The employee was holding a piece of wood to brace the metal while another employee was hitting in place. The other employee went to swing the sledgehammer getting it caught in the chain fall chains knocking EE off balance causing EE to hit the EE's hand.

Employee was mucking out the cone of 4cy10 vessel and was struck in the right shoulder by piece of falling debris.

Employee was using a crowbar to break rocks stuck in the vibrating screen. one rock broke easier than expected, causing to reach farther then intended and fingers were smashed between bar and screens.

Employee was pounding on a ripper tooth with a hammer trying to dislodge it from the ripper on a D11 dozer when a small piece of metal broke off the tooth and hit the employee in the lower leg. Mechanic was driven to the emergency room where it was determined to leave the metal in the leg and apply 2 stitches. The mechanic returned to full duty the next day.

Employee injured his left hand while doing repairs to a sledgehammer in the tool room. Employee was hammering a wedge into the top of sledgehammer to tighten the handle to the head, the sledgehammer was clamped in a vice, and he hit the base of his left thumb with a glancing blow while trying to drive the wedge in with a hammer. No broken bones, released for modified duty.

Employee was assisting in opening a rail car bottom dump door by attaching a come-along to the door to try to pull it open. The day after the event, he felt discomfort in the left side of his back. On Aug.20th the employee reported that his back was still hurting, the company arranged for a Dr's. appointment that day. The extent of the impairment is not yet known.

Employee was pushing upward with a long metal rod trying to dislodge a tire that was jammed up on the tire rollers (conveying system). In doing so, employee strained his lower back.

WHILE EE WAS ADJUSTING THE PULLER, HE PULLED HIS BACK MUSCLES CAUSING SPASM TO HIS MID BACK.

Rubber tires were moving along a roller conveyor. A tire became jammed against a steel beam, other tires continued to tighted up the row of tires. The injured employee attempted to 'unjam' the tires by pulling up on the first tire in the row. As he pulled on the tire with a long hooked bar, he strained a muscle in his lower back.

Employee was shoveling material from around a tail pulley, when a clump of material fell onto his foot, spraining his ankle.

Using sledge hammer, swung and missed piece of angle welded to shaft on screw for 4SC29. Pinched right middle finger between handle of sledge and the angle iron that was sticking up.

Employee was attempting to un-hook a com-a-long from a chute and cut his arm on the chute.

Swinging a sledge hammer at a wedge in a cone crusher and missed the wedge. While swinging through, he felt a sting in his upper left shoulder.

Hitting side of chute, smashed ring finger between sledge handle and fin on ball hopper chute. Employee not watching clearances.

Employee was cleaning out a chute that had build-up of material with a shovel. He stabbed at the build-up with the shovel and felt pain in his arm. Reported injury on 3/29/05.

Cut finger while using a knife to cut lunchmeat. Finger was cut to the extent that it was necessary to seek medical assistance. Stitches were required..

TIGHTENING BOLTS IN OSEPA INLET LOUVER AND STRAINED SHOULDER. *EMPLOYEE DID NOT SEEK MEDICAL ATTENTION AT TIME OF INCIDENT NOT REPORTED UNTIL RECENTLY.

USING BREAKER BAR TO UNCLOG FEEDER - RIGHT MIDDLE FINGER WAS SMASHED BETWEEN BAR AND RAIL.

WHILE REPLACING A WATER CONTROL VALVE. WRENCH SLIPPED ON FITTING WHILE REPLACING A VALVE, HITTING HAND ON STRUCTURE.

EMPLOYEE WAS OPENING VALVE ABOVE 5FK10 WITH PIPE WRENCH - PULLED ARM AND SHOULDER.

5/6/02 SPOKE WITH IRENE- EE WAS USING A SLEDGE HAMMER & OVER EXTENDED. REOCCURANCE OF OLD.

EE WAS HELPING REPLACE SCREENS, ATTEMPTING TO REMOVE A WEDGE WITH A 4LB. SLEDGE HAMMER IN A HARD TO REACH LOCATION. HE INADVERTANTLY HIT THE RUBBER DONUT. THE HAMMER BOUNCED BACK & HIT HIS FIN GER AGAINST THE SHAKER SCREEN BASE.

EE WAS TRIMMING SCREEN PANEL TO FIT. KNIFE SLIPPED AND CUT LEG ABOVE RIGHT KNEE.

EE WAS POKING DOWN WHEN DUST CAME THROUGH THE SAFETY EQUIPMENT INTO LEFT EYE.

