The worker was cleaning a shaft with grinder with a wire wheel when it caught a groove and bounced in to their face causing some abrasions
Employee was cutting a piece of rope with a utility knife and cut the upper part of the thumb on their left hand.
Employee was using chainsaw to cut and remove branches. Chainsaw became ensnared in crook of tree causing the chainsaw to pull backwards sharply. The chainsaw contacted employee's upper left thigh cutting it.
Employee was walking on floating catwalk on conveyor. Employee was observing conveyor belt while walking and became entangled in hose on catwalk and twisted and overexerted his left ankle
2 employees working on the plant one of them was hitting a part with a hammer when it bounced off the part and hit the other employee in the mouth.
Drilling out part using a air reamer. Reamer jammed and spun around causing me to lose balance falling to the floor the reamer dislodged from the reamer bit hitting employee in the head causing a laceration and shoulder contusion
Employee was exiting from his haul truck and missed the last step off the truck and fell to the ground. Weather nor equipment contributed to the accident.
Employees working on screener in stone plant. After de-activating the power supply and locking out, went to remove the "v-belt" from the drive motor and had to use a wrench to brace the belt back from the motor. Tension from the pulley released when the employee completed a full rotation causing the wheel and belt to turn freely catching his finger between the tool and machinery.
Employee was hosing out under the plant. A rock came from the upper level conveyor and hit the employee on the back side of the head, causing a cut from the hard hat which required 8 stitches. He was bend over at the time of impact
Employee was cutting urethane for the plant with a utility knife. Utility knife slipped and he stabbed himself in the thigh.
Employee was exiting the dozer and caught his thumb in the door while closing. Resulted in employee cutting end of thumb and receiving stitches.

The employee was walking on one of our catwalks and struck his head on an overhead pipe. Employee was properly wearing a hard hat at the time of the incident. He continued to work the remaining shift time. The day after the incident the employee complained of a headache and neck pain but remained at work.
On 08/26/09, EE was repairing a dredge bucket when he slipped on a wet, oily surface and fell. He caught himself while falling by grabbing a nearby steel cable, but sustained contusions to his left hip, a swollen right knee, and a tender right arm. It was determined on 11/11/09 that he would need surgery to repair a tear of the distal biceps tendon on his right arm.
Employee was cutting bolts off of screening machine, grinder kicked back and cut the knuckle on left hand.

EE was adjusting the brakes on the rock truck. was pushing wrench with his foot and his knee popped.
EMPLOYEE WAS GREASING DOZER & LAYED THE GREASE GUN DOWN ON BLADE. EE STEPPED ON GREASE GUN & LOST HIS BALANCE. HE FELL & HIT HIS KNEE ON THE BLADE SUPPORT.
EE was stacking hay on the back of the pickup. Another guy was throwing the bales down to him and hit his knee. Knee popped out of place. Went to hospital, may have torn ligaments.
The employee was injured while assisting with the installation of return idlers on a floating conveyor system. He was using his shoulders and back to lift the conveyor belt up out of the way so that new idlers could be mounted to the truss frame. In the process of lifting the heavy belt, he strained his lower back.

