Employee was stripping electrical wire using a box knife with a motion towards self and stabbed self in the right thigh. This resulted in a one inch laceration the depth of the blade, requiring three stitches and tetanus shot.

Miner was installing a bearing set beam in the shaft at the 1-5 level, miner was using a spud wrench to align the beam when the wrench slipped out of the beam and caught a fingertip between the beam and wrench.

Employee was using a fusion welder, pipe rolled causing the clamp to pinch the right thumb

Pulling on ratchet. Ratchet and socket slipped and hit Rt. upper lip causing a cut that required sutures.

Worker was cutting a rubber hose with a razor knife when the knife slipped he cut himself on the inner left thigh requiring three (3) stitches.

EE STRUCK THE HUB OF A SCRUBBER FAN WITH A 16" SLEDGE HAMMER & FELT A SEVERE PAIN IN HIS BACK

WHILE REPAIRING A FEED AUGER HE SWUNG A HAMMER TO KNOCK OUT A PIN. HE SMASHED HIS FINGER BETWEEN THE HANDLE & THE AUGER SUPPORT

EMPLOYEE WAS ATTEMPTING TO CUT METAL BANDS THAT HOLD DRUMS TOGETHER DURING TRANSPORT. THE TIN SNIPS WERE DULL, SO IT REQUIRED SOME EFFORT. WHEN THE BAND FINALLY BROKE, THE END OF BAND WHIPPED AND CUT EMPLOYEE'S ARM.
