Employee was assisting with 518 hoist rope change out. While pushing on prybar to push the rope employee strained EE's L arm.

Chipping stuck copper off plate when misjudged momentum caused L thumb to be struck by hammer. L Thumb was struck between hammer and plate. L Thumb 1/2in by 1/2in laceration. Light bruising. Light swelling.
Pulling on a 4" nylon sling, bent down to pull up and felt a pop on left hip, on 01/23/24 EE was placed on restricted duty and returned to regular duty on 01/25/2024.
Involved employee was using a manual crimper to crimp lugs to a 4/0 wire. While in the process of crimping the employee felt a strain in their abdomen.
The miner was working on a bobcat skid steer. The machine needed a hoist cylinder replaced. Upon attempting to push out a seized pin the port-a-power slipped. When the port-a-power shifted it struck their left ring finger against the hoist frame of machine
Employee was attempting to beat a bent deck back into place with a 8 lb. hammer. EE was swinging the hammer upward when EE missed the deck, and the hammer went through a hole. This action caused EE's right ring finger to be pinched between the hammer handle and deck causing a crushing injury of right ring finger and a one-inch laceration without damage to nail.
Field service tech was utilizing a utility knife to cut a seal. The utility knife slipped, striking left ring finger causing a laceration. Field service tech was wearing proper PPE.
EE was performing housekeeping. EE was chipping a build up of dirt with a pry bar & struck the back of hand against a stairway edge. The edge of the metal stairway cut through EE's leather glove & created a puncture wound on the back of EE's left hand. Sutures were used to close the 1/2 inch wound.
Employee was using a box cutter to open a tape measure package. When the employee tried to cut, the box cutter slipped and cut the employee's left wrist by EE's thumb. The employee got bandaged on site and was taken off site to a medical clinic and EE received 3 sutures.
Employees were repairing the latch bar on the shovel dipper. The employee was prying with a bar to loosen the piece of the bar. The dipper door moved and pinched off the fingers on the employee's left hand.
Employee was removing lip shroud from bucket with a sledge hammer. The GET shattered and a piece of metal passed through the employee's right calf muscle.
Employee was swinging a 15lb sledge hammer to drive in bars and strained a muscle in EE's upper middle back.
Employee was repositioning jack pad with pry bar, heard pop felt pain in left elbow.
Employee was closing the 1 pump valve at the 7 booster to balance flows at the plant. EE went to the 7 Booster to close the valve with a pipe wrench. After the valve was closed, EE walked to check the pressure gauge and felt pain in EE's right shoulder and arm.
Employee was cutting sandbags and the knife employee was using slipped off the material and employee cut left hand.
Employee was hand stripping copper in the stripping station using a bar. The bar slipped and hit the employee in the chin. EE received 9 stitches.
Employee was swinging a double jack to drive out a pin on 324 grader. EE needed to switch to swinging left handed because of the awkwardness of pin. Later in the day EE started to feel pain in EE's lower back. The employee did not report the injury until the next day.
While using a 3 ton come-a-long to remove the left dipper handle pin from the 518 dipper, the pin jumped approximently an inch causing EE to move suddenly and tweek EE's back on the right side. At first EE requested to report the injury only, but later decided to request medical attention. The onsite EMR treated EE.
Employee was using a 6lb hammer to knock mud off of a haul truck and afterwards felt pain in EE's right elbow.
EE was removing GET from shovel bucket with all appropriate PPE. EE struck lip shroud with a hammer, a piece of metal struck EE's right leg below the knee.
Employee felt a pop in the right elbow when driving in a grounding rod for testing. Employee did not seek medical attention until 2/7/2019.
Employee was installing some hoses on the mast of a drill, when they got stuck EE pried on the hoses with a small pry bar to move them and felt pain in EE's right shoulder.
EE was disconnecting electrical harness & connector would not release. EE used screwdriver to release locking pin on connector. Connector broke, allowing screwdriver to slip and puncture EE right hand between thumb and index finger. EE was take to clinic.
Was removing worn liners and placing them in a scrap boat, co-worker tilting the liner with a six foot bar to help remove the cable and pacman, EE reached under the liner to remove the pacman from the cable when the bar slipped from the liner allowing the liner to fall on employee's right hand smashing EE's middle finger under the cable causing lacerated to EE's middle finger.
Employee was removing a 190 float machine. The machine was suspended from the float deck crane, employee was adjusting the machine with a come a long to facilitate removal from the cell. While adjusting the come a long the eye-bolt failed causing the barrel assembly and mounting plate to swing and hit employee's lower left leg.
Employee was cutting a sticker off an electrical cord, when employee drew knife towards left hand and pierced left index finger knuckle. Employee received 1 stitch.
Employee was removing transmission from a truck, when the wrench slipped. This caused ee to strike a bracket and cut hand. EE received 4 stitches.
Employee was shoveling, when the shovel slipped under employee's weight and employee struck hand on a hanging guard. This resulted in a laceration across left hand. The employee received 11 stitches.
Repairman was trimming edge of a rubber panel for a double deck screen. The blade snapped and remainder of the blade went between left thumb and index finger cutting the left hand. The employee received 5 stitches.
Employee cut right knee while cutting a conveyor belt. Employee was pulling the knife towards self while making the cut. No Lost Time, No Restricted duty resulted from this injury. Employee received 3 stitches.
Employee was cutting a 1/2 inch air hose with pocket knife. The knife slipped and cut left forearm. Sutures were required.
Employee was using a bar to align cutting edges on a track dozer when EE heard/felt a pop in EE's elbow followed by pain, 3/11/2016. Referred to specialist, doctor took employee off of work 4/27.
Employee was tightening flange bolts on discharge valve when EE felt pain in EE's lower back.
Employee cut his left hand above base of thumb while trying to remove a splice jacket on a cable.
Employee was removing rocks from a feeder using a specialty tool designed with a hook on the end to dislodge the rocks. While pulling up on a rock, the hook slipped from beneath the rock causing the tool to move upwards, pinching the employees left middle finger between the metal tool handle and the feeder chute.
Employee was cutting a rubber hose off of a water tube for the engine when the utility knife slipped and cut his right leg.
Hammering a piece of metal when a sliver came off and went through glove into left hand, palm.
Employee was pulling down on wrench loosening a hose fitting when the fitting broke loose and the wrench came down cutting his right hand and twisting his thumb. Two sutures required to close wound.
Employee was working on placing the front spindle assembly on a haul truck. He was using a pry bar to turn the assembly when he felt sudden pain in his left shoulder. Employee was taken off work 9/30/2014
Employee was driving a pin out of ripper shank assembly with sledgehammer; when he struck pin and a sliver of metal struck him in his left leg just below the knee. Employee received stitches.
