HAND BETWEEN THE CHEATER BAR AND THE DOZER.
EE WAS CUTTING A PIECE OF PLEXIGLASS WITH A RAZOR BLADE UTILITY KNIFE. THE KNIFE RAN OFF COURSE AND CUT THE EE'S THUMB ON THE HAND THAT WAS HOLDING THE SHEET OF PLEXIGLASS.
EMPLOYEES WERE INSTALLING A 12" X 6" GUARD EXTENSION. EMPLOYEE WAS WELDING ON ONE END WHILE ANOTHER WAS TRYING TO GET THE OTHER END BOLTED. THE END (OPPOSITE THE END INJURED WAS WORKING ON) S LIPPED WITH THE GUARD HINGED ON THE ONE BOLT ALREADY INSTALLED SWUNG DOWN AND CAUGHT INJURED'S FINGER IN A PINCH POINT.
AN INEXPERIENCED HELPER WAS USING A SLEEVE WRENCH TO PULL OUT A SLEEVE ON OUR DRILL PIPE WHEN IT SLIPPED, SMASHED HIS SMALL FINGER.
HE WAS INSTRUCTED BY THE PLANT FOREMAN TO INSTALL A CAUSTIC DRAIN LINE FROM THE TANK TO THE BARREN POND. DIRT WORK WAS GOING TO BE NEEDED SO THE OLD LINE THAT HAD PREVIOUSLY PLUGGED NEEDED TO BE DISCONNECTED. THIS LINE WAS A 2" GRAVITY FEED LINE FROM THE TANK TO THE BARREN POND. ALL THE VALVES COMING FROM THE TANK WERE CLOSED. WHEN HE WENT TO DISCONNECT THE LINE BY BREAKING APART A
IGNOCIO WAS CLEANING A PUMP PART ON THE WORK BENCH AT THE PROCESS PLANT WITH A WIRE BRUSH AND EMRY CLOTH. WHILE HE WAS TURNING THE PART OVER TO CLEAN THE OTHER SIDE, HE CAUGHT HIS FINGER BETWE EN THE PUMP PART AND THE WORK BENCH, SMASHING HIS FINGER. THE PUMP PART WEIGHED ABOUT 25 POUNDS.
EE WAS USING A CHISEL AND HAMMER WHEN THE HAMMER SLIPPED OFF THE CHISEL AND HIT HIS KNUCKLE ON HIS LEFT FOREFINGER.
EMPLOYEE WAS CUTTING 1/2" PLASTIC DRIP LINE WITH A KNIFE AS HE PULLED THE KNIFE TOWARDS HIM TH EKNIFE SLIPPED AND CUT HIS THUMB.
EMPLOYEE WASTIGHTENING A BOLT ON A CONE CRUSHER. HE DID NOT MISS ANY WORK AS A RESULT OF THIS INJURY UNTIL 5-8-91. HE DID VISIT THE CHIROPRACTOR ON & OFF. HE CONTINUED TO PERFORM HIS REGULA R DUTIES WITHOUT ANY RESTRICTIONS.

CHANGING TEETH ON LOADER BUCKET REMOVED PIN HOLDING TOOTH STRUCK TOOTH WITH HAMMER CHIP FLEW OFF LODGED IN HIS THUMB
EMPLOYEE WAS LIFTING A MOTOR GRADER TIRE WITH A CHAIN THE CHAIN SLIPPED & THE TIRE FELL STRIKING THE EMPLOYEE ANKLE.
EMPL USING PORTA-POWER TO HELP INSTALL PIECE OF HEAVY STEEL LINER.STEEL MOVED FARTHER THAN EXPECTED.PORTAPOWER RAM LOST ITS GRIP FALLING DOWN HITTIN EMPL UNDERNEATH.
EMPLOYEE WAS TAKING NUTS OFF OF A FURNACE AS HE PULLED ON THE WRENCH HE TWISTED HIS KNEE AND FELT A SHARP PAIN .
EMP WAS REPLACI3G A CENTER PIN ON THE LEAF SPRINGS OF A SERVICE TRUCK THE TRUCK WAS SUPPORTED BY 2 BLOCKS AND A JACK UNDER THE MAIN FRAME AND THE SPRINGS BY A FLOR JACK. AS THE FLOOR JACK WAS LOWERED TO 9EADJUST THE OTHER JACK SLIPPED CAUSING THE AXLE TO FALL STRIKING EMPLOYEE
EMPLOYEE WAS TI1HTENING A BOLT WHEN HIS HAND SLIPPED, CAUSINGHIS ARM TO STRETCH AND HIS FOREARM TO BE STRAINED.
