Employee was scraping out buildup in 28D-1 chute when felt a pop in shoulder. Employee seen by nurse. Employee returned back to work. Continued medical surveillance. Surgery to shoulder on 1-24-18.
Warehouse ee was cutting cardboard, at end of shift, with a box cutter so cardboard would fit into compactor when blade of box cutter contacted employees hand resulting in a cut. Employee was taken to UP Health System Bell for medical care. Employee received seven stitches. Employee was returned to work with no restrictions. Gloves were not worn, at time of incident.
Amendment for narrative: Employee was moving two empty railcar's using the rail car puller when something on the rail car puller cable assembly caught on a metal plate. This caused the metal plate to dislodge from nesting place striking employee in legs.
As employee moved a saw downward, the 2nd knuckle on the left hand index finger made contact with blade. Blade cut through leather glove contacting skin. Employee was seen by on-site nurse then taken to clinic where employee received three stitches and returned to work.
While removing a beam trolley, the employee was prying the wheel off with a Sleever bar when it slipped pinching the finger between the bar and the beam. The employee's left ring finger got a laceration and needed ten stitches.
EE was using a dead blow hammer to tap on a Nelson drive cover. While aligning the cover and tapping with the hammer, the right hand holding the hammer contacted a raised edge on the drive cover resulting in a laceration to the right pointer finger. The ee was evaluated by a Physician and 7 sutures closed the wound; the ee returned to work.
EE was cutting a used piece of water hose with a utility knife. The knife slipped and struck the ee in the right forearm. He was evaluated by a Physician and sutures closed the wound on 11-9-14.
The employee was assisting a mechanic with removing a bolt from the dozer with a breaker bar. The bolt came loose suddenly, causing the employee to lose his balance and strain his left arm. He was evaluated by a physician and taken off of work.
A port-a-power was being used to remove a side frame from a shovel, when the ram slipped from it's position and the port-a-power struck the employee in the face. He was transported to the hospital for evaluation, where sutures were administered for facial and scalp lacerations. He was released back to work with restrictions.
Employee alleges back pain from removing spider nuts (approx. 12 lbs.) from the primary crusher. He was evaluated by a physician and taken off of work. He returned to work on 6/24/14 with restrictions
Employee was using a sledge hammer and wrench to tighten bolts. The sledge hammer bounced off the wrench and struck the employee in the left lower leg. He was evaluated by a physician and released with restrictions. The employee did not work his 6/18 shift. He returned to work with restrictions on 6/19. He was released to full duty work on 6/20.
EE was barring a stuck gate on a conveyor feeder why the pry bar slipped. As a result, the ee caught his left thumb between the pry bar & the angle iron brace causing a laceration. The ee was seen by a physician and sutures closed the wound. The employee returned to work.
EE was cutting a hose with a Lenox folding razor knife when the knife slipped and struck the employee in the left bicep causing a laceration. He was evaluated by a Physician and 4 sutures closed the wound. He returned to work.
EE was installing a support shaft wheel on the grate. He was turning a bolt with a wrench from eye level downward when he felt a tightness in his neck and R shoulder. He was evaluated by a Physician, given rx pain medication, and released back to work with restrictions. He took himself off of work 7/19 & 20 due to pain med administration.
EE strained left shoulder while using a 4 ft. prybar to move a large piece of 2 1/4 grating into position so it could be welded down. The employee was evaluated by a Physician and given restrictions and returned to work.
On 8-23-12, the employee was cleaning dirt from a tripper car when his rake twisted, causing a left forearm strain. Employee was evaluated by a Physician and Physical therapy was ordered. He completed his second session on 9-19-12.
Employee allegedly reported pain in his lower back while pulling on a come-along. Employee returned to work and did not seek medical attention at that time. He was evaluated on 4-11-12 and taken off of work by a Physician.
Employee was cutting a belt to be used as a deflector, the knife slipped and he cut his right calf. He was evaluated by a Physician and sutures closed the wound.