CLEANING OUT A PLUGGED CHUTE AT A CONVEYOR TRANSFER POINT. MATERIAL BROKE LOOSE AND STRUCK EE ON THE ARM

EMPLOYEE WAS POKING OUT THE HOPPER. WHEN HALF THE BALLS WERE REMOVED FROM HOPPER, THE HOPPER LIFTED UP AND LANDED ON EMPLYEES FOOT THEN BOUNCED BACK UP.

WHILE CLEANING 4CD4 OVERFLOW PIPE MATERIAL FROM TOP DISLODGED AND CAME DOWN PIPE AND EXITED THE CLEAN OUT HOLE STRIKING EE ON RT SIDE OF FACE, HOT MATERIAL CAUSED SOME BLISTERING.

SHOVELING DIRT. DIRT POSSIBLY GOT IN EAR.

SHOVELING- CRACKED HANDS AND FEET.

SHOVELING MATERIAL OUT OF 40DC-1. LEFT THUMB SORE COULD NOT BEND IT.

ASSISTING WORKING ON SCREW CONVEYOR, STANDING NEXT TO ANOTHER EMPLOYEE SWINGING A HAMMER TO HIT A BOLT SLIPPED OUT OF HIS HAND AND HIT THE INJURED EMPLOYEE IN THE GROIN.

EE WAS PRYING ON A ROLLER WITH A PRYBAR WHEN HIS FINGER GOT PINCHED BETWEEN PRYBAR AND THE BELT FRAME.

A ROCK WAS STUCK IN THE JAW CRUSHER. PLANT FOREMAN SHUT DOWN CRUSHER. THE PLANT OPERATOR TRIED TO PRY OUT THE ROCK, WHEN ANOTHER ROCK BECAME DISLODGED AND FRACTURED HIS LEFT FOOT.

WHILE CLEANIGN CHUTES WIH A SHOVEL, A ROCK CAM ELOOSE AND STRUCK EMPLOYEE IN RIGHT ELBOW.

EE WAS CUTTING OFF PLASTIC X TABS FROM THE PLASTIC SCREEN MODULES WHEN THE KNIFE SLIPPED AND CUT HIS HAND.

EE WAS TRYING TO DISLODGE ROCK FROM FEED OPENING OF DOZER TRAP USING A PRYBAR.

EMPLOYEE WAS PICKING UP METAL SCRAPS WHE HE RECEIVED AN INSECT BITE. DID NTO REPORT INCIDENT UNTIL LATE.

EMPLOYEE WAS ATTEMPTING TO CHANGE RIPPER TEETH ON D-9-N WHEN STEEL SLIVER BROKE OFF & HIT EMPLOYEE IN LEG.

HOLDING DRIVE CHAIN WITH A BAR WHILE FELLOW EMPLOYEES WERE BRINGING CHAIN TOGEETHER THE BAR SLIPPED CAUSING EMPOLOYEE TO FALL TO DECK 5FT BELOW

WHILE TRYING TO DISLODGE ROCK BEHIND THE PLATE OF THE C-2 HEADBOX, THE ROCK CAME LOOSE AND STRUCK CLAIMANTS HAND AGAINS WALL OF HOPPER.

EMPLOYEE ALLEGES 1982 INJURY REOCCURRED 2-15-93. NOW CLAIMS NECK IS BROKEN AND BLOOD PRESSURE. IN 1982, WAS BREAKING LOOSE BOLTS WHEN HE FELT PAIN IN RIGHT SHOULDER.

EE WAS SWINGING METAL BAR TO DISLODGE MATERIAL ON GATE 3GA1A. BAR BOUNCED OFF OF THE MATERIAL STIRKING AND SMAHSED FINGER AGAINST GRATING ABOVE THE GATE.

EMPLOYEE WAS USING SLEDGE HAMMER TO PENETRATE ROCK TO PUT IN GATE TO CLOSE OFF FEED. EMPLOYEE MISSED THE GATE AND HAND HIT GATE.

EE WAS USING PRY BAR TO REMOVE A ROCK FROM THE CONVEYOR BELT TAIL PULLEY SECTION. BAR SLIPPED OUT FROM UNDER ROCK CAUSING FINGER TO MAKE CONTACT WITH THE FRAME OF THE CONVEYOR.

CHAIN HOIST FROZE UP WHILE IN THE PROCESS OF ADJUSTING 4' CRUSHER.EMPLOYEE PULLED HARDER ON HOIST AND CHAIN BROKE.EMPLOYEE LOST FOOTING AND FELL BACK LANDING ON A ROCK.NOTE: EMPLOYEE DID NOT SEEK MEDICAL TREATMENT UNTIL 11-12-91.
