A plant Mechanic employed by Concrete Supply was using a prybar to lift the side of a belt to place a roller and guard. Holding the bar in position their shoulder popped and suffered a shoulder subluxation. Their arm is mobile but very sore. Sent to orthopedic doctor this afternoon to see if more damage was caused.

Employee was removing a stuck bearing race off of a PTO shaft. Employee cut one side off and it freed the rear end of the shaft - it caught one burr of imperfection on the shaft and while hammering the imperfection off a piece of metal flew off and struck employee in leg.

PUSHING FLASHING, BENDING IT BACK WITH A PRYBAR. PRYBAR QUICKLY SNAPPED BACK CAUSING SLIGHT FRACTURE TO RIGHT RING FINGER.
CHANGIN A BEARING ON THE TAIL PULLEY BREAKING LOOSE THE BOLTS WITH A BREAKER BAR. WHEN THE BOLT CAME LOOSE HE BUMPED HIS ELBOW ON THE TAIL PULLEY COVER DOOR.