Employee reported pain in right wrist after turning pipe wrench while tightening a belt. He has been seen by a Physician and a splint was ordered.
Employee was lining in the Primary Mill. He was using an alignment bar to position the liner in place, the liner popped up and the alignment bar struck the employee on the left side. He was evaluated by a Physician and released back to work. The employee missed one shift due to medication administration. He returned to work after missing one shift.
Employee was using a sledge hammer -slugging down bolts in secondary crusher. He complained of pain in the groin area. He was evaluated by a physician-and on 7/20/11 he was diagnosed with a hernia. Loss time started 7/29/11 due to surgery.
Employee pinched his left ring finger between the bolt driver and the tool holder resulting in sutures to his finger.
Employee was using a hole saw to cut a hole in a metal box when he felt something go in his left eye. He flushed his eye; however, on 5/5/10 complained of an irritation in his eye. He was evaluated by a physician and a foreign body was removed from his eye and eye medication was prescribed.
Employee was tightening bolts with a 1/2" ratchet and felt pain in his right wrist. He was evaluated by a physician on 12/10/09. He was given a splint to wear.
Employee was removing a bolt when the wrench slipped off of the bolt and struck him in the mouth chipping a tooth. He was seen by a dentist for repair on this tooth on 10/06/08.
While shoveling out a launder, employee struck his right elbow against a horizontal floor support. He was evaluated by a physician and on 1-9-09 physical therapy was started.
A rope and pulley system was being used to raise a temporary trap door. An employee pulled on the rope and the pulley ancorage failed. The pulley struck the employee in the face requiring stitches (left side of nose and right side of chin).
Employee was loosening a bolt from a motor mount. The wrench broke free, causing him to strike his hand against a shim. He received a laceration, requiring sutures.
While stripping back the outer jacket on a cable with a knife, the knife slipped, cutting his right index finger. 3 sutures required.
Employee was taking an oil sample on a reducer. He was loosening the plug with a wrench and the plug broke free and he experienced pain in his right shoulder. He was evaluated by a physician and physical therapy started on 8/13/07.
Employee was removing a heater hose fitting. He was cutting the hose off the fitting when the knife slipped and punctured his hand between his thumb and index finger. He received a laceration that required 2 stitches.
While using a 3' pry bar to loosen a screen panel, the panel suddenly came loose and employee's finger was pinched between the pry bar and the panel hold down angle. He received a small cut and bruise of the right 4th finger. On 4/02/07 it was diagnosed as a subtle fracture of the distal tip. No treatment was ordered.
Employee was removing a cutting edge from a 992D. In the process of cleaning the recessed area around the retaining bolt nuts, the small pry bar hit the employee in the mouth causing a chip to his tooth. He had his tooth repaired on 1/22/07.
A one ton chain fell and hit employee in the mouth chipping a tooth. He was treated immediatley with repair done on 10/26/06. This was then reviewed and determined as work related.
While cutting a conveyor belt employee's knife slipped and cut the tip of his left index finger. He required stitches
Working on a suction valve in the booster pump house with a hammer. While pounding he missed and his finger was pinched between the handle and a bolt for the valve. He was treated for a tuft fracture and splinted to protect the area.
Employee was using a 6 ton come-a-long on the transformer change out on grinding line 6. While using the come-a-long, he felt a pop in his right shoulder. Physical therapy started on 12/1/05.
Employee was cutting skirting for a conveyor line when the box cutter slipped, cutting his left thigh. He received 4 stitches.
Using a porta power jack repetitiously to remove bit from the chunkbreaker. The bit was coming out very slowly and it took many pumps to remove it. Employee felt soreness to his left shoulder and neck. He is receiving Physical Therapy
He was positioning an aluminum pick when the pick slipped. He pinched his upper arm on the handrail trying to hold on to it. Lost time started on 2/10/05.
