THE EMPLOYEE WAS IN THE PROCESS OF LOOSENING A 9/16" NUT ON THE CATWALK OF THE CONCRETE SAND CONVEYOR WHEN THE ENERGY AND EFFOR TEXERTED TO BREAK THE NUT LOOSE CAUSED THE EMPLOYEE TO SUSTAINED A CERVICAL STRAIN IN THE NECK AND SHOULDER AREA OF HIS BODY.

THE EMPLOYEE WHO RECEIVED THE REPORTABLE INJURY WAS IN THE PROCESS OF EXTRACTING A PIN HOLDING A STEERING CYLINDER HOUSING ON A CAT 980C LOADER WHEN HIS TAPPING ACTION ON THE SIDE OF THE HOUSI NG WITH A 4LB HAMMER CAUSED THE PIN TO SUDDENLY SHOOT OUT AND STRIKE HIM ON THE TOP OF THE HEAD. THE MECHANIC WAS ATTEMPTING TO DISLODGE A JACK HANDLE FROM ITS PROPER POSITIONING ON A SHOP HOIST WHEN THE JACK HANDLE SUDDENLY BROKE LOOSE,FLYING UP AND STRIKING HIM IN THE FOREHEAD.
