Employee was attempting to break up compacted material inside extruder hopper. Employee used a large auger part to strike the side of the hopper. Employee smashed employee's finger between the auger part and the hopper.
Employee was removing a drive chain from a gearbox with a pry bar. The pry bar slipped, smashing left index finger between the pry bar and gearbox. Employee's left index finger was fractured.
Employee was trying to remove a bearing from the bearing housing. EE was using a sledge hammer to knock the bearing loose. When EE struck the head of the sledge hammer against the bearing a small piece of metal flew off the bearing and punctured left leg. Employee received one stitch.
Employee was removing the motor connection cover. He had a box knife on top of the connection cover. When he broke loose the cover he cut his left hand with the box knife.
Employee was removing laces on C-10 belt. He was using a flexco punch to remove the laces, as he hit the punch a small metal shard struck him in the left chest.
At 8:45am EE was remodeling the birdhouse (load out shack). He shot his left index finger with a staple gun.
EE was shoveling underneath a conveyor belt. He struck an object embedded in the ground with the shovel. This caused a contusion to his right wrist.
Employee was installing new V-belts on K-3 shuttle drive using a pipe wrench to roll on belts when the pipe wrench slipped smashing his left index finger between the wrench and motor base. Employee suffered an open wound and hematoma.
Co-worker was using a hammer to drive the pin out of the excavator tracks employee was on the other side when the pin came out and struck his arm. This caused a large cut on employee's right forearm. He returned to work the following day.
Employee was installing a bearing on a conveyor head pulley shaft, he slid the bearing onto the shaft and was using a chisel and hammer to adjust the bearing. While hammering on chisel the employee felt something strike his left wrist, when he examined his wrist he could see a piece of metal embedded in his wrist.
Employee was using 56 inch pry bar to move chute on Primary Shaker, the pry bar slipped and employee fell back approximately 3 feet and struck rib cage on middle bar of the hand rail, fracturing ribs 7,8 and9 on his right side, doctor released him to restricted duty.
Employee was cutting a conveyor belt with a knife, the knife slipped and cut his right leg. Employee received four stitches and was return to work.
Employee was removing wiper blade on primary shaker, while loosening bolts he felt pain in his left shoulder and neck, he was examined by a physician and was released to work with restrictions and will follow up with doctor on 4-28-2011.
While installing tracks on a John Deere 750 Excavator, the 20' chain that was hooked from the bucket to the tracks to pull the track onto the track frame broke, striking the employee that was standing approximately five feet from the left track frame on the left forearm, hand and forehead. Employee was examined by physician and released to restricted duty.
REPAIRING PEA GRAVEL BELT WHEN UTILITY KNIFE CUT LEFT KNEE AREA. RETURNED TO WORK WITH NO CLIMBING, FOR ONE WEEK RESTRICTION.
EE WAS TRYING TO LOOSEN A NUT ON A MIRROR HEAD FOR A TRUCK DOOR. THE NUT WAS VERY TIGHT AND WHEN HE APPLIED PRESSURE THE NUT CAME LOOSE (SUDDENLY) CAUSING HIS FINGER TO BE CUT BY THE MIRROR BRACKET. RESULTING IN HAVING TO HAVE 6 STITCHES.
WORKING IN CLOSE QUARTERS WITH HEAVY ITEMS. BEARING & BEARING HOUSING WAS PUSHED ACROSS FLOOR WITH COMEALONG. RAISED & THEN TURNED AND LASHED INTO PLACE.
CLEANING AROUND CRUSHER WITH PRYBAR AND HAMMER. LEFT ARM FROM ELBOW TO MUD SHOULDER BLADE EXPERIENCING PAIN.
EE WAS REMOVING TEETH FROM TRACKHOE BUCKET. HE PLACED A 3/4" NUT IN PIN SLOT TO KNOCK PIN LOOSEWHEN HE STRUCK THE NUT IT SHOT BACK & HIT HIS LEFT HAND CUTTING & BRUISING IT. HE WENT TO DOCTOR & WAS TOLD WHILE IT WAS NOT BROKEN TO KEEP IT ELEVATED FOR A FEW DAYS. HE RETURNED TO WORK ON RESTRICTED DUTY & WILL SEE DOCTOR ON 1/5/01.
EE WAS ALIGNING TRACK PAD WITH COMEALONG AND FELT A SHARP PAIN IN HIS LOWER BACK. EE WENT TO THE DR. ON 9-20-00 AND THE DR. SAID HE HAD PULLED A MUSCLE IN HIS LOWER BACK. EE WAS GIVEN MEDICATI ON AND TOLD TO COME BACK ON 9-22-00.
EE WAS WORKING ON B-30 Y2143. REMOVING WHEEL HUB USING PIPE TO PRY HUB. PIPE SLIPPED CAUSING EE TO HIT LEFT SHOULDER ON FENDER. RESULTED IN FRACTURED CLAVICLE.