Employee was pounding in a panel retaining sleeve with a ball peen hammer. When he struck it with the hammer, the hammer slipped out of his hand and bounced up hitting his tooth. The tooth was broken off and required dental restoration.
WORKING AT PRECIPITATOR BOTTOM AT FALSE FLOOR IN HOPPER, JACKING FRAME TO ALIGN BOLT HOLES. HIGH VOLTAGE "RIGITRODES" FRAME CORNER LET LOOSE AND THE TEMPORARY SUPPORT MEMBER BROKE. THE FRAME CORNER CAME DOWN, STRIKING HARD HAT ON EE.
EMPLOYEE WAS WORKING ON CHANGING THE DRIVE MECHANISM ON GRINDING LINE 8 SIMPLICITY SCREEN. HE WAS REMOVING A LEAKING HOSE AND HAD TO CUT A PLASTIC WRAP WHICH HELD THE POWER CORD TO THE HOSE. HE USED A UTILITY KNIFE TO CUT IT. THE KNIFE SLIPPED AND CUT HIS FINGER AND THUMB RESULTING IN A TOTAL OF SEVEN SUTURES.
EE WAS OPERATING A DOZER IN THE ORE BARN. THE RIGHT SIDE COVE ON THE ENGINE CAME LOOSE AND STARTED TO HIT THE RIGHT TRACK. HE STOPPED TO ADJUST THE COVER. USED A PICK AS A PRYBAR. HE COMPLAINE D OF BACK PAIN WHILE LIFTING THE PICK. WAS EVALUATED BY A DR. & TAKEN OFF WORK.
EE WAS CONNECTING INTERMEDIATE PROPEL CHAIN.HE WAS HAMMERING A CHAIN PIN & LOCK IN WHEN HE FELT A PAIN TO HIS RIGHT SHOULDER. HE WAS EVALUATED BY A PHYSICIAN & ON 8-5-02 HE STARTED PHYSICAL TH ERAPY.
EMPLOYEE WAS CUTTING A LENGTH OF SKIRTING WITH A BOX CUTTER KNIFE. AS HE WAS PUTTING DOWN PRESSURE ON THE KNIFE AND PULLING IT THROUGH THE SKIRTING THE BLADE SLIPPED AND MADE CONTACT WITH HIS RIGHT LEG CAUSING A LACERATION. HE REQUIRED SUTURES.
TIL 2 NORTH CLASSIFIER-FABRICATING NEW CHUTE ON CLASSIFIER EE WAS CHIPPING DIRT WITH A 4' PRIBAR WHEN HIS RIGHT MIDDLE FINGER CAME IN CONTACT WITH A SHARP PIECE OF METAL. THE METAL RIPPED HIS GLOVE AND CUT HIS FINGER. HE REQUIRED 5 STITCHES.
EE WAS WORKING ON LINING #6 PRIMARY MILL. HE WAS PREPARING TO TAKE A LIFTER BAR OFF OF A PALLET. HE HAD CUT THE STEEL BAND FURTHEST FROM HIM AND WAS CUTTING THE OTHER BAND WHEN THE TOP BAR SLI D OFF AND PINNED HIS RIGHT LOWER LEG. HE RECEIVED SUTURES TO THE LOWER LEG.
EE WAS REMOVING & INSTALLING DSM PANELS ON LINE 1. HE HAD A NEW PANEL IN & WAS HOLDING IT WITH HIS LEFT HAND & HAMMERING WITH HIS RIGHT. HE HIT HIS LEFT RING FINGER WITH THE HAMMER. HE WAS EVA LUATED BY A PHYSICIAN ON 2-28-02 & HAS A FRACTURE TO HIS LEFT RING FINGER.
WHILE WORKING IN LINE 4 MOVABLE CHUTE INSTALLINGLINERS, HE LIFTED HIS HEAD UP TO MOVE THE COME-ALONG AND BUMPED HIS HEAD ON THE BEAM. DUST CAME DOWN & ENTERED HIS EYE. EYE MEDICATION ORDERED.
