Employee was using a crowbar to break rocks stuck in the vibrating screen. one rock broke easier than expected, causing to reach farther then intended and fingers were smashed between bar and screens.
Employee was pounding on a ripper tooth with a hammer trying to dislodge it from the ripper on a D11 dozer when a small piece of metal broke off the tooth and hit the employee in the lower leg. Mechanic was driven to the emergency room where it was determined to leave the metal in the leg and apply 2 stitches. The mechanic returned to full duty the next day.
Employee injured his left hand while doing repairs to a sledgehammer in the tool room. Employee was hammering a wedge into the top of sledgehammer to tighten the handle to the head, the sledgehammer was clamped in a vice, and he hit the base of his left thumb with a glancing blow while trying to drive the wedge in with a hammer. No broken bones, released for modified duty.
Employee was assisting in opening a rail car bottom dump door by attaching a come-along to the door to try to pull it open. The day after the event, he felt discomfort in the left side of his back. On Aug.20th the employee reported that his back was still hurting, the company arranged for a Dr's. appointment that day. The extent of the impairment is not yet known.
Employee was pushing upward with a long metal rod trying to dislodge a tire that was jammed up on the tire rollers (conveying system). In doing so, employee strained his lower back.
WHILE EE WAS ADJUSTING THE PULLER, HE PULLED HIS BACK MUSCLES CAUSING SPASM TO HIS MID BACK.
Rubber tires were moving along a roller conveyor. A tire became jammed against a steel beam, other tires continued to tighted up the row of tires. The injured employee attempted to 'unjam' the tires by pulling up on the first tire in the row. As he pulled on the tire with a long hooked bar, he strained a muscle in his lower back.
Employee was shoveling material from around a tail pulley, when a clump of material fell onto his foot, spraining his ankle.
Using sledge hammer, swung and missed piece of angle welded to shaft on screw for 4SC29. Pinched right middle finger between handle of sledge and the angle iron that was sticking up.
Employee was attempting to un-hook a com-a-long from a chute and cut his arm on the chute.

EE WAS HELPING REPLACE SCREENS, ATTEMPTING TO REMOVE A WEDGE WITH A 4LB. SLEDGE HAMMER IN A HARD TO REACH LOCATION. HE INADVERTANTLY HIT THE RUBBER DONUT. THE HAMMER BOUNCED BACK & HIT HIS FIN GER AGAINST THE SHAKER SCREEN BASE.
EE WAS TRIMMING SCREEN PANEL TO FIT. KNIFE SLIPPED AND CUT LEG ABOVE RIGHT KNEE.
EE WAS POKING DOWN WHEN DUST CAME THROUGH THE SAFETY EQUIPMENT INTO LEFT EYE.
