Employee was removing & replacing the bucket teeth on an excavator. Employee cut them with the torch and was using a hammer to break them free when a piece of metal struck the employee in the leg resulting an ER visit to remove the foreign object. Released to work the following day.

Mostrando los 35Employee was attempting to trim a plastic tab on hard hat halo attachment using a pocket knife when the knife slipped and caused a laceration to left thumb below the first joint.
Employee was standing near 10-wheeler boom truck about 12-15" from hoist cables at #2 D/L. Two other employees were using a 8 lb. sledge hammer and RR maul to drive wedge out of hoist socket. A piece of metal sheared from the maul and traveled approx. 12-15" striking employee in back of right calf.
Employee was positioned about 8-10 feet from two other workers that began to remove the shroud from the dragline bucket using a hammer. Upon impact a shard of metal from the shourd contacted employee cutting through his jeans and into his left knee. He felt a burning sensation and noticed his knee was cut. Employee received five stitches to his left knee.
Stripping cable with a knife and cut the thumb on his left hand. 5 sutures required to close the laceration.
Employee was trying to break loose built up material in the urea auger using a hammer. The hammer hit the auger and jammed his right thumb into the head of the hammer. An air chisel should have been used for this job and a chisel and air hoses were readily available in the area. Employee was wearing all the proper PPE.
Employee hit his left thumb with small sledge hit as he was driving a bull pin in a plate dog installing steel panels inside a tank. suffered a laceration and fractured left thumb tip. laceration required sutures to close wound
While Cutting a Tagline with a Knife employee's hand slipped and knife penetrated Right Thigh. Laceration resulted in six stitches.
Employee was closing bottom door on railcar using bar, while pushing on bar, it slipped out of the hole on door closing mechanism. Employee felt sharp pain in right side of lower back.
Employee stated that while he was mopping the floor inside the house at #3 dragline, he felt discomfort in his lower back.
Employee was using a hammer wrench when a bolt broke free and hit the employee's pinky finger against an adjacent nut resulting in a laceration requiring 4 stitches and a fracture to the tip of the finger.
Employee was using a knocker wrench to tighten the bolts on #1 sandtailings. When he hit the knocker wrench with a sledge hammer, the wrench came off and hit him in the mouth.
Employee was using a belt knife to cut oil cloth. Knife slipped and lacerated left indext finger. On-site nurse practitioner placed 6 sutures.
Employee states he was trying to drive out cut off bolts using ball ping hammer. Does not know what caused injury but looked down and noticed blood on left lower arm. Applied pressure and was brought to first aid. Sent to New Bern Family Practice. X-ray revealed foreign body in left forearm. Employee was given prescription medication and 2 days restricted duty.
Employee was assisting with a vulcanized conveyor belt splice using a Hyde hook blade knife to remove excess rubber from the belts internal cables. While pulling the knife toward his body with his right hand the blade tip slipped off the cable and came in contact with his left forearm about 4" above the wrist causing a 3.25" laceration which required 6 sutures to close.
Employee was changing out a lower cone on a secondard cyclone. Employee was tightening up bolt in flange when the rachet slipped off of bolt and stuck operator in the mouth (lip) causing a 1/4 inch laceration.
Using knife cutting a strap off of a water hose; as strap broke lose, cut self with knife.
Employee was replacing a damaged hyraulic line on a D8R tractor. Employee had to lie on his stomach to access the connection of the line. He then had to reach as far as possible to get his wrench on the bolt heads. As he went to loosen the bolts, his wrench slipped off the bolt causing a sudden jerk, straining his neck. Employee recently began physical therapy for treatment.
Employee was replacing an internediate cable on the 8200 Dragline. The task involved swinging a 20lb hammer, pulling on 3-ton com-a-longs, and walking up and down the mast of the dragline. While performing this task, his back started feeling stiff. Woke up the next morning with back pain.
CUSTOMER TRUCK DRIVER WAS REMOVING SEAL FROM HIS TRAP CRANK SO HE COULD ADD MORE PRODUCT TO HIS LOAD BECAUSE HE WAS UNDER WEIGHT. HE HELD THE SEAL WITH RIGHT HAND & CUT THE SEAL WITH POCKET KNIFE USING LEFT HAND. AFTER THE SEAL WAS CUT HIS HAND KEPT GOING & HE CUT THE TOP OF HIS RIGHT FOREARM.
