Employee was using a porta-power to spread the arms on a 966F loader to replace the bucket attachment. After releasing the pressure, the porta-power became wedged beween the dog bone and the lift arms. Employee used a sledge hammer to try and dislodge the porta-power. Upon striking the porta-power, it dislodged, traveled upward and struck him in the chest

WHILE EMPLOYEE WAS HITTING A DOZER TRACK, A METAL SHAVING FLEW OFF THE TRACK AND ENTERED INTO THE EMPLOYEE'S CHEST.

WHILE HAMMERING WITH A 3LB HAMMER EE MISSED THE WEDGE AND STRUCK HIS AGAINST A WIRE SCREEN CAUSING THE WIRE SCREEN TO LACERATE HIS FINGER.

EE WAS CUTTING WIPERS FOR HEAD PULLEY WHEN THE CUTTER SLIPPED AND CUTH THE FLESHY AREA BELOW THE THUMB. HE WENT IN FOR STITCHES AND THE ACCIDENT WAS TREATED AS FIRST AID. BY CONTINUING TO USE IT AT WORK AND DIRT BIKES IT WAS NOT HEALING RIGHT WHEN HE WENT IN TO HAVE STITCHES REMOVE. THE DR REQUESTED HE TAKE THREE DAYS OFF AND COME BACK ON FRIDAY.

EE WAS INSIDE THE BELLY PAN CLEARING MAT'L FROM THE WALLS, WHEN A LARGE PIECE OF MAT'L BROKE LOOSE FROM THE SOUTH WALL & HIT THE LADDER HE WAS STANDING ON. HE FELL FROM THE LADDER & SOME MATER IAL HIT HIS FACE.

ATTEMPTING TO RAISE FEEDER PANEL WITH A COME ALONG MATERIAL SLID INTO FEEDER CAUSING EE TO FALL AMDN MATERIAL COVERED HIS LEG.

MOTOR SLID OFF PRYBAR & LANDED ON FINGER.
