Injured was using a wrench to tighten a bolt on a crusher. The wrench slipped off and injured's right palm struck the bolt head. The resulting laceration required 6 stitches.

EE WAS REPLACING THE LEFT SPRING ON THE WATER TRUCK (BRAKE) WHEN HE HIT HIS THUMB WITH A HAMMER.
EE WAS PRYING LOOSE ROCK WITH A PRV 13 AR AND THE BAR BROKE CAUSING EE TO LUNG FOREARD COMPLAINING OF SHOULDER PAIN. REPORTED THIS TO SUPERVISOR ON 1-15-03.