While greasing H-link on backhoe, two grease fittings would not accept grease. Mechanic removed glove to adjust grease fitting. Without putting glove back on, mechanic began greasing fittings. The first fitting accepted grease. While attempting to grease the second fitting, the grease line leaked, injecting a small amount of grease into his right index finger.
EE WAS PUTTING FLANGES ON 18" DIA. X 50' LONG USED PIPE. ROLLING PIPE TO GAIN ACCESS TO OPPOSITE SIDE OF FLANGE, USING A 40"-LONG BAR WITH PEGS. PIPE WAS CURVED FROM PREVIOUS USE AND SHIFTED DURING ROTATION. PIPE JUMPED WOODEN WEDGES (USED TO STOP PIPE), BROKE PEG OFF OF PIPE ROLLING BAR AND FORCED EE'S RIGHT HAND BETWEEN BAR & FLANGE ON NEXT PIECE OF PIPE. STITCHES REQ.
While adjusting seat in 8200 dragline, noticed cable on seat adjustment was broken. While tightening a screw on the cable the nut slipped in wrench and screwdriver went into top of right thumb.
Employee was taking a jib pin wrench out of rack. The jib pin wrench rotated and jammed right hand against crane body mashing right ring finger. (had gloves on)
EMPLOYEE WAS USING A HAWKBILL KNIFE WORKING ON GROUNDING STICK. CUT HIMSELF AT THE BASE OF HIS LEFT THUMB.
HOLDING ONTO PRYBAR, ROLLING OVER A COUPLING AND COME-A-LONG THAT WAS CONNECTED TO A STUMP. A PIECE OF THE STUMP BROKE OFF CAUSING THE COUPLING TO ROLL BACK. LEFT SHOULDER AND PRYBAR CAUGHT IN FORWARD MOTION.
WHILE SPLICING A BELT. EE WAS KNEELING DOWN ON BOTH KNEES, CUTTING THE BELT. HE WAS HOLDING THE KNIFE WITH BOTH HANDS AS HE CUT, WHEN BLADE SLIPPED OFF OF BELT AND CUT INTO HIS RIGHT CALF. 5 INSIDE AND 7 OUTSIDE STITCHES WERE REQUIRED TOCLOSE THE WOUND.
EE WAS SPLICING A BELT & WAS KNEELING TO CUT THE BELT. THE FORCE OF PULLING CAUSED THE KNIFE TO GO THROUGH THE BELT & CUT LEFT KNEE.
CUTTING A PIECE OF ROPE OUT OF TOP ROLLER ON D10 DOZER. CUTTING WITH LEFT HAND IN AWKWARD POSITION. CUT HIS RIGHT THUMB.
EE WAS LOOSENING A BOLT AND IT TIGHTENED UP ON HIM. THE HEAD OF THE BOLT BROKE OFF AND ELBOW HIT A PIECE OF PLATE ON THE ROLLER STAND.
WHILE USING A HOOK BLADE KNIFE TO CUT BELT, HOOK BROKE OFF OF BLADE. WHEN IT DID, THE MOMENTUM CAUSED THE KNIFE TO COME IN CONTACT WITH RIGHT THIGH. 1-1 1/2" LACERATION, 3" ABOVE KNEE.
EMPLOYEE HAD BOTH HANDS ON A 4 LB HAMMER STRIKING A PIECE OF FLATBAR IN A VICE. HAMMER GLANCED OFF OF VICE AND HIT FINGER BETWEEN HAMMER HANDLE AND FLAT BAR.
A ROCK FELL WRONG IN JAW CRUSHER. EE HIT ROCK WITH SLEDGE HAMMER AND HANDLE SHATTERED INSIDE OF HAMMER HEAD LETTING IT GO BY THE ROCK, STRIKING THE HANDLE AGAINST FINGER AGAINST CRUSHER FRAME .
EE HAD THE TASK OF REMOVING PARTS FROM A 3406 CAT D8N ENGINE. IN REMOVING PART FROM THE CYLINDER HEAD, EE HAD A PROBLEM WITH THE BOLTS SECURING THE TURBO MOUNT TO EXHAUST MANIFOLD. THE NUTS WE RE NOT HEAT RESISTANT AND HAD ROUNDED OFF. EE MADE THE DECISION TO USE A CHISEL. EE HAD ALMOST FINISHED THE CUTTING, WHEN HE MISSED CAUSING THE CHISEL TO HIT HIS L THUMB AGAINST MANIFOLD, CUT
CUTTING WITH A BELT KNIFE, CUT THROUGH GLOVE INTO FINGERS.
