Mostrando 50 de 89Employee was breaking up a hardened caustic chunk so that it would fit into a hopper when EE smashed finger against a solid object. This resulted in a fracture to the tip of the pinky finger on the right hand.
Employee was stripping electrical wire using a box knife with a motion towards self and stabbed self in the right thigh. This resulted in a one inch laceration the depth of the blade, requiring three stitches and tetanus shot.
The employees were working on a 150 lbs. head tank frame that was sitting on the walkway grating. As they went to move the frame, EE #1 noticed welding rods and attempted to move them as EE #2 moved the frame and pinched EE #1's middle finger, right hand, between the frame and the grating. As a result the fleshy end of the finger was cut requiring stitches.
I tripped over the fork of a Telle Handler while flying a drone.
A miner was using an angle grinder to cut conduit when the miner reached around the conduit to cut the back side the grinder bound up and bounced towards the miner striking left forearm causing the injury. The miner received 6 stitches and a tetanus shot. There was no time lost and no work restrictions. The guard was in place at the time of the accident.
Miner was installing a bearing set beam in the shaft at the 1-5 level, miner was using a spud wrench to align the beam when the wrench slipped out of the beam and caught a fingertip between the beam and wrench.
While hoist was being inspected and ran thru its pre shift checks it had a brake failure and lost the sinking bucket 100 to 150 feet down the shaft with the hoisting cable.
While drilling a face round with a jack leg drill, miner drilled into a vertical exploration hole from the surface, the hole was full of water and the pressure of 1300' of water forced the drill back into the miner chipping tooth.
Miner was unloading materials onto work deck in shaft from conveyance. Two beams were lying on the deck, miner was straddling the beams. When EE unhooked the rigging, the beams tipped and hit the miners foot.
The employee was walking on an icy surface and EE's feet went out from under EE causing EE to land on EE's bottom. The employee dropped EE's right arm to soften the fall and injured EE's shoulder. The employee has seen several doctors since Feb. 16 and now will have surgery.
Drilling Boulders with sinking hammer to be shot miss stepped on small 5 inch rock lost balance and fell to ground, tried to stop fall w/ left arm and fractured left radius bone
While bolting a rib with a jackleg drill the miner twisted his knee while pulling the drill out of drill hole. Third Dr visit showed a torn ACL on 11/11/14
Miner was helping another miner bolting a rib with a jack leg drill and when pulling drill out of hole he placed his finger in pinch point between leg and drill
Miner was lifting half of a 150 lb. deck grate and suffered a hernia on right side
The employee was using a sand blaster when the breather hose got stuck. The employee pulled on the hose slipped on the grit under his feet and lost his balance and lost control of the nozzle. Sand blasting grit struck the employee above the right elbow causing a 4 to 5 inch hole in is bicep.
The employee was lifting one end of a 250 pound cutting edge when he felt a sharp pain in the lower right side of his stomach. The pain went away but a report was filled out. He was later diagnosed with a hernia when the pain returned. On 10/4 he was put on light duty. On 10/18 he had surgery returning to light duty on 10/21.
Miner was drilling a hole in the rib and after completion of the hole leaned the drill against the rib and had his hand in a pinch point between the dog nut of the steel and the chuck of the drill.
While setting steel, a 10' piece of 3/8" angle iron fell three feet vertically onto the miners right foot. The steel punctured the miners right boot and also the skin on his foot. His status is Full Restriction, stay off feet and keep foot elevated. Possible small fracture and infection.
Miner was drilling a raise round when his chuck tender broke on his jackleg drill causing the drill to come off the drill steel and the steel fell out of the hole striking his finger breaking it.
This is an injury or illness from repetition of work duties, not related to one instance, symptoms are numbness in hands and tingling of neck.
Miner was hanging utilities and slipped on wet ground, falling and landing on a jagged rock with his right forearm, causing a cut near the elbow, requiring 4 stitches.
2 Miners were lowering a jackleg drill down from a raise, the drill slipped from their grasp and 1 miners hand was caught between the drill and a piece of wooden lagging.
The employee was removing a cutting edge of a loader bucket using an impact wrench. The weight of the impact wrench pushed the spinning edge of the bolt out of the recessed hole of the bucket the bolt cut through the employees gloves and the under side of his wrist and thumb. The employee received several stitches and 10 days of modified duty.
Driller slipped on a small bit of grease and fell straining his knee.
While drilling with a jackleg, the jackleg was extended up faster than its penetration rate and broke off the steel, the hammer drill then struck the face of the Miner
While bolting the back a 6 inch rock came loose from the back and struck the miner on the hand on top of the drill causing a cut that needed 4 sutures.
A miner was kneeling on the ground repairing an airline, when he went to stand up, his leg made a pop and gave out. He went to the hospital for examination.
Electrician was wiring in a starter box when he stepped on a 4 inch rock rolling over his ankle. causing a small bone to break.