THE FOREIGN BODY WAS MOST LIKELY A METAL SHAVING THAT WAS THROWN FROM A HAND HELD GRINDER HE WAS USING. A LOCAL OPTOMETRIST REMOVED THE PARTICLE AND OTHER MATTER FROM HIS EYE USING AN ALGER BRUSH. THE MEDICAL PROCEDURE WAS SUCCESSFUL.
UNLOADING 100 LB STEEL PIN FROM BED OF PICKUP, PIN STARTED TO FALL TRIED TO CATCH IT AND FELT PAIN IN LEFT WRIST.
EE WAS REMOVING PIECES OF STEEL FROM CRUSHER & CAUGHT THUMB ON STEEL. CUT RT. THUMB REQUIRING 5 SUTURES.
EE WAS LOADING STEEL IN BUCKET OF HILIFT AND GOT HIS RIGHT HAND CAUGHT BETWEEN STEEL AND CAT WALK ON PLANT.
EE WAS WORKING ON A MAINTENANCE PROJECT IN THE SHOP WHEN HE SUSTAINED A SLIGHT FRACTURE TO THE BONE IN HIS RIGHT MIDDLE FINGER. THE INJURY WAS NOT REPORTED AT THE TIME OF THE ACCIDENT BECAUSE IT DID NOT APPEAR TO BE SERIOUS. HOWEVER, AN INFECTION DEVELOPED IN THE FINGER THAT REQUIRED MEDICAL ATTENTION. THE FINGER WAS XRAYED AS A PRECAUTION AND AN ANTIBIOTIC WAS PRESCRIBED AS A TREA
EE WAS LIFTING DOOR OFF OF HAZEMAG CRUSHER. HE PULLED MUSCLES IN LOWER STOMACH AREA.
EMPLOYEE WAS TRYING TO PRIME CARBERATOR ON PU WHEN IT BACKFIRED THRU THE CARB., SPLASHING ON HIS BODY, FACE AND HANDS. THE GAS IGNITED BURNING EMPLOYEE.
EE WAS IN THE PROCESS OF DUMPING A LOAD OF "WASTE MATERIAL" FROM HIS EUCLID R30 PIT TRUCK INTO A SMALL LAKE WHEN THE BANK BY THE WATER'S EDGE SUDDENLY BROKE LOOSE SENDING THE TRUCK AND THE DRI VER INTO THE WATER. WHILE EXTRACTING HIMSELF FROM THE TRUCK, EE SUFFERED A LIGAMENTOUS SPRAIN OF THE RIGHT WRIST. A RESULTING INFECTION IS NOW BEING TREATED.
EMPLOYEE HAD BEEN OPERATING A FRONT END LOADER FOR AN EXTENDED PERIOD OF TIME WHEN HE BEGAN TO EXPERIENCE SOME DISCOMFORT INHIS LOWER BACK. WHEN THE PAIN DID NOT SUBSIDE WITHIN A SHORT PERIOD OF TIME HE DECIDED TO SEEK MEDICAL ATTENTION. HIS INITIAL EVALUATION REVEALED A DISLOCATION OF THE SPINE IN THE LOWER BACK.
EMPLOYEE WAS IN THE PROCESS OF CLEANING A BUILD UP OF DIRT FROM AROUND A GUARD ON A PORTABLE CRUSHING PLANT WHEN HE FELL FROM HIS POSITION ON TOP OF A SMALL RETAINING WALL SURROUNDING THE PLAN T. THE AWKWARD FALL RESULTED IN A DISLOCATION OF THE SPINE IN THE LOWER BACK AREA. ALTHOUGH THE INJURY MAY NOT BE SERIOUS, TREATMENTS HAVE BEEN GIVEN TO ALLEVIATE THE PAIN.
EMPLOYEE WAS ATTEMPTING TO REPOSITION A "HANDING" BELT CONVEYOR ON THE SAND PLANT BY PULLING ON AN ATTACHED ROPE; HE STRAINED HIS LOWER BACK AND SOUGHT IMMEDIATE MEDICAL ATTENTION FROM CHIROPR ACTOR FOR SEVERE PAIN. A SPIRAL MANIPULATION WAS ADMINISTERED BY DOCTOR TO RELIEVE PAIN; EMPLOYEE WAS ADVISED TO STAY HOME FROM WORK AT LEAST ONE DAY TO AVOID RE-AGGRAVATION OF INJURY.
EMPLOYEE WAS PULLING CABLE THRU SHEAVE, WHEN CABLE CAME THRU IT HIT HIM ON THE RIGHT ANKLE.
EE WAS WORKING ON HOUGH 90 FRONT END LOADER WHEN HE BEGAN TO EXPERIENCE LOWER BACK PAIN ON THE LEFT SIDE. AFTER SEVERAL HOURS THE PAIN BECAME TO INTENSE FOR HIM TO CONTINUE. HE WENT HOME TO RE ST BEFORE SEEKING MEDICAL ATTENTION FROM A LOCALCHIROPRACTOR. A SPINAL MANIPULATION WAS ADMINISTERED BY THE DR TO HELP ALLEVIATE THE PAIN. HE RETURNED AFTER 1 1/2 DAYS REST. FOLLOW-UP SCHEDUL
WAS HELPING TO REMOVE OLD WIRE FROM OFFICE BUILDING LOST BALANCE ON LADDER HIT HANDRIAL.
EE WAS DIGGING A DITCH WITH A HAND-HELD SHOVEL WHEN HIS LEFT KNEE "POPPED" WHILE ATTEMPTING TO STAND FROM HIS CROUCHED POSITION. THE INJURY RESULTED IN A TORN MENISCUS IN THE LEFT KNEE. TREATM ENTS HAVE BEEN ADMINISTERED AND MEDICATIONS HAVE BEEN PRESCRIBED TO ASSIST THE HEALING PROCESS. THE INJURY WILL MOST LIKELY RESULT IN ORTHOPEDIC SURGERY AND EXTENSIVE THERAPY.
EE WAS WORKING AS A GRP LEADER OF DREDGE ASSEMBLY WHEN HE DISLOCATED HIS RIGHT SHOULDER ON 2/23/00. WHILE ATTEMPTING TO STEADY HIMSELF AFTER LOSING HIS BALANCE. THE INCIDENT WAS REPROTED TO TH IS OFFICE THE FOLLOWING DAY DESPITE THE FACT THAT NO MED TREATMENT WAS SOUGHT AT THAT TIME. ON 3/20/00 EE REINJURED THE SAME SHOULDER WHILE ATTEMPTING TO REACH ABOVE HIS HEAD TO GRAB A BRACKE
EE WAS WORKING ON A PIECE OF STEEL WITH A HAND-HELD GINDER WHEN HE WAS STRUCK INT HE RIGHT EYE WITH A FOREGIN BODY. PRESUMABLY A SHOVING THE INJURED RESULTED IN A SCRATCHED CORNEA WHICH REQUIR ED THE EYE TO BE FLUSHED BY A LOCAL OPTOMETRIST.A PRESCRIBED MEDICATION WAS ALSO ADMINSTERED BY THE EE TO ASSIST THE HEALING PROCESS.
