Mostrando los 17Employee attempted to unclog bin with shovel. As he poked up into the bin his hand got caught on the flow switch rod hanging at the bottom of the bin causing a puncture wound between thumb and pointer finger.

EE HAD A HARNESS & LANYARD ON BUT VIOLATED THE COMPANY CONFINED SPACE POLICY.
WHILE TRYING TO SPREAD THE SIDES OF A 30' X 60" GRIZZLEY FEEDER WITH A HYDRAULIC JACK AND PIPE SPREADER, THE JACK AND PIPE SPRUNG OUT TOWARDS EE STRIKING HIM ON THE LEFT JAW.