The injury report was received by fax on June 16/11. - Late report - The injured was operating an LHD & trying to pick up a large rock. The LHD slipped over the rock causing him to jar his back against the seat. Note: Worker has been off since June 1/11.
Previous shift overflowed a tank with heated solution and soda ash. The crew then removed a set of grates in the sump area to pump the solutions back into the circuit. The crew then changed at shift change and did not barricade the sump area, nor told the oncoming crew about the removed grating.
Employee was cleaning the dust off of the roll bar while driving a 7 ton buggy. At heading 4014 straight, his right hand got caught between the roll bar and the back. Skin was removed from 3 fingers exposing bone. Bones were crushed in the palm of the hand and 3 and a half fingers were removed during surgery. Skin grafting will need to be performed.
EE was bolting back and when he went to collaring the hole a small piece of muck fell striking his left middle finger. EE's left middle finger sustained a laceration and he had to have stitches.
Men were air lancing inside tank to break up solids to drain. Coupler came loose from hose and began whipping, hitting employee on hand.
Blasted drift and was ready for baring down and mucking. Unknown cause of death at this time. The victim was hand scaling the north rib of the left 4500 drift when loose material fell on him.
The injured was dismounting the rock truck when his right knee gave out. He fell to the ground, twisting his knee. He was sent to dr for assessment and nothing was foung. He was later sent for further assessment to an Orthopedic specialist due to knee pain. He was given a knee brace on 5/13/10 and RTW on 5/17/10.
Injured miner was in process of drilling out 8' round. When finished drilling hole he pulled the steel out of the hole and was in the process of pulling the 8' drill steel out of the jackleg. Placed his right hand at saddle and when machine came off the drill steel the machine came down catching right hand between the machine & leg which caused crush injury to his wrist.
Working in the # 2 manway compartments at approximately the 770 foot steel set, he was removing an old piece of lagging. While attempting to remove the wood cribbing from the bottom the top started to tip towards him. He put his hand up to stop it from falling.
Hoistman had shut off the air to the clutch actuator piston to make adj. Miners rang for the cage, hoistman forgot to reopen the valve, which left the clutch disengaged. Hoistman lowered the cage, it started to free fall. He then slammed on the brake causing the cage to stop suddenly. Injuring 1 of the miners. The miner that was injured has a lumbar disc herniation.
EE was rehabilitating back in the 344 west just past the 989 cross cut. He finished drilling a 6' hole with jack leg & had pushed the bolt into the hole; there was a plate 8' matt hanging off the bolt. He felt that the bolt was secure. He turned to grab jack leg when he turned back the bolt fell from the hole cut his lip. The injured went & had the cut cleaned and had 13 stitches.
EE operating truck was turning right out of a heading into the main haulage & got too close to the right rib & when turned the truck back to the left, right corner of the truck bed caught 6" air line breaking it apart. When pipe broke apart it was bent by truck bed. The bent part of the pipe was against pinning him in the seat, breaking his left leg below the knee.
After blasting a round & barring down & then mucking out the round it was found that the lifters had not pulled. While cleaning out the holes to re-shoot them, a rock fell from the back or rib hitting the ground and rolling across the miner's foot breaking it.
Injured & his partner were on a 6 x 6 raft and pulling themselves toward the dewater pump to do the monthly P.M. The partner pulled on the rope used to move them forward when the raft tipped sideways. The injured worker grabbed the wall to support himself and in doing so injured his right hand. The water is 12" deep.
The employee was helping load drill supplies. When he went to pick up a jackleg drill, he stepped on the plate of a split-set bolt assemble and the bolt flipped up striking him in the face below his left eye causing a cut that required medicla treatment.
Moving axle with hand dolly, employee repositioned axle on dolly and lost control of it. Axle fell over and landed on his right foot.
The miner had previously inspected and barred down the heading before drilling. An unseen rock fell from the rib onto the bench and slid down striking the driller in the back. The driller then walked to a nearby worker and they determined that he should go to the clinic for further examination.
The injured employee was working in a warehouse stocking shelves and organizing floor area under another employee who was stocking selves. He was stocking the selves dropped a 4" vic coupling striking the injured on the head.
EE was hooking up a power cable disconnect box, the power had not been locked out another worker was instructed to turn the power on when the pilot wire light came on at load center, power was turned on he was still hooking up the last phase of power, his left wrist came in contact with the live wire causing an electrical shock.
EE was pulling a 40ft piece of plastic pipe when he felt pain in his left knee. He had previously injured this knee on May 13,08 at the topaz mine. He was sent for an assessment and has been refered to a specialist as there was no sign of injury. NOTE: this is a reoccurence of the previous injury, we have the worker on restricted duty until further assessment.
The employee was doing seasonal maintenance on the swamp coolers where the mine employees live. He was leaving the roof to go down the ladder. When the employee put his weight on the ladder, the safety catches released & the employee fell (maximum fall 11'2" from eave of trailer to the ground). Injuries were four fractured ribs and a cut spleen.
